Objective:The aim of this study was to explore the status of knowledge,treatment and control of hypertension in Rugao,Jiangsu Province,and to investigate factors that affect the knowledge,treatment and control of hypertension,so as to provide a scientific evidence for the prevention and control of hypertension.Method:A questionnaire survey and physical examination was conducted among residents aged 18 and above in Rugao city of Jiangsu Province using a cross sectional multistage randomized sampling method.Totally 14037 participants were included in the analysis.Database was created by EpiDATA3.1,and SPSS17.0 was used to collect and analyze the data.Rate or proportion was used to describe for categorical variables,and the?2 test was used to compare the awareness rate,treatment rate and control rate among patients with different demographic characteristics,different living habits and different disease status.The unconditional Logistic regression was used to explore the association between the control rate of hypertension and influence factors.In this study,a two-side significant level was set as 0.05.Results:1.Basic characteristics:The average age of all participants is 56 years old,and males accounted for 45.81%.Individuals with junior high school and below education accounted for 90.67%,the number of married and remarried accounted for 84.48%,farmers accounted for 69.54%,moderate physical laborers accounted for 54.01%,the proportion of smokers was 20.65%,and the proportion of people who drink 1 and more times per week was 17.97%;and the proportion of participants in physical activity was 7.35%.Among the 14037 patients with hypertension,4957 were hypertension,with the prevalence of 35.31%,35.16%in males,and 35.44%in females.With the increase of age,the prevalence of hypertension increased significantly among men and women,however,the prevalence of hypertension in women increased more than men,and the prevalence of women over the age of 50 was significantly higher than that in men.2.The general situation of the patients with hypertension and the rate of treatment:Among 4957 patients with hypertension,the awareness rate of hypertension was 59.51%and the treatment rate was 55.82%.The awareness rate and treatment rate of patients with hypertension gradually increased with age.Patients with primary school education or high school education and above had higher the awareness rate and treatment rate than those with the primary school or junior high school education.The awareness rate(64.49%)and the treatment rate(61.73%)of the divorced and widowed people were higher than the unmarried people.The retired had the highest awareness rate x(70.43%)and treatment rate(68.11%)among different occupational hypertension patients.While patients with the monthly average per capita income of less than 1,300 yuan and more than 3,000 yuan had higher awareness and treatment rates than those with the monthly average per capita income of 1300-3000 yuan.The awareness rate(62.86%)and treatment rate(60.02%)of the crowd with the daily sitting time>4.0 h were higher;In addition,patients who smoked,drank alcohol and participated in less physical exercise had lower awareness and treatment rates of hypertension,and the awareness and treatment rate of hypertension in patients with family history of hypertension were higher than those without hypertension family history.Patients with hypertension who were overweight/obese and with cardiovascular history had higher rates of awareness and treatment.However,there was no significant difference in the awareness rate and treatment rate of hypertension between hypertension patients with normal blood lipid and patients with dyslipidemia.3.Control rate of hypertensive patients who took medication,exercise and diet managements:The control rate of hypertension was 37.62%among 2767 hypertensive patients who took control measures,there was no significant difference in blood pressure control level among patients taking different control measures.4.Control rate of the patients with different demographic characteristics who took measures to control blood pressure:Patients under 45 years have a higher control rate of blood pressure than those aged over 45 years.The control rate of male patients was not significantly different from that of female patients.Patients with primary school education or high school education and above had higher the control rate than those with the primary school or junior high school education.The rate of hypertension control in divorced and widowed patients is higher than that in unmarried patients.The control rate of employees in individual industry,commerce and service industry is relatively higher.Patients with lower income have higher rates of control than those with higher incomes.However,there was no statistically significant difference in blood pressure control rate between patients with family history of hypertension and without family history.5.Control rate of blood pressure control in hypertensive patients with different living habits:There was no significant difference in the control rate of blood pressure control among different smoking,drinking,sitting time,labor intensity and physical exercise status(p>0.05).6.Control rate of blood pressure control in patients with hypertension under different disease states:Patients with overweight/obese,diabetes,dyslipidemia,and coronary heart disease had a lower control rate of hypertension than those without these diseases(p<0.05).7.Factors affecting blood pressure control in patients with hypertension:Demographic characteristics,living habits and disease status were included in the stepwise Logistic regression model.Patients aged greater than 45 years of age had lower blood pressure control than those younger than 45 years,the probablity of blood pressure control in patients with high school and above is 1.471 times as that of patients with junior high school and lower education..The odds of blood pressure control in patients participating physical exercise is 1.388 times as that of those not exercising.Overweight/obese patients had an odds of 0.752 times as patients with BMI less than24 kg/m~2 to achieve blood pressure control.Moreover,patients with diabetes or dyslipidemia had lower odds of blood pressure control than their counterparts.Conclusion:1.The survey results showed that Rugao residents'awareness of hypertension,treatment rates,control rates were higher than the national average.2.Weshould take the overweight/obese patients,smokers,drinkers,and patients not participant in exercise,and patients with diabetes mellitus,dyslipidemia as a key focus of control hypertension.Health education and knowledge of hypertension prevention and control need be carried out for the key population of hypertension prevention and control,in order to improve the awareness,treatment and control rate of hypertension.Measures of drug treatment and non-drug interventions and other measures should be taken to reduce the incidence of cardiovascular and cerebrovascular diseases and death among hypertension.
