PurposeThis topic use the puncture and bloodletting at the temple to treat migraine.By controlling the different amount of bloodletting,the Visual Analogue Scale(VAS)will be used to score.At the same time,the changes of blood flow velocity and headache index of Transcranial Doppler(TCD)will be monitored before and after treatment,so as to understand the effect of different amount of bloodletting on pain relief of patients.To explore the relationship between the amount of bleeding and the clinical efficacy,and to provide a basis for better use of pricking blood therapy in the treatment of migraine.MethodsSixty patients who met the inclusion criteria were randomly divided into three groups(group A,B and C)according to different blood volume according to the order of visiting the doctor.Each group had 20 patients.The three groups have the same operation methods.The specific operation methods are as follows: to keep the patient's sitting position relaxed,first massage and pat the affected side of the temple to fill the local blood vessels,then routinely disinfect with 1% iodophor,fix the skin around the acupoint with the left thumb and index finger,so that the temple is fully exposed,right hand three-edged needle is quickly pricked,and then quickly pull out the prick with cupping,according to the requirements of the group.In group A,the amount of bleeding was less than 2ml,in group B,2~5 ml,and in group C,6~10ml,the cupping was removed and hemostasis was compressed.Treatment course: once a week,three weeks for a treatment course.Visual Analogue Scale(VAS)score,mean blood flow velocity change of Transcranial Doppler(TCD)and headache index were observed before and after treatment in each group.The clinical efficiency of each group was counted.Migraine relief and improvement were compared before and after treatment in each group.The relationship between the clinical efficacy of pricking blood and the amount of pricking blood was evaluated.The results 1.General data study: There was no significant difference in age,sex and body mass index between the three groups before treatment(P>0.05),which was comparable.2.Comparison before and after treatment in group A,group B and group C: VAS score,TCD artery blood flow velocity and headache index after treatment in group A,group B and group C were lower than those before treatment,and the difference was significant(P<0.05).3.Comparison before and after treatment: Before treatment,there was no significant difference in VAS score,TCD artery blood flow velocity and headache index between the three groups(P>0.05);after treatment,the indexes of group B were significantly lower than those of group A(P<0.05),and the indexes of group C were significantly lower than those of group B(P<0.05),and the differences were significant(P<0.05),and the indexes of group C were significantly lower than those of group A(P<0.05).Significant difference was found between the two groups(P<0.01).Conclusion 1.Blood pricking therapy can significantly improve the clinical symptoms of migraine patients;2.Within a certain range of bleeding volume(10ml),the amount of bleeding was positively correlated with the clinical efficacy.
