Purpose: To assess the effectiveness of Modified Angelicae Sinensis Decoction forSupplementing Blood on preventing from interferon-alpha (IFNa) induced leukopenia andneutropenia in chronic hepatitis B.Methods: A total of 91 treatment-naive patients with liver biopsy proven chronic hepatitis Bwere enrolled in this study from December, 2005 to October, 2006 in Hepatology Unit ofXiamen Hospital of Traditional Chinese Medicine affiliated to Fujian College of TraditionalChinese Medicine. After giving written consensus, patients were divided into group A (n=21)or group B (n=70) by their intentions. Subjects in the group A were treated with IFNa 1bsubcutaneously at a dose of 5 MU thrice weekly and received Modified Angelicae SinensisDecoction for Supplementing Blood by oral one dose daily within the first eight weeks of theIFNa regimen; those in the group B received IFNa therapy alone. White blood cells (WBC),neutrophils (NEUT), lymphocytes (LY), red blood cells (RBC) and blood platelets (Plt) inperipheral blood were measured within one week before treatment start and weekly within thefirst eight weeks of treatment course with Synchron Clinical System CX?7. Leucopenia wasdefined as WBC counts below 4.0×109/L, neutropenia as NEUT counts below 2.0×109/L, andthrombocytopenia as Plt counts below 100×109/L. For statistic analysis, an intention-to-treatanalysis was used in this paper.Results: 1. There were no significant difference between gender, age, time ofHBsAg-positivity lasting, status of HBeAg-ve+/ve-, number of complications in two groupsat baseline. 2. WBC, NEUT, RBC and Plt counts in group A was 4.20 (3.10~8.10)×109/L,1.50(0.90~4.26)×109/L, 4.73 (3.89~5.69)×109 /L and (154.76±2.07)×109/L, respectively, at baseline, and their lowest counts was (4.30±1.06)×109/L (Z=-0.297, p=0.007), 1.40 (0.20~3.60)×109/L (Z=-2.278, p=0.023), (4.61±0.48)×109/L (Z=-0.419, p=0.675), and(132.29±44.12)×109/L (t=-0.968, p=0.336), respectively, during the first eight weeks oftreatment course. WBC, NEUT, RBC and Plt counts in group B was (5.68±1.37)×109/L,3.00(2.00~5.50)×109/L,4.83 (3.67~5.85)×109/L and 171.00(103.00~324.00)×109/L,respectively, at baseline, and their lowest counts was 3.45(1.50~6.70)×109/L (Z=-8.640,p=0.000), 1.50 (0.70~5.00)×109/L (Z=-3.289, p=0.001),(4.62±0.41)×109/L (Z=-5.498,p=0.000), and 128.50 (48.00~284.00)×109/L (Z=-0.968,p=0.336), respectively, during thefirst eight weeks of treatment course. The difference between at baseline and the lowest countwas 1.40 (-0.20~2.30)×109/L for WBC and (1.06±0.61)×109/L for NEUT in group A,respectively, while 1.85 (0.40~5.90)×109/L and 1.55 (0.00~3.80)×109/L in group B,respectively, respectively, Z=-2.697, p=0.007 and Z=-3.289, p=0.001, respectively, by 2independent samples t test. Leucopenia, neutropenia, and thrombocytopenia occurred in47.6%, 57.1%, and 28.6% of patients in Group A, respectively, while in 68.6% (x2=3.068,p=0.080), 91.4% (x2=11.416, p=0.001), and 15.7% (x2=1.013, p=0.314) in Group B,respectively. 3. Kinetics of such the hematological parameters in the first week of treatmentcourse showed a significant decrease in WBC counts (Z=-2.441, p=0.015) in group A, whilein NEUT (Z=-6.902, p=0.000) and Plt (Z=-2.208, p=0.027) besides WBC (Z=-5.535,p=0.000) in group B, respectively.Conclusions: The results of this study indicated that Modified Angelicae Sinensis Decoctionwould help to decrease the severity of interferon-alpha induced leucopenia and neutropenia inchronic hepatitis B.
