Exploration Of Clinical Experience In The Treatment Of T2DM With Jianpi Xiaoke Decoction

Objective:The data mining technology was used to make statistics and analysis on the clinical case data related to the treatment of type 2 diabetes(T2DM)with Jianpi Xiaoke Decoction,and summarized the clinical experience of Jianpi Xiaoke Decoction in treating T2DM.Methods:A total of 258 patients were collected from the outpatient and ward of Endocrinology Department of Affiliated Hospital of Shandong University of Traditional Chinese Medicine from January 2021 to June 2022.According to the requirements of statistical analysis and data mining,a Microsoft Excel database for the treatment of T2DM with Jianpi Xiaoke Decoction was established.Through SPSS Statistics 26,SPSS Modeler 18.0 and Microsoft Excel2016 software,frequency analysis,system cluster analysis and association rule analysis are performed on the data in the database,and the analysis results are discussed.Research results:1.Among the 258 medical records collected,the male to female ratio is 1.5:1;The age range is mostly between 41 and 50 years old;The course of the disease is mostly within 5 years;The number of people with concomitant hyperlipidemia is the highest,at 102.2.The results of tongue pulse statistics showed that the tongue texture was mainly red tongue,light red tongue,fat tongue and light purple tongue;The tongue coating is mainly thin yellow coating,dry coating,thin white coating and white greasy coating;The pulse is mainly composed of deep and slow pulse,thin and slow pulse,and the main single pulse is thin pulse,slow pulse,deep pulse,number pulse and string pulse.3.The most common symptoms are fatigue,thirst and frequent urination.The system cluster analysis of high frequency symptoms obtained five groups of high frequency symptoms: C1: low back pain,low back and knee weakness,frequent urination,dizziness,edema,and dizziness;C2: numbness of limbs and fear of cold;C3: fatigue,thirst,and sleeplessness;C4: Post-eating sleepiness,abdominal distension,anorexia,loose stool;C5: thirsty,drink more,eat more,suffer from hunger,increase urine volume,constipation,and spontaneous sweating.It can be divided into: spleen deficiency dampness heat syndrome,yin deficiency dryness heat syndrome,spleen and kidney deficiency syndrome,qi yin injury syndrome,and yang deficiency blood stasis syndrome.4.A total of 258 prescriptions were collected from 258 medical records,including 136 kinds of drugs,and the total frequency of use of traditional Chinese medicine was 3282 times.There are 27 high-frequency drugs(>30 times).The top 10 drugs ranked by frequency are: astragalus(244),raw rehmannia(240),coptis chinensis(235),trichosanthin(222),pelan(203),achyranthes bidentata(170),poria cocos(109),roasted licorice(102),yam(84),ophiopogon japonicus(69).The statistical results of drug properties and taste showed that the proportion of mild,cold,mild cold,warm and mild temperature was relatively large;Sweet,bitter and pungent are the most common in the taste of medicine.The statistical results of meridian classification of drugs showed that antipyretic drugs and tonic drugs accounted for more;Among the meridians,most belong to the spleen,lung,liver,stomach and kidney meridians.5.A total of five clusters were obtained by systematic cluster analysis of high-frequency drugs,which were P1: Potentilla discolor,mulberry branches,ophiopogon japonicus,Schisandra chinensis,pueraria lobata,cauliflora henryi,anemarrhena asphodeloides,coptis chinensis,trichosanthes pollen,raw rehmannia glutinosa;P2: Pinellia ternate,scutellaria baicalensis,roasted licorice,bupleurum,codonopsis pilosula;P3: cassia twig,white peony,angelica;P4: Peilan,Achyranthes bidentata,Atractylodes macrocephala,Astragalus membranaceus;P5: Cornus officinalis,peony bark,Alisma orientalis,Poria cocos,Chinese yam.6.In the analysis of association rules of high-frequency flavored drugs,the support degree ≥10%,the confidence degree≥80%,and the gain>1 were set to obtain 6 association rules of 2 drugs,of which the cornus officinalis+rehmannia glutinosa has the highest promotion degree,and the poria cocos+alisma orientalis has the highest confidence degree;There are33 rules for the association of the three drugs,of which the support and confidence level of the three drugs are the highest,and the promotion level of the three drugs is the highest.These drug combinations can be regarded as the two and three most commonly used drugs in the treatment of T2DM with Jianpi Xiaoke Decoction.Conclusions:1.The majority of T2DM patients treated with Jianpi Xiaoke Decoction are middle-aged,with a course of disease mostly within 5 years and a history of other diseases.The most prominent symptoms are fatigue,thirst,excessive drinking,and frequent urination.The syndrome types are mostly based on spleen deficiency,which can evolve into spleen deficiency dampness heat syndrome,yin deficiency dryness heat syndrome,spleen kidney deficiency syndrome,qi yin injury syndrome,and yang deficiency blood stasis syndrome.2.In the treatment of T2DM,Jianpi Xiaoke Decoction has the characteristics of combining cold and warm,sweet and bitter and bitter,clearing and tonifying,and taking calmness as the basis.Five groups of clustering formulas were obtained by high-frequency drug cluster analysis,which were respectively the products of invigorating the spleen and removing dampness,tonifying the kidney and spleen,nourishing yin and clearing heat,soothing the liver and strengthening the spleen,warming the meridians and activating blood circulation.Commonly used drug pairs include cornus officinalis+cooked rehmannia,poria cocos+alisma orientalis,and commonly used horn drugs include chicken root extract+anemarrhena+yam,chicken root extract+kudzu root+ophiopogon japonicus,etc.
