| ObjectiveThrough retrospective analysis,this paper explores the medication rule of Professor Jin Lili in the treatment of hypertension and liver-yang hyperactivity;In order to clarify its effectiveness and safety in the treatment of primary hypertension,we used clinical case-control method to explore the clinical efficacy and adverse effects of Rougan Decoction on patients with hypertension with liver-yang hyperactivity,so as to provide theoretical support for clinical application and promotion.MethodsIn the retrospective study,based on particular inclusion and exclusion criteria,a total of 204 eligible cases were collected from patients who were admitted to the CardiovascularOutpatient Department of the Second Hospital of Guangdong Province from November 2017 to April 2019.Their basic information,Chinese medicine prescription,and other related contents were taken into the Excel software,and then we use the IBM SPSS statistics software 23.0 to make descriptive statistical analysis and clustering analysis.We use IBM SPSS Modeler 18 to make association rule analysis.Both methods are used to analyze Professor JinLiLi’s using drugs regularity on treating hypertension and liver-yang hyperactivity;In the clinical research section,according to some inclusion and exclusion criteria,along with a method of randomized controlled study to select 76 cases of first-class hypertension and syndromes of liver-yang hyperactivity.According to a method of random number table,the patients were divided into the test group and the control group,both groups have 38 patients.The control group was given lifestyle intervention,and the test group was treated with Rougan Decoction(traditional Chinese medicine granules)on the basis of lifestyle intervention of the control group.The clinical effect was observed for 4 weeks,and the blood pressure,symptoms and scale scores were recorded before and after treatment.IBM SPSS statistics 23.0 was used for statistical analysis to investigate the clinical efficacy and adverse effects of Rougan Decoction on patients with hypertension with liver-yang hyperactivity.Results1.Retrospective research part:(1)In 204 eligible cases,Professor Jin Lili used 89 Chinese herbs for treating hypertension and liver-yang hyperactivity,a total of 2098 uses,and among them,74.79%were 11 drugs used more than 30 times.And the drugs used more than 100 times are Gastrodia elata BI,gambir plant,mulberry parasitism,Eucommia,Achyranthes bidentata B1,Radix Paeoniae Alba,poria,spine date seed.(2)A total of 2,268 rules were obtained by correlation analysis of these 89 traditional Chinese herbs,among the more closely related drugs used are as followed:mulberry parasitism,Eucommia,Gastrodia elata Bl,gambir plant,poria,Radix Paeoniae Alba,Achyranthes bidentata Bl,spine date seed;The most correlated medicine suit is gambir plant-Radix Paeoniae Alba.ln third order correlation,the most supportive medicine group is Eucommia-mulberry parasitism-Radix Paeoniae Alba.In forth order correlation,the most supportive medicine group is Eucommia-mulberry parasitism-Gastrodia elata Bl-Radix Paeoniae Alba.(3)Cluster analysis was carried out on 42 drugs used more than 5 times,and a total of 7 groups of drugs were obtained,including 1 group of commonly used prescriptions,which reflected the basic pathogenesis of hypertension and liver-yang hyperactivity and the treatment principle and treatment method of Professor Jin Lili in the treatment of the disease from different angles.2.Clinical research part:(1)Comparison of baseline data between the two groups:Before treatment,the two groups of patients were compared in terms of gender,disease course,age,past history,personal history,blood pressure,TCM syndrome score,hypertension PRO scale score and SF-36 life scale score,etc.There was no significant statistical difference(P>0.05),the two groups were comparable.(2)blood pressure:The levels of systolic and diastolic blood pressure in the experimental group and control group were significantly lower than before,and the comparison between groups showed that compared with the control group,the antihypertensive effect of the test group was better(P<0.01);for the control group and the test group,the total effective rate of the former was 60%,while the later was 85%,there is statistical conspicuousnessl difference between the two groups(P<0.01).(3)TCM symptom score:The scores of TCM syndromes in the two groups were statistically conspicuousness from those before treatment(P<0.01).After treatment,the total effective rate of TCM syndromes in the test group was 85%,while that in the observation group was 71%,compared with the control group,the score of the test group was more significant,and there was statistical conspicuousness between the two groups(P<0.05),and the symptom improvement effect of the test group was more obvious than that of the control group.(4)PRO scale scores of hypertension:Before and after therapy the distinction of PRO scale scores of hypertension between the test group and the control group was statistically conspicuousness(P<0.01).After treatment,the improvement effect of the test group on the score of hypertension PRO scale was more significant than that of the control group,and there was a statistically conspicuousness between the two groups(P<0.01).(5)SF-36 life scale scores:Before and after therapy the distinction of SF-36 life scale scores between the test group and the control group was statistically conspicuousness(P<0.01).After treatment,the improvement effect of the test group on the score of SF-36 life scale was more significant than that of the control group,and there was a statistically,conspicuousness between the two groups(P<0.01).(6)Safety evaluation:Before and after treatment,the two groups of patients had no obvious abnormalities in safety indicators such as blood routine,stool routine,urine routine,electrocardiogram,ALT,AST,BUN,and CRE.Conclusion1.According to the drug association analysis,Professor Jin Lili usually uses the combination of Pinggan and Xifeng drugs and tonic drugs to treat hypertension ofliver-yang hyperactivity.The routine treatment methods are Pingganrougan and Yishenningxin.2.Through frequency analysis,it can be seen that Professor Jin Lili’s commonly used drugs for treating hypertension and liver-yang hyperactivity syndrome are Gastrodia elata Bl,gambir plant,mulberry parasitism,Eucommia,Achyranthes bidentata Bl,Radix Paeoniae Alba,poria,spine date seed.3.Through cluster analysis,it can be seen that Professor Jin Lili used Rougan Decoction to treat hypertensive liver-yang hyperactivity syndrome.4.The use of Rougan Decoction in the treatment of Grade 1 hypertension liver-yang hyperactivity syndrome can significantly reduce patients’ blood pressure,improve clinical symptoms,and improve quality of life.The cure and prescription are worthy of further scientific research and clinical application. |
