Clinical Study Of Guishenwan-xiaoyaosan Decoction Combined With Clomiphene Citrate (CC) In Treating The Ovulation Obstacle Infertility Of Polycystic Ovarian Syndrome(PCOS) Due To Kidney Deficiency And Liver Depression Type

Objective TO observe the clinical curative effect of ovulatory disfunction due to PCOS treated by the decoction of Guishenwan-xiaoyaosan combined with CC, and to evaluate clinical effectiveness of Guishenwan-xiaoyaosan preliminarily, also, to explore its mechanism. It would provide objective evidence for clinical Chinese medicine treatment based on syndrome differentiation of PCOS.Methods 75 patients of ovulatory dysfunction due to PCOS met the inclusion criteria as the object of observation, were randomly divided into two groups:37 patients in treatment group (the decoction of Guishenwan-xiaoyaosan combined with CC); 38 patients in the control group(only CC). To observe the symptoms of traditional Chinese medicine, scores of cervical mucus, scores of endometrial thickness and the proportion of type A endometrium in the oviposit day, the rate of the cycle of ovulation, the rate of the total pregnancy, the cycle pregnancy and miscarriage between the two groups of patients after treatment. Evaluate the clinical curative effect of ovulatory disfunction due to PCOS treated by the decoction of Guishenwan-xiaoyaosan combined with CC.Results (1) After three cycles treatment, the treatment group was better than the control group in clinical total effective rate, in improving symptoms related to Traditional Chinese medicine and in TCM symptom score after treatment. The differences were significant (P<0.05). There were no significant differences about the cycle ovulation rate between the two groups. However, the cycle LUFS(Luteinized Unruptured Follicle Syndrome) rate of the treatment group was lower than the control group. There were significant differences between the two groups(P<0.05). There were no significant differences between the two groups about cycle OHSS(Ovarian Hyperstimulation Syndrome) rate. The treatment group was higher than the control group in the rate of the total pregnancy and was lower in the rate of miscarriage. The differences were significant (P<0.05).(2) The treatment group being higher than the control group in the cervical mucus scores, the differences were significant (P<0.05). There were significant differences in the thickness of endometrium in the oviposit day between the two groups (P<0.01). Pregnancy group was better than no pregnancy group in the thickness of endometrium in the oviposit day. There were also significant differences between the two groups in the number of type A endometrium in the oviposit day, which the treatment group was higher than the control group (P<0.05). Cervical mucus score, endometrial thickness and type may be related to pregnancy.Conclusion Through the treatment of ovulatory disfunction due to PCOS, the decoction of Guishenwan-xiaoyaosan combined with CC could improve the clinical symptoms of patients with Kidney Deficiency and Liver Depression Type, improve the cervical mucus, increase the endometrial thickness and the proportion of type A endometrium in the oviposit day, promote the follicular development and maturation, enhance the rate of ovulation and total pregnancy, lessen the rate of LUFS and miscarriage.
