Research On The Clinical Experience Of Famous Medical Experts In The Treatment Of Dilated Cardiomyopathy Based On Data Mining

Objective: By collecting and sorting the medical cases of dilated cardiomyopathy treated by famous doctors and experts,extracting the relevant information of medical cases,establishin g the medical case database,and using the data mining method to explore the experience of famous doctors and experts in the treatment of dilated cardiomyopathy,in order to provide reference for the clinical treatment of dilated cardiomyopathy.Methods: Collect and organize medical records of famous medical experts for the treatment of DCM,and retrieve the literature(including medical records,clinical experience)of famous medical experts in the treatment of DCM from the Chinese database(CNKI,VIP,Wanfang Data),the retrieval time is from the beginning of database construction to December 31,2019.Searchable online libraries include Superstar Digital Library and Duxiu Academic Search,consult the DCM treatment books in the libraries of Henan University of Chinese Medicine and Nanyang Medical College,and extract relevant medical records information,such as gender,age,Symptoms,tongue and veins,TCM syndrome differentiation,treatment,prescription,etc.entered into Excel 2010 and established a med ical record database,unified standardization of four diagnosis information,syndrome differentiation,and prescription medicine,using three data mining methods of frequency calculation,cluster analysis,and association rules to analyze the distribution of syndromes,syndromes,and prescriptions,so as to explore the clinical experience of famous medical experts in treating this disease.Results:1.General information(1)A total of 195 medical records that meet the requirements were searched in this study,with a total of 475 consultations(2)In the medical records collected,141 male patients accounted for 72.3%,and 54 female patients accounted for 27.7%;The average age of patients was 48.43 years old,with the highest proportion of 50-59 years old,accounting for 27.2%.2.Specific situation of data mining(1)According to the statistics of syndrome frequency,a total of 159 syndromes were involved in 195 medical cases,among which the top 5 symptoms were chest tightness,palpitations,lassitude,wheezing,and shortness of breath;the top 5 tongue signs were thin white tongue,pale red tongue,dark red tongue,dull tongue,red tongue;the top 5pulses were thin pulse,deep pulse,stringy pulse,rapid pulse,weak pulse.Syndrome clustering analysis shows that the syndromes can be clustered into six categories,the first category: cough,expectoration,chest tightness,insomnia,dark red tongue;and the possible syndromes may be include lung,heart,phlegm,fluid,and blood stasis.The second category: lassitude,shortness of breath,palpitations,sweating,fatigue;and the possible syndromes may be include heart,spleen,qi deficiency,yin deficiency,yang deficiency.The third category: loss of appetite,abdominal fullness,dysuria,swelling of the legs,wheezing,suffocated,rapid pulse;and the possible syndromes may be include lung,spleen,kidney,yang deficiency,phlegm,fluid,hot.The fourth category: dark tongue,greasy white fur;and the possible syndromes may be include phlegm,blood stasis,damp.The fifth category:dentate tongue,fat tongue,pale tongue,weak pulse;and the possible syndromes may be include yang deficiency,qi deficiency,blood deficiency,damp,phlegm,fluid.The sixth category: pale red tongue,thin white tongue,deep pulse;and the possible syndrome may be include qi deficiency.Syndrome correlation analysis shows that the group with the highe st rule support is deep pulse and thin pulse,which is 49.053%;the group with the highest rule confidence is pale red tongue and thin pulse,which is 82.955%;The group with the highest gain was lassitude,palpitations,and shortness of breath,which is 2.346.(2)According to statistics of syndrome frequency,a total of 29 pathological syndromes are extracted from medical records,including 11 disease location syndromes and 18 nature syndromes,among them,disease location syndromes with a frequency greater than 5% are heart,kidney,spleen,meridian,lung,liver;nature syndromes with a frequency of more than 5% are qi deficiency,blood stasis,water retention,yang deficiency,yin Deficiency,dampness,phlegm,fluid,blood deficiency.According to cluster analysis of syndromes,syndromes are clustered into seven categories,the first category: surface,qi is not solid,lung;the second categ ory: spleen,blood deficiency;the third category: kidney,yang deficiency,heart,water retention;The fourth category: liver,heat,gall bladder,dampness;the fifth category: kidney deficiency and prostration;the sixth category: cold and poison;the seventh category:Qi deficiency,blood stasis.Syndrome correlation analysis shows that the group with the highest rule support is Qi deficiency-blood stasis,which is 68.842%;the group with the highest rule confidence is sputum-meridian,blood stasis-meridian,which is 86.275%;The group with the highest gain was meridian-sputum,sputum-meridian,which is 6.027.(3)According to the statistics of medication frequency,the top 10 Chinese herbal medicines are Huangqi,Fuling,Baizhu,Gancao,Danshen,Maidong,Tinglizi,Dangshen,Guizhi,Wuweizi;The composition herbs were classified by their category,and the frequency rank from high to low,the top three are tonify deficiency medicine,promoting blood circulation and removing blood stasis medicine,damp-clearing medicine.According to the cluster analysis of medicines,Chinese medicines are grouped into ten categories,the first category: Chashugen,Huixincao,Wuzhualong,Geshanxiao,Shengma,Jegeng,Chaihu,Zelan,Xianhecao,Dangshen,Yimucao,Gancao;The second category: Chenpi,Banxia;The third category: Taoren,Honghua,Danshen,Zheke;The fourth category: Tinglizi,Sangbaipi,Cheqianzi;The fifth category: Fuling,Baizhu,Zhuling,Zexie;The sixth category: Wuweizi,Maidong,Taizishen;The seventh category: Chuanxiong,Suanzaoren,Huangqi;The eighth category: Ganjiang,Dazao,Fuzi,Longgu,Muli,Rougui;The ninth category: Renshen,Yuzhu,Shengdihuang,Shanzhuyu,Huanglian,Yiyiren;The tenth category: Guizhi,Baishao,Danggui,Chishao.Drug association analysis shows that the group with the highest rule support is Dangshen-Huangqi,which is 45.895%;the group with the highest rule confidence is Zelan,Fuling-Huangqi,which is 97.143%;The group with the highest gain was Chaihu,Huangqi-Gancao,which is 1.597.Conclusion:1.The incidence of dilated cardiomyopathy is m ostly in male patients,and the age of onset is mainly concentrated in the 40-59-year-old stage.The core clinical manifestations are chest tightness,palpitations,lassitude,deep pulse,and thin pulse.2.The core disease of dilated cardiomyopathy lies in the three organs of heart,spleen and kidney,with qi deficiency and yang deficiency as the base,and blood stasis and water stop as the targets..3.The treatment of dilated cardiomyopathy is always based on invigorating qi,activating blood,clearing damp.According to different clinical synd romes,it is supplemented with warming yang,tonifying yin,activating circulation to remove blood stasis,tonifying heart,spleen strengthening,phlegm relieving,venation restoration.4.The clinical medications for dilated cardiomyopathy always focus on supplementing deficiency medicine,followed by activating blood circulation and removing blood stasis medicine and diuretic wetting medicine.Huangqi is the most popular single medicine,followed by Fuling,Baizhu,Gancao,Danshen,Maidong,Tinglizi,Dangshen,Guizhi,Wuweizi.
