The preterm neonate is a special group, whether innate or acquired immunity is immature, leading to the concept of an "immunodeficiency of immaturity". Transplacental passage of maternally derived immunoglobulin to the fetus is the main mechanism to protect neonates from infection .According to different structure and function , IgG is divided into four subclasses: IgGl IgG2 IgG3, IgG4 IgG3 IgG4 constitute 6% and4% of IgG separately, IgG4 of 20%-30% healthy adults can not be measured. This study was to observe the changes of serum IgG IgGl and IgG2 levels and clinical effect after using two different dosages of FVTG in preterm neonates.Research I Determination of IgG IgGl and IgG2 levels in pretermneonatesMethods: IgG IgGl and IgG2 levels of 60 preterm neonates were determined within 48h after birth and analysed their characteristics. Results :l.The IgG IgGl and IgG2 levels in preterm neonates were associated with gestational age, the serum IgG IgGl and IgG2 levels of preterm neonates whose gestational age below 35 weeks were lower thanthose above 35 .(I values were :2.67 8.63 8.98, p<0.05).Gestational age below 32 weeks were group I ,33-34 weeks were group II, above 35 weeks were group III. There were only statistical differences in the serum IgG levels between group I and group III, but there were statistical differences in the serum IgGl IgG2 levels among the three groups . 2.No significant relationship were found between the serum IgG IgGl IgG2 levels in preterm neonates and sex AGA/SGA mothers age labor fashion asphyxia and lung diseases .3.Preterm neonates were divided into two groups according to weight 2000g ,and then compared IgGl and IgG2 levels ,it indicated that IgGl and IgG2 levels in preterm neonates whose weight below 2000g were lower than those above 2000g,but there was statistical difference in gestational age between the two groups .4.Preterm neonates whose gestational age were above 35 weeks and below 35 were divided into two groups according to weight again ,there were no remarkable differences between IgGl IgG2 levels and weight except interfering with gestational age .Conclusion : IgG IgGl and IgG2 levels in preterm neonates are associated with gestational age. IgG IgGl and IgG2 levels in preterm neonates whose gestational age below 35 weeks are lower than those above 35 .Research II Effects of using IVIG on the serum IgG IgGl and IgG2levels of preterm neonates and its clinical observation Methods: sixty preterm neonates were randomly divided into 3 groups according to IVIG dosages: group A200mg/kg group B 400mg/kg group C control cases , one dose IVIG was administered within 48h after birth, serum IgG IgGl and IgG2 of each case were measured after 7 days with IVIG used, and then compared the three groups about the duration of antibiotics used the days of patients condition stabilized the duration of hospitalization the calories of feeding on 7th day . Results:1 There were no statistical differences among the three groups in gestational age weight sex AGA/SGA mothers'age labor fashions asphyxia and lung diseases o Group A and group B showed a significant differences in the serum IgG levels before and after IVIG used.2.1nspection of F, It demonstrated that IgG IgGl and IgG2 levels increased after IVIG used. IgG IgGl and IgG2 levels of both group A and group B increased after FVTG used, but there were no statistical differences between group A and group B .3. IgG IgGl and IgG2 increased values after FVIG used were observed and indicated that though gestational age and weight were different, there were no statistical differences between IgG IgGl and IgG2 increased values.There were no remarkable differences between IVIG dosages and IgG2 increased values, but existed statistical differences in those of IgG 1. IgG 1 increased values of group B were higher than those of group A, t value: 4.44, p<0.05.4.
