| Objectives: To investigate the mechanism of acupotomy lysis therapy KOA, assess its efficacy quantifiable and objective by observeing changes of IL-1β and TNF-α content before and after treatment.Methods: 80 KOA patients fitted the standard are randomly divided into treatment group and control group with 40 patients for each group. The treatment group uses the acupotomy lysis to treat by once a week and five times for a course. The control group takes the articular injection of sodium hyaluronate treatment,the frequency and courses like the former. The data in this two groups before and after treatment should be recorded,which included the content of IL-1β and TNF-α,the total integral of symptoms and signs,joint function score and NRS score.Gathering and analyzing the data after 5 weeks by the software of SPSS 17.0.Results: In clinical indicators point of view,the total efficiency of treatment group has a rate of 94.23%,while control group is 82.35%, with a extremely significant difference(p<0.01); comparing the total integral of symptoms and signs,joint function score and NRS score,which within group before and after treatment,the result has a extremely significant difference(p<0.01); comparing above-mentioned index which between treatment and control group after treatment,the two groups have a significant difference(p<0.05);the results illustrate both needle knife and sodium hyaluronate are in good efficacy to treat KOA,but the fomer is much better than the latter in improving symptoms,relieving pain, improving joint function.In laboratory indicators point of view, comparing the content of IL-1β and TNF-α before treatment with those after treatment,it has a extremely significant difference(p<0.01); comparing the above-mentioned content of two groups after treatment,it also has a significant difference(p<0.05).This shows that two groups can both effectively reduce the content of IL-1β and TNF-α in serum of KOA,but the degree of the treatment group decreases more obviously.Conclusions: 1.Acupotomy lysis could obviously ease joint pain, activity limitation and other symptoms.The conclusion further proves the traditional theory of acupotomy that biomechanics imbalance and soft tissue’s adhesion or contracture induce KOA is right;2. Acupotomy lysis could effectively reduce the content of IL-1β and TNF-α in serum of KOA,it shows that IL-1β and TNF-α is closely associated with KOA, the mechanism may be that the acupotomy lysis effect of two kinds of cytokines metabolism, the specific ways need to be further studied;3. Acupotomy lysis can be effective in the treatment of KOA, and has the advantages of simple operation, obvious curative effect and high safety.It also has the characteristics of simple, convenient, cheap and effective. |
