Objective To investigate and analyze the status of occurrence and knowledge, attitude and practice related needle stick injuries among clinical nurses. To explore the intervention effect of failure mode and effects analysis on occurrence and knowledge, attitude and practice of needle stick injuries among clinical nurses.Methods Apply failure mode and effects analysis to analyze potential factors, consequences and preventive measures systematically. Implement procedures reengineering, establish standard procedure and criteria to formulate preventive measures of needle stick injuries based on failure mode and effects analysis. 150 clinical nurses were recruited from General Hospital of Ningxia Medical University by cluster sampling and were randomly divided into control group and intervention group. The control group got routine management of needle stick injuries, while the nurses in the intervention group received preventive measures based on failure mode and effects analysis for six months. The status of occurrence and knowledge, attitude and practice related needle stick injuries among clinical nurses were compared and analyzed before and after intervention. Evaluated the intervention effect of failure mode and effects analysis on precaution of needle stick injuries among clinical nurses.Results 1. A total of 147 clinical nurses including 75 in intervention group and 72 in control group ultimately finished the intervention. Among them 2 cases were male, 145 cases were female, and the average age was(29.06±5.77) years old. 2. The incidence of needle stick injuries among clinical nurses in the past six months was 36.73%. A total of 39 times needle stick injuries, the occurrence frequency was 1.06 one person. Happened in one to two times was 32.65% and 4.08% was happened in three to four times. In the events of needle stick injuries, 53.71% of nurses were injured by needles had been used to patients. But 46.30% of nurses could not make sure whether the patients suffering from infectious diseases. Majority of patients were diagnosed with hepatitis B, accounting for 11.11%. 3. Among nursing procedures, the link of needle stick injuries occur most was sorting out stuff, accounted for 22.08%. 19.48% of needle stick injuries occurred in setting back to the needle cap or ampoule, the process of pulling needle accounted for 10.40%. And the main causes were having other things to deal with immediately(22.08%), absent-minded(15.58%), patient move restlessly(12.99%). 4. The reporting rate of needle stick injuries was lower, only 12.96%. 38.30% of nurses thought “nobody will pay attention to this event”, 29.78% of nurses thought they were too busy and they have no time. 5. The total score of knowledge about needle stick injuries was(11.17±1.42). The correct rate of the item about gloves can decrease infection probability was lowest. The total score of attitude was(45.73±3.99). The item of nurses flinch from needle stick injuries got the lowest score. The total score of practice was(44.39±2.63). The item of nurses bare-handed separate used syringe needle got the lowest score. 6. Before intervention, the general information, the incidence of needle stick injuries in the past six months, the score of knowledge, attitude and practice related needle stick injuries were not statistically significant between the two groups(p>0.05). 7. After intervention, the risk priority number of key step about needle stick injuries in the intervention group was significantly lower than those in the control group(p<0.05). 8. The incidence of needle stick injuries in the intervention group was significantly lower than that in the control group after intervention(p<0.05). 9. Implementing intervention, the scores of knowledge, attitude and practice related needle stick injuries were all significantly higher than those in the control group(p<0.05).Conclusion 1. The incidence of needle stick injuries among clinical nurses was still higher, but reporting rate was in the lower level. It is crucial to improve nurses' precautionary awareness. 2. Needle stick injuries occurred after operation could make great harm to nurses and cause blood-borne infections. Nurses must strictly carry out operation procedures to avoid needle stick injuries. 3. The knowledge, attitude, practice of needle stick injuries among nurses were not in the high level. 4. Apply failure mode and effects analysis to implement procedures reengineering and improve scheme, which is helpful to the continuous improvement of protective measures. 5. Protective measures of needle stick injuries based on failure mode and effects analysis could reduce the incidence of needle stick injuries. 6. Protective measures of needle stick injuries based on failure mode and effects analysis could significantly improve the knowledge, attitude and practice related needle stick injuries among clinical nurses.
