| Objective:The purpose of this study is to investigate the TCM syndrome types,seven-emotions and psychological state of the threatened abortion patients with advanced maternal age,to study the TCM syndromes distribution of the threatened abortion patients with advanced maternal age,and their seven-emotions and psychological state,to explore the correlation between the syndrome types,seven-emotions and psychological state,which provides theoretical reference for prevention and treatment of threatened abortion with advanced maternal age with traditional Chinese Medicine.Method:All research objects were presented in Xiamen Hospital of traditional Chinese medicine,Xiamen Huli maternal and child health hospital,Jiangtou,Heshan,Jinshan and Gaoqi community health service centers affiliated to Xiamen Hospital of traditional Chinese medicine between January 2020 to December 2020.Collected their general information,seven-emotions,psychological state and other information by questionnaire survey.At the same time,collected the TCM syndrome types of threatened abortion patients,followed up and recorded the outcome of prevent miscarriage.Excel software and SPSS 24.0 statistical software were used to analyze the TCM syndromes distribution of the threatened abortion patients with advanced maternal age,and their seven-emotions and psychological state,to explore the correlation between the syndrome types,seven-emotions and psychological state of the threatened abortion patients with advanced maternal age.Results:1.A total of 278 eligible subjects were included in this study,including 94 threatened abortion patients with advanced maternal age,88 threatened abortion patients with non advanced maternal age and 91 normal pregnant women with advanced maternal age.273 valid data were recovered,and the effective rate was 98.2%.2.There was no significant difference in gestational age among the three groups(P>0.05).In the comparison of the number of previous abortions,the number of previous abortions in the threatened abortion patients with advanced maternal age group and the normal pregnant women with advanced maternal age group was significantly more than that in the hreatened abortion patients with non advanced maternal age group(P<0.05).3.In terms of seven-emotions,the threatened abortion patients with advanced maternal age had the highest score of "happiness",followed by "anger","thinking","fear","worry","surprise" and "Sadness".The scores of "anger","thinking","fear" and "shock" were higher than the hreatened abortion patients with non advanced maternal age,and the difference was statistically significant(P<0.05).Compared with the normal pregnant women with advanced maternal age,the threatened abortion patients with advanced maternal age had lower "happiness" score and higher average scores of other emotion,and the difference was statistically significant(P<0.05).4.There were 52 patients with pregnancy related anxiety in the distribution of the threatened abortion patients with advanced maternal age group,accounting for 44.68%,which was significantly more than that in the normal pregnant women with advanced maternal age group(P<0.05).There was no significant difference between the distribution of the threatened abortion patients with advanced maternal age group and the threatened abortion patients with non advanced maternal age group(P>0.05).5.The anxiety about fetal health was the highest in the threatened abortion patients with advanced maternal age group,which was different from the hreatened abortion patients with non-advanced maternal age and the normal pregnant women with advanced maternal age group.The scores of self-concern and worry about delivery in the hreatened abortion patients with non-advanced maternal age group were higher than those in the hreatened abortion patients with advanced maternal age group(P < 0.05).The scores of self-concern and fetal health in the hreatened abortion patients with advanced maternal age group were higher than those in the normal pregnant women with advanced maternal age group(P < 0.05).6.In the distribution of susceptible personality types,there were 76 patients in the threatened abortion patients with advanced maternal age group,accounting for 80.85%.The number of susceptible personality in the threatened abortion patients with advanced maternal age group was significantly more than that in the normal pregnant women with advanced maternal age group(P < 0.05),but there was no significant difference in the distribution between the threatened abortion patients with advanced maternal age group and the hreatened abortion patients with non-advanced maternal age group(P > 0.05).7.The scores of coping,sensitivity and organizational personality were the highest in the threatened abortion patients with advanced maternal age group,which were different from the hreatened abortion patients with non-advanced maternal age group and the normal pregnant women with advanced maternal age group.The score of expressive personality in the hreatened abortion patients with non-advanced maternal age group was higher than that in the hreatened abortion patients with advanced maternal age group(P < 0.05).The scores of timidity,sensitivity and compulsion in the the hreatened abortion patients with advanced maternal age group were higher than those in the normal pregnant women with advanced maternal age group,while the scores of coping personality in the threatened abortion patients with advanced maternal age group were lower than