| Objective:To evaluate nourishing Yin and purging Fire prescription with hormone clinical efficency of primary nephrotic syndrome.Methods:(1)All patients came from the Wuhan Integrated TCM & Western medicine hospital from May 2010 to February 2011 in the nephrosis internal medicine department outpatient service and in hospital case, and these cases were diagnosed with primary nephrosis syndrome. Acording to the inclusion criteria and exclusion criteria,61 cases met the study criteria, And 46 examples had the kidney pathology result material, Minimal Change Disease (MCD) were 22 examples, the (MsPGN) Mesangial Proliferative Glomerulonephritis 13 examples, the Membranes Nephrosis (MN) 4 example, Focal Segmental Glomerularsclerosis (FSGS) 4 examples, Mesangioca illary Glomerulonephritis (MCGN)3 examples, in the course of treatment 3 examples of the western medcine group occurred Serious complications, thus ejected, finally there left 58 examples left.(2)each of the patients were randomly divided into control group and combination of chinese medcine and western medcine group, Control group were given standard hormone therapy that is widely used stages of prednisone 1. Omg/(kg·d) Symptomatic and supportive therapy (including diuretic, lower blood pressure, lipid-lowering anticoagulant, anti-infection, etc.), Integrative Medicine group were based on the same side to nourishing Yin and purging Fire prescription (Amur Corktree Bark, Carapax Amydae, zhimu, unprocessed rehmannia root, Yerbadetajo Herb, Ligustrum lucidum, Tuckahoe, yamaimo) treatment(3)Treatment for 8 weeks.(4)Observed in patients before treatment, after 1 week,2 weeks, 4 weeks,8 weeks of clinical symptoms and signs,24-hour urinary protein, serum albumin, Tradition Chinese Medicine symptom score, concluded effect of nephrotic syndrome nourishing Yin and purging Fire prescription with hormone.(5)Data using statistical package deal SPSS17.0Results:(1)Comparison oftwo groups After 8 weeks treatment, Integrative Medicine and Western medicine treatment group clinical control group were 16 cases (56.3%) & 10 patients (38.5%); Significant effect 6 cases (18.8%) & 3 cases (11.5%); Effective for 6 cases (18.8%) & 4 cases (15.4%); ineffective as in 4 cases (12.5%) & 9 patients (34.6%); Comparison of two groups, Chinese and Western medicine group's total efficiency were93.8%. Western medicine treatment group was 65.4%, they had a significant difference (x2= 4.034, p<0.05). (2)Comparative efficacy of two Chinese,After8 weeks treatment, Section'of Integrative Medicine and Western medicine treatment group were clinically cured 7 cases (21.9%) & 6 cases (23.1%); markedly for the 14 cases (43.8%) & 8 cases (30.5%); for 7 cases (21.9%) and3 (11.5%); ineffective for 4 cases (12.5%)9& patients (34.6%), Comparison of two groups, Chinese and Western medicine group had a higher total efficiency 87.5%, and. Western medicine treatment group's 65.3%, they had a significant difference betewen two groups (x2=4.03, p<0.05).(3)After 2 weeks of treatment, compared two groups after treatment indicators show:Two groups of urinary protein before treatment compared with no difference between groups (p> 0.05), After 2 weeks of Integrative Medicine of urinary protein excretion (2.97±1.77) g/d significantly improved than (p<0.05), The control group hadno significant changed in urine protein (p> 0.05). this showed that the Integrative Medicine group could reduc urinary protein earlierly than the Western group(4)4,8 weeks after treatment, biochemical indexes of the two groups,4 weeks after the treatment group and western medicine with TCM treatment group were urinary protein excretion (1.91±1.57, 3.48±1.71) g/d, Serum albumin, respectively (38.91±4.93,31.48±6.36) g/1, After 8 weeks in both groups before treatment group and western medicine combined with Western medicine urinary protein were (1.04±0.92,2.33±1.37) g/d , Serum albumin, respectively (39.41±3.92,33.18±5.98g) g/1, Were significantly improved (p<0.01), the treatment group than the control group (p<0.05 or p<0.01).(5)This study shows that the two groups before treatment, no significant difference between TCM syndrome score (p> 0.05), Yin Jiang Huo side by combined hormone therapy after 8 weeks of liver and kidney, Qi and Yin Deficiency symptom points (8.00±0.00, 8.50±0.57) was significantly less than the Western treatment group, points (20.00±0.71,18.00±0.70) (p<0.05).Conclusion:(1)This study shows that:Integrative group therapy in primary nephrotic syndrome, both in traditional Chinese medicine efficacy, or efficacy in Western medicine, better than the Western medcine grou.(2)Integrative group can reduce urinary protein occurred earlierthan Western medcine group. |
