| Depression is a series of clinical symptoms or states with the essence of self-experience of depressed mood, and the main clinical characteristics are continuous and significant depression, which is manifested as insufficient energy, persistent fatigue, reduced activity and loss of interest,or accompanied by various physical symptoms[1].In recent years, depression has become the most common mental disorder in the world[2],which is a human public health problem that seriously consumes social resources.Depression is closely related to many other physical diseases, such as increasing the incidence of diabetes, coronary heart disease, etc.,and is the main cause of hundreds of thousands of suicides worldwide every year[3].At present, the modern medical treatment methods for depression mainly include oral antidepressants, electroconvulsive therapy, psychological therapy, etc.,with many adverse reactions, low tolerance of patients, and susceptibility to addiction[4-5].Studies have shown that acupuncture and moxibustion has a good clinical effect on depression, which is safe and reliable, with obvious advantages of simplicity, efficiency and low cost[6].According to clinical epidemiological investigation, the syndrome of liver depression and spleen deficiency is a more common TCM syndrome type of depression[7].Professor Feng Chen, the tutor, has collected more than 30 years of clinical experience and formed a complete treatment system for depression. The selection of this group of acupoints is simple, curative effect is outstanding, simple operation, suitable for clinical promotion. The purpose of this project is to make a preliminary and objective evaluation of the clinical efficacy of acupuncture "Peitu Kaiyu" group on treating mild and moderate depression of liver-qi depression and spleen deficiency, and summarize professor Feng Chen’s experience,so as to better promote clinical application.1 Research objectiveClinical evaluation was made on the therapeutic effect of acupuncture "Peitu Kaiyu"group on treating mild and moderate depression of liver-qi depression and spleen deficiency, and the clinical application value of this method on treating mild and moderate depression of liver-qi depression and spleen deficiency was discussed.2 Research methodsA prospective randomized control design was used in this study.Patients meeting the inclusion criteria were randomly divided into the experimental group and the con t-rol group. Before the treatment, the Hamilton depression scale(HAMD)and the TCM syndrome scale of depression with liver-qi depression and spleen-deficiency were use dto evaluate the depression of the patients in the two groups. The acupuncture points in the test group were selected as those in the "Peitu Kaiyu" group(ST9,ST36,ST44,PC6,HT7,GB40),while those in the control group were selected as those in the body ac-u puncture group recommended by the TCM diagnosis and treatment plan for 95 diseas esin 22specialties(LR14,LR3,ST40,BL20,ST36,RN22).Three times a week and six weeks aft ercontinuous treatment, the above two scales were used again to evaluate the depressi onof the patients, and the efficacy of the two acupuncture schemes were analyzed and compared,SPSS20.0 software was used for statistics and analysis of all data in the st-udy,chisquare test was performed for counting data,t test or Wilcoxon rank sum test was performed for measurement data, and rank sum test was performed for rank data. When P<0.05 was considered statistically significant.3 Research results3.1 Baseline dataThis study included a total of 72 patients with mild and moderate depression of liver-depression and spleen deficiency treated in the outpatient department of acupuncture and moxibustion,Wangjing Hospital of Chinese academy of traditional Chinese medicine from January 2018 to January 2019,36 in the observation group and 36 in the control group.Among them,1 patient in both the observation group and the control group was excluded due to other treatments,so 35 patients in the final test group and 35 patients in the control group were excluded. There were 30 males and 40 females. There were 50 cases of mild depression and 20 cases of moderate depression.Average age 42.30±12.00;The mean Hamilton depression scale(HAMD)score was 25.39±4.27,and the mean liver depression and spleen deficiency scale score was 32.50±6.29Statistical analysis was performed on the above data, and the test results showed that P>0.05,and there was no statistically significant difference between the two groups in basic information and condition before treatment, indicating that the baseline data of the two groups were comparable.3.2 Clinical efficacy comparison resultsAfter 6 weeks of treatment, the therapeutic effect of the two groups was analyzed from the aspects of Hamilton depression scale(HAMD)and TCM syndrome scale score of depression with liver depression and spleen deficiency.Points about two scale, upon examination,P<0.05,differences in the group comparison results show that the experimental group and control group after treatment of both scale total score and had significant difference before treatment, observation group and control group in the treatment of all patients with mild and moderate depression type change the Hamilton depression rating scale(HAMD)and change the depression improved syndromes rating scale; After the comparison between the two groups,P<0.05 was detected, and the difference in the reduction scores of the two scales between the two groups was statistically significant, indicating that the improvement degree of HAMD and TCM syndrome scale score of depression, liver depression and spleen deficiency in the observation group was more obvious than that in the treatment group. In terms of the overall efficacy of the two groups of patients, the observation group showed 4 significant effects,27 effective cases and 4 ineffective cases, with a total effective rate of 88.57%;the control group showed 1 effective case,22 effective cases and 12 ineffective cases, with a total effective rate of 65.71%.After test,P<0.05,the difference in efficacy between the two groups was statistically significant. Therefore, the efficacy of acupuncture point selection scheme in the experimental group was better than that in the control group.4 ConclusionAcupuncture of "Peitu Kaiyu" group and the guideline recommended group has curative effect on mild and moderate depression of liver-qi depression and spleen deficiency. The clinical efficacy of acupuncture on "Peitu Kaiyu" group for treating mild and moderate depression with liver depression and spleen deficiency was better than that of the acupuncture guideline recommended group. |
