Cystitis glandularis(CG) is a condition characterized by proliferative, metaplastic changes in the bladder mucosa, the incidence has been greatly increased recent years owing to the development of endourology, improvement of pathological diagnosis and increasing of chemical carcinogenic agents.only 10 cases of CG were treated from 1973 to 1988 in zhejiang university first affiliated hospital,while 150 cases treated from 1989 to 1999 Bladder carcinoma often occurs precedinglyx subsequently or concurrently with CG in clinical practice, as a result,CG is to some extent regarded as potentially premalignant lesions, but some controversial arguments still exists. P53 gene is an important tumor supressor gene regulating cell cycle and apoptosis,approximately 60% human malignant tumor exists P53 gene mutation; Bcl-2 gene is one of the most important anti-apoptosis oncogene To elucidate the correlation between CG and bladder cancer, and to find some valuable indicators of potentially malignant teridancy CG,we studied the expression of P53 ,Bcl-2 in bladder adenocarcinoma, bladder transitional cell carcinoma(TCC)> simple CG and carcinogenic CG immunohistochemically.Materials and MethodsAll specimens, taken from operation or biopsy ,were 10% formalin-fixed paraffin sections archived in pathological department.Specimens were divided into five groups: group I bladder adenocarcinoma(17 cases); group II bladder TCC(14 cases); groupin simple CG(18 cases); groupIV carcinogenic CG(7 cases);group V CG with bladder TCC (para-TCC CG,9 cases).Bladder adenocarcinoma and TCC were graded based on WHO critrion, staged based on UICC-TNM.The expression of P53 % Bcl-2 protein in all specimens were investigated with method of Envision immunohistochemistry.Computer SSPS(Edition10.0) was applied to analyze the outcoming of investigation statistically.ResultsOf 18 cases of simple CG, 3 cases expressed P53 protein ,positive rate was 16.6%; Bcl-2 protein was expressed in all cases,positive rate was 100%.Of 17 cases of bladder adenocarcinoma, 11 cases expressed P53 protein, positive rate was 64.7%;Bcl-2 protein was less expressed in only 4 cases,positive rate was 23.5%.Of 14 cases of bladder TCC, P53 protein was expressed in all cases, positive rate was 100%; 13 cases expressed Bcl-2 protein , positive rate was 92.8%.Of 7 cases of carcinogenic CG, 5 cases showed expression of P53 protein,positive rate was 71.4%;6 cases showed expression of Bcl-2 protein, positive rate was 85.7%.Of 9 cases of para-TCC CG, 2 cases expressed P53 protein,positiverate was 22.2%;4 cases showed expression of Bcl-2 protein,positive.rate was 44.4%.Among 17 cases of simple CG, only 2 cases demonstrated dysplasia or tissue malformation,the incidence rate was 11.7%;whereas among 7 cases of carcinogenic CG,5 cases demonstrated dysplsia or tissue malformation, the incidence rate was 71.4%.there was statistical significance between two groups(P=0.007).The expression of P53 protein (P=0.008) and Bcl-2 protein (P< 0.001 ) between carcinogenic CG and simple CG were statistically significant.The expression of P53 and Bcl-2 protein were detected in 7 cases of carcinogenic CG and in its successively occurring bladder TCC, respectively, there was no statistical significance between them(P> 0.05).The expression of P53 protein was higher in bladder TCC than in bladder adenocarcinoma than in simple CG,while the expression of Bcl-2 protein was higher in simple CG than in bladder TCC than in bladder adenocarcinoma,there was statistical significance among these three groups (P< 0.001) .By comparison between two groups, there was statistical significance for the expression of P53 protein between bladder adenocarcinoma and simple CG, bladder TCC and simple CG (P all < 0.05) ;no significance existed between bladder adenocarcinoma and bladder TCC.also,there existed statistical significance for the expression of Bcl-2 protein between bladder adenocarcinoma and simple CG (P< 0.01 ), bladder adenocarcinoma and bladder TCC(P< 0.05);though B...
