ObjectiveArtificial abortion is one of the important remedial measures for female in childbearing ages who have unexpected pregnancy or contraceptive failure.Vacuum aspiration is often used in artificial abortion.Repeated uterine suction will lead to amenorrhea,intrauterine adhesion,infertility and other complications because of the damage of endometrial basement layer,laying a hidden danger for female’s reproductive health.There are some disadvantages and risks in traditional vacuum aspiration such as incomplete suction,uterine perforation and missed aspiration.In recent years,more use of ultrasound can improve the safety and reliability of the operation by monitoring the thickness of uterine muscle layer and uterine cavity.However,ultrasound is a two-dimensional indirect image with low resolution,poor stereoscopic sense and unclear image,which can not detect the uterine cavity directly.At the same time,it is difficult to operate accurately due to the influence of uterine position,curvature,thickness of abdominal fat,intestinal gas and other factors.By installing a micro camera on the top of the suction tube,the one-time uterine cavity observation and aspiration surgical system can detect the uterine cavity directly and locate the position of the gestational sac quickly and accurately.The examination and suction are carried out simultaneously,which can not only suck out the gestational sac and decidua tissue completely,but also protect the endometrium to a maximum extent.one-time uterine cavity observation and aspiration surgical system is more valuable in high-risk abortion.To explore the safety and operation effect of one-time uterine cavity observation and aspiration surgical system and transabdominal ultrasound-guided abortion,and to select appropriate surgical methods to reduce the intraoperative and postoperative risks and complications of induced abortion,especially in high-risk abortion,so as to provide reference for clinical application.MethodsWomen who voluntarily chose artificial abortion from February 2019 to October 2019 were selected from the gynecology clinic of Zhengzhou university hospital.Patients were randomly divided into observation group 100 patients,high-risk abortion observation group 38 patients(Separate from overall observation group),control group 100 patients,high-risk abortion control group 42 patients(Separate from overall control group).The observation group and the high-risk abortion observation group were given painless artificial abortion by the one-time uterine cavity observation and aspiration surgical system,the control group and the high-risk abortion control group were given painless artificial abortion transabdominal ultrasound-guided abortion.To analyze the operation index,postoperative complications,abortion effect of each group.Results1.There was no significant difference in age,menopause days and pregnancy times between the control group and the observation group(P>0.05),the general clinical data were comparable.2.There was significant difference in intraoperative blood loss between the control group and the observation group(P<0.05),but there was no significant difference in operation time,postoperative vaginal bleeding time and postoperative menstrual recovery time(P>0.05).There was significant difference in intraoperative blood loss between high-risk control group and high-risk observation group(P<0.05),but there was no significant difference in postoperative vaginal bleeding time,postoperative menstrual recovery time and operation time(P>0.05).3.There was significant difference in the incidence of cervical adhesion and intrauterine adhesion between the control group and the observation group(P<0.05),but there was no significant difference in uterine perforation and reduction of menstrual flow(P>0.05).There was no significant difference between the high-risk control group and the high-risk observation group in the incidence of decreased menstruation and uterine perforation(P>0.05),but there was significant difference in the total incidence of cervical adhesion,intrauterine adhesion and complications(P<0.05).4.There was a significant difference between the control group and the observation group in the occurrence of uterine insufficiency(P<0.05).There was significant difference between the high-risk control group and the high-risk observation group(P<0.05).Conclusions1.Abortion by the one-time uterine cavity observation and aspiration surgical system has a higher effectiveness、a lower incidence of postoperative complications and safer than transabdominal ultrasound-guided abortion.2.Abortion by the one-time uterine cavity observation and attraction surgical system has better results than transabdominal ultrasound-guided abortion.3.Abortion by the one-time uterine cavity observation and attraction surgical system has more clinical value for high-risk abortion.
