| Objective:The medical records of inpatients diagnosed as Unstable angina pectoris,UAP in the three departments of cardiology of the First Affiliated Hospital of Heilongjiang University of traditional Chinese medicine were collated,and the disease conditions,characteristics of TCM syndrome differentiation and treatment,traditional Chinese medicine use and acupuncture acupuncture were explored,so as to provide more valuable clinical references for the prevention and treatment of the disease.Methods:Collected 306 inpatients who were first diagnosed with UA in the First Affiliated Hospital of Heilongjiang University of traditional Chinese Medicine from December 2018 to December 2021.Statistics of age,gender,course of disease,morbidity season,TCM tongue diagnosis and pulse diagnosis,TCM syndrome differentiation and use of TCM,acupuncture points,etc.,and the collected clinical medical records are sorted and analyzed.Result:1.Gender,age and course of disease: The 306 patients included in the study had a maximum age of 86 years and a minimum age of 41 years,with a mean age of 68.51 ± 9.91 years.Among them,there were 208 female patients(67.97%),the oldest age was 86 years old,and the youngest age was 48 years old.There were 98 males(32.03%),the oldest age was 85 years old,and the youngest age was 41 years.The course of disease was less than 2 years,46 people,28 people in 2-5 years,94 people in 5-10 years,60 patients in 10-15 years,36 patients in 15-20 years,31 patients in 20-25 years,and 11 patients in more than 25 years,with an average course of disease of 10.59±7.60 years.2.Tongue and pulse conditions: A total of 8 kinds of tongue texture were counted,and the top 4 cases of tongue statistical frequency were 91 cases(29.74%)of light dark tongue,83 cases(27.12%)of dark red tongue with ecchymosis,58 cases(18.95%)of purple dark tongue,and 37 cases(12.09%)of purple tongue.This time,a total of 5 kinds of tongue moss were counted:white moss,yellow moss,white moss,dry moss,and yellow moss.There were105 cases(34.31%)of white moss and 132 cases(43.14%)of white moss,which accounted for 77.45% of the total tongue moss.This time,a total of 14 pulses manifestation were counted,of which the first 8 pulses are: string fine pulse,string astringent pulse,fine astringent pulse,string slip pulse,Confucian slow pulse,dull pulse,knot pulse,string tight pulse.Among them,40 cases(13.07%)were in the excess pulse,112 cases(36.60%)were in the feeble pulse,and 154 cases(50.33%)were in the intermingled deficiency and excess pulse.3.TCM syndrome type:This statistical study divided the TCM syndrome type into 8 types,the top three are: 92 cases(30.07%)of blood stasis due to qi deficiency type,58 cases(18.95%)of stagnant blockade of heart blood type,and 52 cases(16.99%)of phlegm-turbid obstruction type.4.Triggers: Among the first triggers,There were 63 cases(20.59%)who did not find obvious predisposing factors,243 cases(79.41%)with clear predisposing factors,again aggravating factors,70 cases(22.88%)patients with no obvious exacerbation factors,and 236 cases(77.12%)with clear exacerbation factors.5.Onset of solar terms: The top six solar terms with the highest incidence are the beginning of winter,heavy snow,great cold,winter solstice,light snow,all of which occur in winter.6.Frequency of acupuncture point selection: The top six with the highest frequency are Neiguan Point,Shenmen Point,Haomen Point,Zusanli Point,Blood Sea Point,and Diaphragm Yu Point.7.Acupuncture point cluster analysis : The 25 acupuncture points collected were clustered and analyzed,and the results were composed of 5types of acupuncture points,which were always based on relieve pain,regulating qi and activating blood.8.This study included 376 prescriptions,a total of 169 traditional Chinese medicines,a total frequency of 6272,of which the top 10 traditional Chinese medicines used were peach seed,safflower,chinese angelica,milkvetch root,debark peony root,largehead atractylodes rhizome,tangshen,danshen,indian bread,and sichuan lovage rhizome,all of which were used more than 100 times.9.Cluster analysis of traditional Chinese medicines: The cluster analysis of the top 50 traditional Chinese medicines used in frequency was carried out,and the clustering results obtained a combination of 5 commonly used drugs,with the main effects of activating blood circulation and removing stasis,regulating qi and expectorant,strengthening the spleen and invigorating qi,nourishing the mind and calming the mind,and nourishing the liver and kidneys.10.Comorbidities: The top three patients with comorbidities were 184(60.13%)with hyperlipidemia,167(54.58%)with hypertension,and 142(46.40%)with lacunar cerebral infarction.11.Comparison before and after treatment:There were 306 patients with 0cases of asymptomatic angina before treatment,52 cases of grade I.,197 cases of grade II.and 57 cases of grade III.before treatment,and 33 cases of asymptomatic angina,111 cases of grade I.,136 cases of grade II.and 26 cases of grade III.after treatment.Conclusion:1.The present study showed that the high prevalence season of UAP was winter;the prevalence of the disease tended to increase after the age of 50 years,overall female patients were higher than male;hyperlipidemia accounted for the highest percentage of comorbidities.2.This study showed that the tongue texture of UAP patients was mainly blood stasis tongue,tongue lichen white moss,white lichen were predominant,and the tongue were mainly intermingled deficiency and excess pulse.Among the patients,the medical symptom types are mainly blood stasis due to qi deficiency type,stagnant blockade of heart blood type,phlegm-turbid obstruction type.3.In this study,the main prescriptions for the treatment of chest palsy and heartache were peach seed,safflower,chinese angelica,milkvetch root,debark peony root,largehead atractylodes rhizome,tangshen,danshen,indian bread,and sichuan lovage rhizome.4.The acupuncture points are mainly Neiguan,Shenmen,Haomen,and Zusanli. |
