| Objective:1.To establish the peak bone mass density and investigate the changes of the database of Bai Nationality in Dali city Yunnan province,By measuring the calcaneal quantitative ultrasound.2.To investigate the osteoporosis prevalence of Bai nationality and explore the related risk factors.3.To verify whether OSTA system is applicable to the Bai Nationality of osteoporosis self diagnosis and prediction, and provide this people a reference for the diagnosis and prevention of osteoporosis.4.To Determinate osteoporosis and serum markers of bone metabolism of Bai nationality over the age of 40,and investigate its changes.Methods:l.By cluster sampling, Bai nationality Dali city of Yunnan province were collected from 200 patients, including 57 cases of male, female 143 cases,the age was from 23 to 87 (age group by 10 years),200 people is divided into 7 groups. Each group of patients were carried out by questionnaire survey of calcaneal quantitative ultrasound detection, basic sign detection, osteoporosis,statistical analysis of test and survey results.2.By random sampling method, selected 103 Bai nationality people over the age of 40, aged 40-87 years, were divided into 5 groups according to age, serum markers of bone metabolism in 25-Hydroxy Vitamin D3ã€PTH , Osteocalcin(Bone Gla Protein) and Calcitonin (CT) by ELISA method, and analysis the the detection results.Results:1. male and female PBMD appeared in the 20-29 ages.The male SI peak in 20~29 ages; the female peak value of SI appeared in 30-39ages.It began to decline after the PBMD and peak SI ages, female loss of BMDã€SI at the age of 50-59 and 60~69 two ages are most quickly.2.The PROverall of Bai nationality was 9.5%,PRmale was 3.5%;PRfemale was 11.89%.At the age of 70~79,80~89 ages,female had the highest rate,and compared with other age groups had significant difference.A negative correlation appeared between the age,pregnancy times delivery times and the changes of bone mineral density, weight,height and BMD changes were positively correlated;Brittle fracture in the Logistic regression analysis odds ratio of 11.86,was the department of Dali nationality population osteoporosis related importantfactors.3.By Spearman rank correlation analysis:there is a positive correlation between the prevalence rate of OP and OSTA risk level. ROC curve AUC of OSTA level display OSTA level have predictive value for human subjects with osteoporosis.4.The average 25-(OH) Vit D3 of people over the age of 40 was 14.51ng/ml,PTH was 46.50pg/ml,BGP was 8.10ng/ml;PCT was 17.20pg/ml.With the increase of age,25-(OH) Vit D3 content decreased,60-69 age of male and female 25-(OH) Vit D3 content decreased significantly;the content of PTH increased with age, and it seemed negative correlation between 25-(OH) Vit D and PTH content.There was not statistically significant between BMD SI and 25-(OH) Vit D3 content,but compared with the T-values compared with positively correlated. After compared the results of CT,BGP and BMD,SI,age,T-values,we fail to find the statistically significant.Conclusion:1.Bai nationality population PBMD appeared in the 20-29 ages,was same with most parts of the country, but the quantity was lower than the mean plain and coastal areas of low altitude; it gradually lost after the age of PBM, lost of 50-59,60-69 two ages was most, therefore, prevention bone loss should be focused between the ages of 50~69.2.The osteoporosis prevalence of the Bai Nationality in Dali city Yunnan province is lower than the national level, compared with the coastal economic developed area is low; the highest rate of 50~59 years old male patients, the prevalence rate of female was highest in 70-79,80-89 two ages. Therefore,osteoporosis prevention and treatment of complications due to focus on the age of high prevalence rate the respective. A negative correlation appeared between the age,pregnancy times delivery times and the changes of bone mineral density, weight height and changes of BMD were positively correlated; a important factors related to Osteoporotic fracture was a important factors related with osteoporosis for the Bai Nationality in Dali City,we should pay attention to the prevention of osteoporosis after Osteoporotic fracture.3.Correlation between the OSTA system and Bai nationality osteoporosis prevalence was remarkable.OSTA system is a effective tool of the Bai nationality for osteoporosis self detection.4.Serum 25-(OH) Vit D was negatively correlated to content and age (60-69 years of age decreased most rapidly),and positively related with calcaneal T-value; the content of serum PTH is positively related with age,and 25-(OH) Vit D content was negative relationship.there were no significant between serum osteocalcin; Compared with BMD SI T-value, calcitonin(BGP) CT showed no significant relation. Detecting Serum 25-(OH) Vit D and PTH content has a certain guiding significance for the Bai nationality osteoporosis. |
