Changes Of Serum Scd40l ,tnf-a And Crp Level In Patients With Obstructive Sleep Apnea-hypopnea Syndrome And Comorbided With Coronary Heart Disease

Objective: To investigate the association among inflammation,OSAHS and OSAHS complicating CHD through observing the serum SCD40L ,TNF-a and CRP levels of them.Methods: Based on apnea hypopnea index(AHI)during sleep monitored by polysomnography,55male patients with OSAHS were recruited and devided into a simple OSAHS group (n=40) and an OSAHS complicating CHD group (n=15).Fifteen healthy subjects were slected as a control group.Serum SCD40L,TNF-a and CRP levels were measured by ELISA.Analysis of variance and pearson correlation analysis were used for statistics.Result: Compared with the serum SCD40L ,TNF-a and CRP levels in control group [(3.06±1.40) ng/ml, (9.75±3.12) pg/ml ,(0.61±0.23) mg/l],there were significant increases in the simple OSAHS group [ (6.51±3.29)ng/ml,(20.12±4.23)pg/ml,(1.88±0.89)mg/l ] , and OSAHS complicating CHD [(9.11±3.01)ng/ml, (25.14±4.15)pg/ml(4.21±2.50)mg/l(allp<0.01)].More-over,there were significant differences in serum SCD40L ,TNF-a and CRP levels between two OSAHS groups (p<0.01).The serum SCD40L and CRP levels of two OSAHS groups were positively correlated with AHI[(r=0.611, 0.515 and r=0.845, 0.493) (all p<0.01)], and negatively correlated with SpO2[ (r=-0.622,-0.606 and r=-0.769, -0.565)(all p<0.01)]. Baseline serum SCD40L levels in serious OSAHS (n=15) [(3.91±2.3)pg/ml] change significantly from the level[(7.13±2.16)pg/ml ] after one night of CPAP treatment (p<0.01).Conclusion: The levels of SCD40L ,TNF-a and CRP increased in patients of OSAHS and OSAHS complicating CHD,which can be related to the stages of atherosclerosis,and may be considered as important factors in developing cardiovascular damage in the patients of OSAHS.