those in the normal pregnant women with advanced maternal age group(P < 0.05).8.In the distribution of TCM Syndromes of the threatened abortion patients with advanced maternal age,blood heat syndrome accounted for the highest proportion,accounting for 31.91%;kidney deficiency syndrome was second only to blood heat syndrome,accounting for 29.79%;Qi and blood deficiency syndrome accounted for 19.15%;blood stasis syndrome accounted for 10 cases,accounting for 10.64%;damp heat syndrome accounted for 8 cases,accounting for 8.51%.9.Logistic regression analysis showed that there was a certain correlation between the syndrome types of the threatened abortion patients with advanced maternal age and seven-emotions.There was a positive correlation between kidney deficiency syndrome and fear,the incidence ratio was 10.125;the incidence ratio of Qi and blood weakness was negatively correlated with Shuzhi,the incidence ratio was 0.303;the incidence ratio of blood heat syndrome and thought record was 0.284;no correlation was found between blood stasis syndrome,damp heat syndrome and seven-emotions(P > 0.05).10.Logistic regression analysis showed that there was a certain correlation between each syndrome type of the threatened abortion patients with advanced maternal age and psychological state.There was a positive correlation between blood heat syndrome and item 2of susceptible personality type,the occurrence ratio was 4.732;there was a negative correlation between blood heat syndrome and item 6 of susceptible personality type,the occurrence ratio was 0.359;there was no correlation between kidney deficiency syndrome,Qi blood weakness syndrome,blood stasis syndrome,damp heat syndrome and mental state(P >0.05).11.Logistic regression analysis showed that the number of previous abortions(1-3)was positively correlated with the kidney deficiency syndrome of the threatened abortion patients with advanced maternal age,with an incidence ratio of 4.3333.12.Logistic regression analysis showed that there was a positive correlation between blood stasis syndrome and failure of fetal protection in the threatened abortion patients with advanced maternal age with threatened abortion,and the incidence ratio was 80.0000.13.In the threatened abortion patients with advanced maternal age group,82 cases were successful and 12 cases were unsuccessful,the abortion rate was 12.77%.In the hreatened abortion patients with non-advanced maternal age group,there were 86 successful cases and 2failed cases,with the failure rate of 2.27%.The abortion failure rate of the threatened abortion patients with advanced maternal age group was higher than that of the hreatened abortion patients with non-advanced maternal age group,and the difference was statistically significant(P < 0.05).14.There was no significant difference in the distribution of seven-emotions and mental state between the failure protection group and the successful protection group(P > 0.05).Logistic regression analysis showed that there was no correlation between the treatment results of threatened abortion and seven-emotions(P > 0.05).The rate of coping with threatened abortion was 242.0.There was no correlation between threatened abortion and other psychological states(P > 0.05).Conclusions:1.In the TCM syndrome types of the threatened abortion with advanced maternal age,kidney deficiency syndrome accounted for 29.79%,Qi and blood deficiency syndrome accounted for 19.15%,blood heat syndrome accounted for 31.91%,blood stasis syndrome accounted for 10.64%,damp heat syndrome accounted for 8.5%.The frequency of blood heat syndrome and kidney deficiency syndrome is the highest.2.In the seven-emotions,the the threatened abortion with advanced maternal age are mainly "happy",and "anger","thinking" and "fear" are more common.In psychological aspect,the main anxiety related to pregnancy is to worry about fetal health,and coping,sensitivity and organizational personality as the main susceptible personality types.3.There are differences in seven-emotions and psychological state among the threatened abortion with advanced maternal age,the threatened abortion with non-advanced maternal age and the normal pregnant women with advanced maternal age.4.There is a certain correlation between each syndrome type of the threatened abortion with advanced maternal age and seven-emotions.There was a positive correlation between the score of "fear" and kidney deficiency syndrome,a negative correlation between the score of "shock" and Qi blood deficiency syndrome,and a negative correlation between the score of "thinking" and blood heat syndrome.5.There is a certain correlation between each syndrome type of the threatened abortion with advanced maternal age and psychological state in the elderly.There is a positive correlation between neuroticism personality score and blood heat syndrome,and a negative correlation between organizational personality score and blood heat syndrome.6.The failure rate of miscarriage prevention in the threatened abortion with advanced maternal age was higher than that of the threatened abortion with non-advanced maternal age.And,the failure rate of blood stasis syndrome in the threatened abortion with advanced maternal age is the highest.7.There was no significant correlation between the seven-emotions and the results of miscarriage prevention of the threatened abortion with advanced maternal age.However,the failure rate of miscarriage prevention was lower in those with higher score of coping personality. |
