Objective:To explore the diagnostic value of homocysteine(Hcy),CystatinC(Cys C)and two combined with early renal damage in hypertens-ive nephropathy.Mthods : From January 2013 to January 2015,70 patients with essential hypertension in Jiangjin center hospital,50 cases of healthy physical examination.70 patients with hypertension were observed,including 34 males and 36 fe-males,aged between 45-75 years old,the average age was 2.12± 62 ye-ars,and all the subjects had no blood relationship.50 healthy persons se-rved as control group,Of which 25 were male and 25 were female,aged 45-70 years old,the average age was 61 ± 3.32 years old.The includedsubjects were excluded from other cardiovascular diseases,respiratory dis-eases,digestive system diseases,immune system diseases,diabetes,urinary tract infection,urinary tract stones and other diseases which can affect the kidney function,etc.After all the patients were fasting for 12 hoursandabove,the venous blood was extracted from the 5ml in the test tube,andthe serum was detected at room temperature.At low temperature by 4?,centrifugal blood,and leaving the serum,testing Hcy?Serum creatinine(Scr)?Blood urea nitrogen(BUN)? CysC Serum concentration by Automatic bioch-emical analyzer.All included subjects were left for 24 hours urine,Detecti-on of Urine trace albumin(UmAlb)also by Automatic biochemical analyzer.According to patients with 24 hUmAlb negativeand positive,the observationgroup were divided into A group and B group,Testing the difference of S-erum concentration in CysC and Hcy between A group and B group,Cys-C?Hcy?Hcy and CysC combined with Hcy+CysC in the early diagnosis of hypertensive nephropathy positive rate.All data were statistically analyz-ed by using statistical software package SPSS19.0.Results:1.There was no significant difference of composition in gender and age b-etween the two groups in the observation group and the control group(p>0.05);There was also no significant difference of composition in gende-r and age between the two groups in A and B groups(p>0.05).2.There was no significant difference in the Serum concentration of BUNand Scr between the observation group and the control group(p>0.05);theSerum concentration of Hcy and CysC,24 h UmAlb quantitative were differ-ent,and the difference was statistically significant(p<0.05),In the observat-ion group,the Serum concentration of Hcy and CysC,24 hUmAlb quantita-tive were significantly higher.3.There were significant differences in the Serum concentration of CysC and Hcy in A group,B group and control group,and the difference was statistically significant(p<0.05),The Serum concentration of CysC and Hcy were significantly higher in the A group and the B group than in the control group,and the Serum concentration of CysC and Hcy in B group was higher than in A group.4.The positive rate of Hcy,CysC and Hcy+CysC in the early diagnosis ofhypertensive nephropathy was different,and the difference was statisticallysignificant(p<0.05),Among them,the positive detection rate of Hcy washigher than that of CysC,and the positive rate of Hcy+CysC was the highest.Conclusion:In the diagnosis of early renal damage in hypertensive nephr-opathy,the Serum concentration of BUN,Scr two indicators are Non directive significance;the Serum concentration of Hcy and CysC,24 hUmAlbquantitative can be used as diagnostic indicators of early renal damag in pa-tients with hypertension.Among them,the Serum concentration of CysC and Hcy were more sensitive and specific than 24 hUmAlb quantitative,th-e Serum concentration of Hcy compared with the Serum concentration ofCysC in the early stage of renal damage in patients with high blood pre-ssure,the positive rate was higher,the positive rate of the combination of the two was the highest.The study of early renal damage in hypertensivenephropathy can guide the diagnosis and clinical treatment.The occurrenc-e and development of kidney damage received intervention or even parti-al reversal,delaying the arrival of ERSD,improve the quality of life and lif-e length of patients.
