| Objective: 1 Through cross-sectional study on patients after coronary artery stent implantation at the early stage,to identify the status and influencing factors of patients’ resilience and quality of life,to explore the relationship between the knowledge score of coronary heart disease, resilience and quality of life, and to provide evidence basis for nursing intervention of patients after coronary artery stent implantation.2 Combining the findings of the cross-sectional study of resilience and quality of life of patients after coronary artery stent implantation, the telephone follow-up intervention based on empowerment theory was implemented to evaluate the effect of the follow-up intervention and to provide theoretical basis and practical reference for hospital and community medical personnel to improve patients’ resilience and quality of life.Method: 1 Cross-sectional study on the resilience and quality of life of patients after coronary artery stent implantation at the early stage Using cross-sectional study method,a total of 622 patients after coronary artery stent implantation was recruited from internal medicine-cardiovascular department of a grade A tertiary hospital in Xi’an.The general information questionnaire, coronary heart disease related knowledge questionnaire,Connor-Davidson resilience scale(CD-RISC)and 36-item short-form health survey(SF-36)were used as investigation tools to explore the status and influencing factors of resilience and quality of life of patients after coronary artery stent implantation. The relationship between the related knowledge of coronary heart disease, resilience and quality of life was also analyzed.2 Effects of telephone follow-up based on empowerment theory on the resilience and quality of life of patients after coronary artery stent implantation The randomized controlled trial was used in the study.A total of 92 patients after coronary artery stent implantation was recruited from internal medicine-cardiovascular department of a grade A tertiary hospital in Xi’an.They were randomly divided into the control group and the intervention group. The control group performed routine care, whereas the intervention group developed a telephone follow-up program based on empowerment theory while carrying out routine care.Patients were investigated by the coronary heart disease related knowledge questionnaire,Connor-Davidson resilience scale(CD-RISC),and 36-item short-form health survey(SF-36)to compare the effects of the intervention before and after 1month of the intervention.Descriptive analysis,t-test,variance analysis,Mann-Whitney U test, and multiple linear regression were used in the process of data analyzing. Results: 1 Scores of coronary heart disease related knowledge, resilience and quality of life of patients after coronary artery stent implantation at the early stage1.1 Scores and influencing factors of knowledge about coronary heart disease in patients after coronary artery stent implantation at the early stage The total average score of knowledge related to coronary heart disease in patients after coronary artery stent implantation at the early stage was(0.67±0.17).The average score of each dimension ranked from high to low as follows: rehabilitation related knowledge, risk factors, treatment methods, induction factors, examination methods, concepts, clinical manifestations of disease attacks, and drug knowledge.Factors influencing the score of coronary heart disease related knowledge of the patients included educational level, residence, main caregiver, and course of the disease.1.2 Resilience and its influencing factors of patients after coronary artery stent implantation at the early stage The total score of resilience of patients after coronary artery stent implantation at the early stage was(61.38±10.21),which was at a moderate level. The average score of each dimension ranked from high to low as follows: tenacity,self-improvement,and optimism. The factors influencing patients’ resilience included patients’ age, gender, monthly income per person in the family, complications, number of operations, amount of sleep and the knowledge related to coronary heart disease.1.3 Status and its influencing factors of quality of life of patients after coronary artery stent implantation at the early stage The total score of quality of life of patients after coronary artery stent implantation at the early stage was(59.20±6.83),which was at a moderate level. The score indicators of each dimension ranked from high to low as follows: vitality, mental health, physiological functions, overall health, physical pain, social functions, physiological functions, and emotional functions. The factors influencing the quality of life of the patients included patients’ age, gender, complications, number of operations, cardiac function grading, amount of sleep, the knowledge related to coronary heart disease and the resilience.2 Effects of telephone follow-up based on empowerment theory on the resilience and quality of life of patients after coronary artery stent implantation After 1 month telephone follow-up based on empowerment theory for patients after coronary artery stent implantation, the variation of the knowledge related to coronary heart disease and its all subscales’ score of the intervention group were higher than that of the control group.Except for the three dimensions of risk factor, induction factor and rehabilitation related knowledge, the variation of the knowledge related to coronary heart disease and the other subscales’ score were different between the two groups, and the difference was statistically significant(P<0.05).The variation of the resilience and its 3 subscales’ score of the intervention group were higher than those of the control group, and the difference between the two groups was statistically significant(P<0.05).The variation of the quality of life of the intervention group and its overall health, emotional functions and social functions were significantly higher than those of the control group, and the difference was statistically significant(P<0.05).Conclusions: 1 Awareness and influencing factors of knowledge related to coronary heart disease of patients after coronary artery stent implantation at the early stage The knowledge related to coronary heart disease of patients after coronary artery stent implantation at the early stage is at a medium to upper level. The main factors affecting the awareness of knowledge related to coronary heart disease include patients’ educational level, residence, main caregiver, and course of the disease.2 Status and influencing factors of resilience of patients after coronary artery stent implantation at the early stage The resilience of patients after coronary artery stent implantation at the early stage is generally at a moderate level. The main factors influencing patients’ resilience include patients’ age, gender, monthly income per person in the family, complications, number of operations, amount of sleep and the knowledge of coronary heart disease.3 Status and influencing factors of quality of life of patients after coronary artery stent implantation at the early stage The quality of life of patients after coronary artery stent implantation at the early stage is generally at a moderate level. The main factors affecting patients’ quality of life include patients’ age, gender, complications, number of operations, cardiac function grading, amount of sleep, the knowledge of coronary heart disease and the resilience.4 Effects of telephone follow-up based on empowerment theory on the resilience and quality of life of patients after coronary artery stent implantation Telephone follow-up based on empowerment theory can effectively improve the resilience and quality of life of patients after coronary artery stent implantation. This followup mode can provide theoretical basis and practice reference for hospitals and communities to carry out targeted continuing nursing for patients after coronary artery stent implantation. |
