| Objective:In This study,we compared the atherogenic index of plasma(AIP)and other related lipid indicators in obstructive coronary heart disease patients and non obstr uctive coronary artery group patients,aiming to explore the relationship of AIP an d Gensini score,and to provide theoretical basis for early identification,diagnosis a nd treatment of coronary heart disease,and improve the national awareness of blo od lipid prevention and control.Methods:This study is a retrospective study.patients who underwent parallel coronary a ngiography in the Affiliated Hospital of Qingdao Universityfrom January 2021 to January 2022 were include.The patient were divided into two groups according to the results of coronary angiography,namely 167 patients in obstructive coronary h eart disease group and 173 patients innon obstructive coronary heart disease group(control group).General clinical data and biochemical indicators were recorded i n detail,including triglycerides(TG),total cholesterol(TC),high density lipopro tein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),apolipop rotein A1(apo A1),apolipoprotein B(apo B),lipoprotein A(LPa),and calcula te the compound lipid parameters such as LDL-C/HDL-C,TC/HDL-C,apo B/apo A1,non HDL-C,Atherosclerosis index(AI),Lipoprotein association index(LCI)and AIP,the degree of coronary lesions was scored according to the Gensini scori ng.Referring to the statistical software SPSS 26.0,compare the general data and biochemical indicators between the two groups and analyze the correlation betwee n AIP and Gensini score.Spearman correlation analysis was used to analyze the correlation of the AIP,Gensini score and other lipid indexes,univariate and multi variate logistic analysis were conducted to explore the independent influencing fact ors of coronary occlusion,and receiver operating characteristic curve(ROC)was a dopted to evaluate the predictive value of the various lipid indexes on obstructive coronary heart disease.The groups were divide into AIP>0.13 and AIP<0.13 acco rding to the AIP levels to analyze the relationship between AIP levels and Gensin i score in patients whose LDL-C cholesterol were less than 1.8 mmol/L,to evalu ate the clinical value of AIP in the diagnosis of coronary stenosis comprehensivel y.Results:1.Comparison of general baseline data: there were no significant differences between coronary occlusion group and control groups in gender,age,body mass i ndex,alcohol consumption,hypertension,diabetes and smoking,fasting blood sugar,blood pressure.the P-values were all greater than 0.05.The lipid parameters whic h conclude TG,TC,LDL-C,LPa,apo B,AI,LCI,non HDL-C,LDL-C/HDL-C,T C/HDL-C and AIP were significantly higher in obstructive group,while HDL-C a nd apo A1 were significantly lower in the coronary occlusion group.2.Multivariate logistic regression analysis showed that non HDL-C,LDL-C/H DL-C,LCI,AIP and LDL-C were positively associated with coronary occlusion,were independent risk factors and AIP was the most closely related lipid paramete rs related to the occurrence of coronary artery obstruction.3.The predictive effect of blood lipid parameters on coronary occlusion was analyzed by ROC curve,and the results suggested that the AIP level had moderat e predictive value.The area under the curve(AUC)was 0.700 when the cut-off value was 0.14,the sensitivity was 65.9% and the specificity was 67.1%.4.The Spearman correlation analysis was used to study the association of AI P with blood lipids.We found that AIP was positively correlated with males,BM I,LDL-C,non HDL-C,TC/HDL-C,LDL-C/HDL-C,apo B/apo A1,apo B,LCI,and AI,while a negative correlation with apo A1,age.5.The Spearman correlation analysis the correlation of blood lipid indexes le vels with Gensini scores,which showed that the AIP,TG,LDL-C,LDL-C/HDL-C,TC/HDL-C,LCI,AI levels were positively associated with the Gensini score,the HDL-C,apo A1 were negatively associated with the Gensini score.The AIP was e ntered into a multivariate stepwise regression equation of Y=44.78*X+23.03.6.The Gensini score was higher than in high AIP regardless of whether LD L-C Levels was less than 1.8mmol/L in the coronary obstruction group.Conclusion:1.The higher AIP levels are closely associated with the occurrence of corona ry obstruction and stenosis.AIP is an independent risk factor for coronary artery occlusion with a sensitivity of 65.9% and 67.1% specificity at a cutoff value of 0.14.2.AIP is positively correlated with Gensini score and is a risk factor for the degree of coronary artery lesions,and AIP may be a potential indicator of the de gree of coronary lesion stenosis.3.AIP is positively associated with male,BMI,TG,LDL-C,non HDL-C,TC/HDL-C,LDL-C/HDL-C,apo B/apo A1,apo B,LCI,AI,and has a negative correlati on with apo A1 and age.4.When the LDL-C cholesterol of patients with CHD meeting the standards,h igher AIP is associated with higher Gensini scores,and AIP can serve as one of the concern indicators of dyslipidemia and another possible indicator for monitorin g lipid-lowering treatment in patients with CHD. |
