| Stress ulcer (stress ulcer, SU) of the patients is due to being suffered from severe trauma, critical illness,mental stimulation and intense psychological pressure,other drugs and foods which have the strong stimulating effect on the stomach that can result in inflammatory erosion, shallow table mucosal and acute gastric mucosal lesions which is characterized by mucosal bleeding is. Its onset urgent, if not been controlled actively and effectively,it can easily lead to massive haemorrhage which can cause death. Therefore, the Thought of " preventive treatment of disease" is an application of traditional Chinese medicine, and it has great important on the active prevention for SU, and reducing the incidence and mortality rate of stress ulcer.The concept of traditional Chinese medicine philosophy " preventive treatment of disease " contains prevention before disease onset, preventing disease to exacerbating and the recurrence of the disease after the cure. In recent years, a large quantity of study shows that traditional Chinese medicine has the significant preventive effect for SU, and the application of acupuncture can warm the meridians,regulate qi and blood, and also has a good effect on SU, it can not only promote the healing of ulcers, but also effectively adjust the overall functional status of the body,reducing the recurrence of ulcer. Compared with Chinese herbs, acupuncture can reduce the burden of the gastrointestinal tract, avoid the poor efficacy resulted by malabsorption of herbs. Moreover, common clinical patients with stomach trouble often accompanied with short sleep duration and poor sleep quality situation. Traditional Chinese medicine hold the opinion that if the stomach qi falls it’s function of reconciling and decline, the foul material in the stomach will ascending,in this way,it would disturb the mind and cause insomnia, namely " Stomach Insomnia."In clinical practice, the use of acupuncture can regulate qi-flowing and harmonize stomach as well as improving sleep quality.Therefore,this study based on the traditional Chinese medicine philosophy " preventive treatment of disease ",adopting the pre-acupuncture method to observe the effect of acupuncture in the pathogenesis of stress ulcer and its sleep-related problems.Objective:In this topic,we gave nomal rats pre-acupuncture before modling,then used restraint water-immersion stress method to copy the SU model, after that,the morphological changes of stomach mucosal tissue of those rats were observed.Meanwhile,the changes of sleep time which induced by sodium pentobarbital were compared in each group.We set the Corticotropin-Releasing Hormone which was one of the key factors in hypothalamic pituitary-adrenal (HPA) axis as the main indicator,to discuss the machnism of pre- acupuncture in the pathogenesis of stress ulcer and its sleep-related problems,so as to provide a laboratory evidence for clinical prevention of the stress ulcer.Method:(1) Wistar male rats of clean grade,40ones, conventional breeding,randomly divided into five groups after one week of adaptive feeding.They were respectively being called the control group,model group,and conciliatory stomach group,sedative group and the combination group,there were8rats in each group8.(2)Using RWIS method to copy SU rat model. For gastro-treatment group,acupoints of (CV12)、(ST36),were choosen for needling,and for dormancy group,acupoints of (K6),(BL62) were used, and (ST36),(CV12),(KI6),(BL62) were applied to combined treatment group.(3) Intraperitoneal injection of sodium pentobarbital at the concentration of2%and40mg/kg body weight of rats, to observe and analyze the induced sleep time in rats by the sodium pentobarbital.(4) Adopted the Guth Scoring method to measure the gastric ulcer area of the rats, and calculated the rats’s gastric ulcer index (ulcer index, UI). And sliced gastric mocusal into tissue sections, then used HE staining and observed those rats’s morphological changes of gastric pathology with the light microscope.(5) using radioimmunoassay to check the CRH levels in hypothalamus, pituitary, adrenal and serum in rats of each group.Results:(1)Mucosal morphology:Compared with the control group, Model group mucosal appeared large area of ulceration, bleeding, A large number of mucosal necrosis,shedding; compared with the model group, conciliatory stomach and the combined group had less and small bleeding; the sedative group are almost with the model group.(2)gastric ulcer index score:Compared with the control group, the other four groups were significantly much higher (P<0.01); compared with the model group, conciliatory stomach group was lower with a significant difference (P<0.05), and the combined group significantly reduced (P<0.01); compared with sedative group, conciliatory stomach group and combination groups were decreased (P<0.05).The ulcer inhibition rate for the pre-acupuncture of each group:the conciliatory stomach group was61.59%; the sedative group was33.36%; the combined group was66.84%.(3)sleep duration:Compared with the control group, the sleep duration in model group was significantly shorter (P<0.01); compared with the model group, the conciliatory stomach group and the combined group as well as the sedative group were significantly higher (P <0.01); compared with the sedative group, the conciliatory stomach group and the combined group were much higher (P<0.05). (4)The correlation between gastric ulcer index score and sleep duration:Application of Spearman and Kendall correlation coefficient analysis:Kendall correlation coefficient-0.587, P=0.000<0.001, Spearman correlation coefficient=-0.762, P=0.000<0.001, the results showed that there was negative linear correlation between sleep duration and gastric ulcer index.(5)CRH content:In the hypothalamus:Compared with the control group, the CRH content of the model group was significantly higher (P<0.01); compared with the model group, the conciliatory stomach group, sedative group together with combination group were significantly lower (P <0.01); and the difference between the latter three groups was not statistically significant (P>0.05).In the pituitary:Compared with the control group, the CRH content of the model group was significantly higher (P<0.01); compared with the model group, the conciliatory stomach group and the combination group both were reduced, and there was a significant difference (respectively, P<0.05, P<0.01); compared with the sedative group, the conciliatory stomach group was lower(P<0.05), and the combined group reduced significantly,there was a statistically significant difference(P<0.01).In the adrenal glands:Compared with the control group, the CRH content of the model group was significantly higher (P<0.01); compared with the model group, the sedative group was lower, and the difference was statistically significant (P<0.05).In the serum:Compared with the control group, the CRH content of model group the was significantly higher(P<0.01); compared with the model group, the conciliatory stomach group was decreased (P<0.05), the sedative group and the combination group were significantly lower (P<0.01); compared with the conciliatory stomach group,the combined group was lower, and the difference was statistically significant (P<0.05).Conclusion:Pre-acupuncture CV12、ST36can reduce ulcer area, improve ulcer inhibition rate, protect the gastric mucosa.②After the modeling of the restraint water-immersion stress method for the rats,the induced sleep time by sodium pentobarbital in rats was significantly shortened.③Pre-acupuncture CV12、ST36、K6、BL62can extend the sleep time of the SU rats and improved the rats’ sleep quality.④Pre-acupuncture the four accupoints can regulate the CRH content in the HPA axis and serum.⑤One of the mechanisms of pre-acupuncture for enhancing the rate of ulcer in rats and prolonging rats’s sleep time,was probably by regulating the CRH content in the HPA axis and serum. |
