BackgroundParalysis or impairment of vesical detrusor muscle may lead to uroschesis. Up to now, the medicines of effectively treating this have not existed. Many clinical studies and literature show that acupuncture can function more effectively. Acupuncture treating uroschesis usually take the acupoints of Bladder Meridians as the major ones, such as the lumbosacral eight Liaos and Hui Yang,etc. Electro-acupuncturing eight Liaos has some efficacy of exciting detrusor muscle, which may be considered relating to nervous manipulative ganglion and meridians.Clinical research found that deeply acupuncturing Zhong Liao, Ci Liao could effectively treat cauda equina injury, benign prostatic hyperplasia as well as acute and chronic uroschesis caused by surgery. As acupuncturing the second and third posterior sacral foramina3.5with4acupuncture needles of45°angle, needling response of getting an electric shock radiates to perineum. With electric acupuncture apparatus working, the anal sphincter’s slightly contracting and the thigh’s rotating to inside can be observed.Because sacral2nd and3rd nerves distributed over anal sphincter and thigh adductor muscle through lumbosacral plexus muscle branches, it could be reasonably inferred that directly stimulating and manipulating sacral2nd and3rd nervous anterior rami could excite pathologically paralytic detrusor muscle to treat uroschesis.However, whether that directly relates to acupuncture touching the sacral nervous anterior rami and whether the off-meridian points of the same ganlion have the same exciting efficacies are the questions which have not been solved yet.Currently, the mechanism of acupuncture exciting detrusor muscle is not clear.In the past, some researches studied the electric signals of cerebral cortex to explore whether acupuncturing central nervous system’s mechanism had efficacies of manipulating vesical function. They found that acupuncture could increase electroneurographic signals of hypothalamus, cerebral cortex and thought that acupuncture could manipulate vesical functions by changing the central nervous system’s excitement related to urinary bladder.Some researches initially proved that acupuncture could assist urine discharge by recovering the impaired nerves. Other relevant research results revealed that acupuncture could remarkably raise the vesical ATP of acute uroschesis; upgrade the expression level of spinal cord’s BDNF and TrkB; improve detrusor muscle’s microstructure and lower bax’s expression, etc. Current mechanism study only can assume one of the electro-acupuncture’s potentially efficacious mechanisms, and the decentralized research results are in want of the exceptional and integrated/thorough research on vesical detrusor muscle.Objectives1. Expliciting whether electro-acupuncturing the acupoints CI Liao, Zhong Liao can excite detrusor muscle to enhance its contracting;2. Setting up the side acupoint group of Ci Liao/Zhong Liao to explore the efficacious exceptions by exciting vesical detrusor muscle with electro-acupuncturing Ci Liao;3. From the point of view of vesical sensory nerve’s manipulation, exploring deeply electro-acupuncturing Ci Liao, Zhong Liao points to increase detrusor muscle’s contracting as well as the sensory transmission channel manipulating relation, initially expounding its afferent nervous mechanism.4. From the point of view of the pelvic nerves transmitting nervous fibers, deliberating that the deeply electro-acupuncturing Ci Liao, Zhong Liao acupoints excites detrusor muscle’s motional transmission channel; initially clarifying its efferent nervous mechanism.Methods1. Using rats of acute uroschesis as models, urinary bladder’s contracting action with vesical internal pressure, explicating whether deeply electro-acupuncturing Ci Liao, Zhong Liao has the efficacy of exciting detrusor muscle, observing whether deeply electro-acupuncturing Ci Liao, Zhong Liao to enhance detrusor muscle’s contracting action has comparative acupoints’exception compared with electro-acupunctured side acupoint group of Ci Liao, Zhong Liao;2.With cut pelvic nervous efferent ends, and the comparison of regular group, model one and deeply electro-acupunctured Zhong Liao, Ci Liao one, analyzing their relevance and discussing whether deeply electro-acupuncturing Zhong Liao, Ci Liao can excite detrusor muscle by stimulating pelvic nervous sensory fibers with the quota of pelvic nervous electric signals and the change of vesical internal pressure;3. With cut pelvic nervous efferent ends, comparing regular group, model one and deeply electro-acupunctured Zhong Liao, Ci Liao one, deliberating that deeply electro-acupuncturing Zhong Liao, Ci Liao excites detrusor muscle by stimulating pelvic nervous motional fibers with the quota of pelvic nervous electric signals and the change of vesical internal pressure.Major Results and Conclusion1. Acupuncturing Ci Liao, Zhong Liao should reach posterior sacral foramina for approaching or touching sacral nervous root’s anterior branch to achieve the best efficacy.2. Deeply electro-acupuncturing Ci Liao, Zhong Liao reduces unsteady contracting rate by shortening uroschesis time of acute uroschesis rats and lowering vesical basic pressure. That proves Ci Liao, Zhong Liao acupoints can improve vesical detrusor muscle contracting and bring down urinary bladder’s unsteady contracting to effectively manipulate vesical functions.3. Deeply acupuncturing Ci Liao, Zhong Liao’s side points with electric acupuncture apparatus unable to have the efficacy of exciting detrusor muscle reveals the above unique efficacy only limited to Ci Liao, Zhong Liao; electro-acupuncture stimulating the same ganglion’s distribution cannot bring about the same effectiveness.4. Deeply electro-acupuncturing Ci Liao, Zhong Liao can excite the afferent nervous fibers of pelvic nerves, and then excite the reflective transmitting channel of pelvic nerves to promote vesical detrusor muscle to contract and urinate, i.e., electro-acupuncturing "the exciting afferent nervous fibers-spinal cord urinative hub-promoting detrusor muscle contracting" transmission channel.5. Deeply electro-acupuncturing Ci Liao, Zhong Liao can promote vesical detrusor muscle contracting to increase urination by directly exciting the efferent nervous fibers of pelvic nerves, i.e., electro-acupuncturing Ci Liao, Zhong Liao "promoting the efferent nervous fiber’s electric motions of pelvic nerves-improving detrusor muscle contracting" channel.
