| Objective:To evaluate the post operative effectiveness of fumigation-washing therapy of Erythrinae decoction on early recovery of elbow joint for inter-condylar comminuted fracture of humerus as well as further research on its benefits for clinical practice.Methods:58 cases of inter-condylar comminuted fracture of humerus (AO/ASIF classification:C2, C3 fractures) which included 34 males and 24 females, aged from 21 to 48 years old (average age:34.83 years old)were selected, all of them were hospitalized from March 2009 to January 2011.All the patients were divided randomly into A and B groups with random cards, in A group, patients were treated with Chinese herbal fumigation (n=29), meanwhile B group was taken as control group (n=29).The treatments as followed:Basic treatment:All the patients were treated with pain relief pre-operatively, operation was carried during the early 5 days after admission, then pain relief and prescription of antibiotics were given postoperatively. Drainage tubes were removed in 24-48 hours postoperatively; continuous passive motion(CPM)& active functional exercise was applied as following treatment. If the wound healed well 2 weeks later, then patients were divided into 2 groups by random. In A group, patients were given strictly functional training and fumigation-washing therapy of Erythrinae decoction. While in the control group, patients were treated strictly functional training only. Measures included:1,Pain intensity.2,Effeteness on detumescence.3,Elbow range of motion (ROM).4,A HSS was used to assess elbow function (the standard of elbow scored from American Hospital of Special Surgery):Excellent:90-100; Good:80-89; General:70-79; Poor:60-69; Worst:less than 60 points. Pain intensity and Elbow range of motion (ROM) were recorded respectively on the 1st day before surgery, the 2nd week and the 4th week postoperatively, maximum circumference of the elbow was measured on the 3rd day and the 4th week after operation, then the swelling dissipated index was applied on the 4th week after operation, elbow function was evaluated on the 1stday preoperatively and 4 weeks after operation respectively. All data of 2 groups were calculated by SPSS 16.0 for windows for statistic analysis.Results:1.There is no statistical significance between two groups before surgery (P>0.05).2. Comparison in the groups:There is statistical significance (P<0.05) of Pain intensity and the ROM between 2 group on the 2nd week postoperatively and the 1st day pre-operatively,the same difference (P<0.05) was found in the 4th week after operation and 2 weeks after operation either, on the effeteness on detumescence and the assessment of elbow function,2 groups are statistical significance on the 4th week postoperatively and the 1st day before surgery (P<0.05).3. Comparison among the groups:There were no statistical significance of the Pain intensity and the ROM between tow groups on the 1st day before surgery and the 2"d week postoperatively (P>0.05), however, statistical significance are found on the 4th week after operation (P<0.05). There is statistical significance of the effeteness on detumescence and the assessment of elbow function between tow groups on the 4th week after operation (P<0.05). On The rate of elbow recovery in each group, A group (93.1%) is much better than B group (75.9%). All patients had no drug allergy and other adverse reactions.Conclusion:1.The 29 patients in the Chinese herbal fumigation-washing group have better clinical recovery on Pain intensity, effeteness on detumescence, ROM and Assessment of elbow function than in the control group.2.Compaired with control group,fumigation-washing therapy of Erythrinae decoction has remarkable clinical effect on the elbow functional rehabilitation in the earlier period after surgery on the inter-condylar comminuted fracture of humerus. |
