Objectives A cross-sectional survey was conducted in coastal area of Fujian Province toinvestigate the current prevalence of high-normal blood pressure (BP), to explore therelationships between high-normal BP and vasoactive peptide Apelin-12, and to facilitateearly intervention of BP progress and to avoid target organ damage.Methods1.A resident group of3344people in coastal area Fujian Province was selectedfor a cross-sectional investigation. Questionnaire, physical examinations and laboratorytests were carried out.2.Cardiovascular risk factors, severity of target organ injury andApelin-12levels were compared in High-normal BP, hypertension and normal BP people.3.Apelin-12levels were compared in high-normal BP with different risk factors.4.Apelin-12levels were classified into Quartile groupings, and covariance was taken tocompared BP difference within four groups.5.multivariate logistic analysis was performedto explore the impact of apelin-12of the high-normal BP.Result(1) The prevalence of high-normal BP was26.32%, the standardized rate of which was29.96%, and the standardized rate of hypertension was30.01%. In the high-normal BPclassification, systolic and diastolic BP both in the high-normal BP range was resulted inthe highest rate of48.1%.(2) About21.59%of people in high-normal BP group had four or more cardiovascularrisk factors,25%three,32.05%two,18.52%one, none2.84%.(3) High-normal BP group had higher levels of age, circumference, body mass index,triglyceride, total cholesterol, low-density lipoprotein, fasting blood glucose, haemoglobinA1C, γ-glutamyl transpeptidase, uric acid levels and inflammation factor of highsensitivity c-reactive protein than normal BP group (P<0.05). Target organ damage factorserum creatinine, uria microalbumin, brachial-ankle pulse wave velocity and centralretinal artery equivalent in high-normal BP group are heavier than that in normal BPgroup, lighter than that in hypertension group (P <0.05).(4) The levels of Apelin-12in high-normal BP group was lower than that in the normal BPgroup, which was higher in hypertension group(P<0.05). The Apelin-12level in the group of high-normal BP accompanied with obesity, diabetes, dyslipidemia, metabolic syndromewas lower than normal BP group with mentioned-above risk factors (P <0.05).(5) Differences of Systolic and diastolic BP of Apelin-12in quartile groups werestatistically significant (P <0.05). Analysis of covariance revealed that systolic BP stillhad statistical difference in quartile groups(P <0.05).(6) Multivariate logistic regression analysis showed that Apelin-12quartile Q1group hadhigh relative risk estimates to be high-normal BP which was group Q2, group Q3andgroup Q41.33-3.55times,1.11-2.94times,1.24-3.19times(P <0.05), respectively.Conclusions(1) High-normal BP incidence of Fujian coastal Xiapu area is lower than the nationalaverage, while the higher prevalence of hypertension in the country.(2) Not only there are one or more cardiovascular risk factors exist in high-normal BP, butalso target organ damages have found.(3) Apelin-12level is negatively correlated with High-normal BP, prompting the newlydiscovered vasoactive peptide Apelin-12involved in the regulation of high-normal BP.
