| Objective:Coronary atherosclerotic heart disease(coronary heart disease, CHD)is a serious threat to human health of common disease, frequently-occurring disease, CHD has become the leading cause of death in population in developed countries, with its high morbidity and mortality. CHD is the result of a chronic coronary artery lesions, under the influence of multiple risk factors. Investigate its etiology, dyslipidemia is the most important risk factor for atherosclerosis. Traditional single index of lipid abnormalities can not fully explain and predict cardiovascular disease incidence and the law of outcome. Clinically, most of the atypical early symptoms of patients with coronary heart disease are not to perceive, so easy to misdiagnosis. Although the coronary angiography can be intuitive degree of coronary artery stenosis and confirmed or excluded coronary heart disease, it belongs to the invasive operation, difficult to widely spread in the crowd. Now,we observed the blood lipid levels between the normal control group and CHD group, explored the relationships between the atherogenic index of plasma and the Gensini score, to provide early active prevention and treatment of CHD with more convenient clinical basis.Methods:400 hospitailized patients who had accepted coronary angiography(CAG) or percutaneous coronary intervention(PCI) from January 2015 to December 2015 in Cardiovascular division of the Second Hospital of KunMing medical university were enrolled. According to the results of the CAG, the patients were divided into two groups:normal control group (non-CHD,n=99), Gensini score =0) and coronary heart disease group(CHD,n=301), and the CHD group were divided into three groups:mild lesion group(MI-CHD 0<Gensini score<30), moderate lesion group (MD-CHD 30<Gensini score< 60) and severe lesion group (S-CHD Gensini score≥60).The clinical data included sex, age, height, weight, BMI, smoking, Drinking, hypertension,diabetes, fasting blood-glucose, total choesterol, triglyceride, high density lipoprotein cholesterol, low-density lipoprotein cholesterol and the result of the coronary angiography. To analyze the clinical data of 400 cases in the hospital, and calculate the AIP=[log(TG/HDL)]. We compared the indicators all above between the two groups.Then we substituted the meaningful variables into the Logistic regression and analyzed the risk factors of CHD to predict the CHD.We compared the differences between the subgroups on the AIP,and analyzed the correlation between the AIP and Gensini score, then made the linear regression analysis on the factors affecting the Gensini score.Results:1ã€Comparison of the CHD group and the control group in general clinical dataThere were no significant differences in gender, age, BMI, the prevalence of hypertension, the prevalence of diabetes, smoking rate, drinking rate, fasting blood-glucose of the control group and the CHD group (P>0.05)2ã€Comparison of the CHD group and the control group in in single lipid indexesThere were significant differences between the CHD group and the control group in the serum levels of HDL-C,LDL-C,(P<0.05). The CHD group LDL-C was higher than these in the control group, HDL-C was lower than that of control group. The CHD group AIP was higher than these in the control group.5ã€Comparison of among the different coronary artery Gensini score subgroups in the AIPThere were significant differences among the different coronary artery Gensini score subgroups in AIP(P<0.05), and rise with the Gensini score. There was a progressively increased tendency on AIP in the three subgroups. LDL-Cã€AIP were the independent risk factor for the Gensini score; HDL-C were the independent protect factor for the Gensini score.Can be thought of as the increase of the AIP Gensini score increased coronary artery in patients with coronary heart disease, coronary stenosis, the higher the lesion severity.Conclusions:1ã€The levels of plasma TC, TG, HDL-C, LDL-C are related to the pathogenesis of coronary artery Gensini score.2ã€AIP can reflect the lipid metabolism of coronary heart disease,and is closely related to the severity of coronary atherosclerosis. There was a progressively increased tendency on AIP in the Gensini score. AIP can predict the onset of coronary heart disease risk, and evaluation of coronary artery stenosis disease severity. |
