| Objective: Diabetic Nephropathy(Diabetic Nephropathy,DN)optimization of Clinical pathway of traditional Chinese medicine(Clinical Pathway,CP),mainly USES the prospectie, randomized, controlled Clinical research method, the changes before and after hospitalization in patients with CP into standard, preliminary summary on implementation effect and economic benefit analysis and evaluation,in order to further modify and perfect, provides the basis for optimization of traditional Chinese medicine(TCM),the promotion of CP.Methods: From January 2018 to January 2019,70 inpatients in the department of endocrinology and metabolic diseases of the affiliated hospital of changchun university of Chinese medicine who met the research criteria of DN TCM CP were randomly divided into 35 cases in the optimal path treatment group and 35 cases in the conventional path control group by using a randomized, controlled and prospective study method. At the same time,35 cases of discharged patients who met the inclusion criteria in nearly 5 years with the routine treatment program in our department were selected as the reference group after the retrospective study and clinical status assessment.Compare three groups of clinical curative effect, fasting glucose, glycosylated serum protein, urine trace albumin/uric creatinine, urine trace albumin,24 h urine protein, hospitalization days and total hospitalization expenses index, and compared with conventional path optimization path and the treatment group and control group in TCM syndrome integral,TCM symptoms curative effect, life quality score, index of patient satisfaction. Results: The total clinical effective rate(94.29%),microalbumin(148.14 185.40mg/L)and microalbumin/creatinine(4.47 3.77mg/mmol)in the optimized pathway treatment group were all better than those in the conventional pathway control group and the reference control group. The total effective rate of TCM symptoms(97.14%),TCM syndrome score(2.46 1.82 points),quality of life score(129.44 6.67 points)and patient satisfaction rate(91.43%)in the optimized path treatment group were all better than those in theconventional path control group. Conclusion: DN(spleen-kidney Yang deficiency with stasis and toxin syndrome)CP optimization of TCM was significantly better than the conventional control group in terms of total clinical efficacy,TCM symptom efficacy,TCM syndrome score, urinary microalbumin, urinary microalbumin/urinary creatinine, life quality score and patient satisfaction. In terms of total clinical efficacy,ualbumin,ualbumin/creatinine,it is also better than the reference control group, which is worthy of clinical promotion. |
