Objective:To investigate the relationship between cystatin C(CysC)and carotid atherosclerosis in type 2 diabetes mellitus(T2DM)patients with normal high blood pressure and its predictive value of 10-year cardiovascular risk.Methods:One hundred and ninety-four patients with T2 DM with normal high blood pressure admitted from January 2016 to January 2017 in our hospital were divided into two groups according to their presence or absence of plaque: According to the 10-year cardiovascular risk score,the patients were divided into low risk group(98 cases),intermediate risk group(56 cases)and high risk group(49 cases)(Framinham Cardiovascular Risk Score> 20% 10%-20% were in the middle-risk group and <10%were in the low-risk group).Collecting general clinical data,collecting blood samples and detecting CysC and biochemical indexes.Logistic regression analysis was used to analyze the relationship between CysC and carotid artery Atherosclerosis and 10-year cardiovascular risk.Results:Compared with the non-plaque group(without AS),age,body mass index(BMI),creatinine,cystatin c,total cholesterol,low-density lipoprotein(LDL-C),cardiovascular risk in 10 years were significantly increased.In addition,men in the plaque group accounted for a higher proportion,diabetes duration is relatively longer,the difference was statistically significant(P <0.05);Person correlation analysis showed that carotid intima-media thickness(IMT)and age,BMI,duration of illness,smoking,serum creatinine,cystatin c,total cholesterol,LDL-C,cardiovascular risk at 10 years were positively correlated(r=0.406,0.319,0.202,0.337,0.226,0.442,0.209,0.215,0.445;All P<0.05);logistic regression analysis found that cystatin c was the main factor affecting carotid plaque.The levels of cystatin c in the low,middle,and high risk groups were(0.60±1.34),(0.71±0.09),and(1.23±0.83),respectively.The difference was statistically significant(P<0.05),and the high risk group was significantly higher than the low risk group.Risk and moderate-risk groups(P<0.01);partial correlation analysis showed that cystatin c was positively correlated with cardiovascular risk at 10 years(r=0.210,P<0.05);logistic regression analysis showed that cystatin c For 10 years cardiovascular risk factors.Conclusion:Carotid atherosclerotic plaques in T2 DM patients with normal high blood pressure are closely related to CysC,duration of disease,age,smoking and BMI.CysC is associated with 10-year cardiovascular risk and can be assessed by CysC,duration,age,smoking,BMI to assess atherosclerosis(AS).In addition,a 10-year cardiovascular risk can be predicted from CysC.
