| Objective:A retrospective study of treatment of 44 cases of intertrochanteric fractures of elderly,to find a better fixation method of the fractures in the hip arthroplasty for intertrochanteric fracture。Method:It is a retrospective study of 44 cases of hip arthroplasty for intertrochanteric fractures of elderly from March 2006 to May 2010. There are 19 men and 25 women, with a mean age of 77.8 years (ranging from 69 to 92 years), including 20 left cases and 23 right cases. All the fractures are unstable according to Evans-Jensen standard. After the follow-up ranges from 4to 32 months (mean 19.3 months).According to the different fixation methods of the fractures, there are 28 cases whose fractures are firmly fixed with wire, and the other 16 cases whose fractures are only reset. Record the operation time, blood transfusions in or after surgery, complications of surgery, healing situation of fractures, hip function after surgery, which is assessed referring to Harris form, and evaluate the two fixation methods. Results:The operation time of the 28 cases whose fractures are firmly fixed is 60-118 minutes, mean 76 minutes; the time on the bed is 4-12 days, mean 7 days; there are two patients have deep vein thrombosis lower extremities; according to Harris form, 11 cases patients'hip function are excellent, 12 cases are good, 4 cases are benign in, 1 case is poor. The operation time of the 16 cases whose fractures are only reset is 50-70 minutes, mean 61 minutes; the time on the bed is 4-13 days, mean 7 days; there is one patient has deep vein thrombosis lower extremities; according to Harris form, 11 cases patients'hip function are excellent, 12 cases are good, 4 cases are benign in, 1 case is poor. The operation time between the two groups has statistic meaning (p=0.026<0.05), the patients whose fracture is only reset is shorter than the one whose fracture is firmly fixed. the time on the bed of he two groups also has statistic meaning (p=0.001<0.05),but has a adverse result with the operation time. Complications of surgery ,hip function after surgery and the time on the bed of the two groups has no statistic meaning (p>0.05),on the basis of the results of the X2 ,t test and t test.Conclusions:1.Arthroplasty shorts the operation time and the time on the bed, so it can decrease the complication and mortality; it is valuable for the old patients with femoral interchanteric fractures.2. We have different fixation methods for the fracture in hip arthroplasty for the old patients with femoral interchanteric fractures, the operation time will be shorter if we only reset the fracture. Complications of surgery, hip function after surgery and the time on the bed of the two groups have no statistic meaning. We should take the most suitable fixation method referencing the actual situation of the patients. |
