In China,malignant tumors account for 23.91% of the total morbidity and mortality of the Chinese population,and the large number of cancer patients,with their different diagnosis and treatment methods and orientation,makes cancer treatment decisions very complicated and requires the participation of patients.However,how to involve patients in the treatment decision-making process requires further exploration and research.On the basis of this,this paper analyzes the role of doctor-patient communication in patient decision-making by combining qualitative and quantitative methods.How patient participation in decision-making influences health outcomes in doctor-patient communication.In order to promote the participation of cancer patients in treatment decisions and guide the communication in clinical treatment decisions.This study uses a hybrid approach to analyze,interpret and study in a quantitative and qualitative way that inspires each other.The details are in three parts: 1.Cancer patients were involved in a study of factors influencing treatment decisions.In this study,a purposeful sample was collected from breast surgery,Beijing hospital and Huashan hospital in the oncology department of the Chinese academy of medical sciences.Among the top three hospitals in this category,doctors took on a huge amount of outpatient and surgical work,giving each patient only a few minutes of outpatient consultation time on average during the daily overload.But patients come here from all over the country with high expectations,and the tension between doctor and patient is highlighted by the fact that,during the brief visit,it is difficult for patients to give a full account of their previous experience and illness.In this context,we collected data from a field of 272 valid questionnaires to explore the influence of individual and interpersonal factors on cancer patients’ participation in treatment decision-making from the perspective of doctor-patient communication,and to model these factors.2.In the current clinical research,the role of patient self-report in patient health evaluation is becoming more and more important.In this study,29 cancer patients and their families were interviewed in a semi-structured way about their experiences of communication and treatment decision-making,and the specific content and main features of each stage of cancer decision-making were identified.3.By combining the results of the study,quantitative and qualitative analysis support each other,complement the demonstration,and construct the theoretical framework of doctor-patient communication and patient participation in decision-making.The study found that there are two pathways for doctors to communicate health outcomes based on their understanding of patient decision preferences.Pathway one,to understand the decision preferences of patients,through patient-centered communication as the intermediary,in communication to promote patient participation in decision-making,to help achieve a positive doctor-patient trust relationship,improve patient mental health.Pathway two,through patient-centered communication that respects patient autonomy and provides targeted information support based on patient decision information needs and concerns,to achieve outcomes that are beneficial to patient health.At the same time,clinicians should respect the patient’s preference for decision participation,and collaborative decision participation does not apply to every patient.Break the institutional arrangement centered on health care,help patients truly access to services,break through the double bind of patients,achieve the situation that is beneficial to the individual patients,and at the same time the health system.The theoretical framework of patient participation in decision-making based on doctorpatient communication is expected to provide a theoretical basis for further research and practice of patient participation in decision-making. |
