China has a large population base,relatively few medical resources per capita,and unbalanced regional development,which results in significant differences in the level and efficiency of resource allocation among different regions.At present,China is facing the important problem of how to rationally distribute the medical resources among various regions.The sudden outbreak of COVID-19 has directly revealed the lack of medical resources,unbalanced distribution and insufficient service capacity that had always existed in healthcare.This thesis selected the panel data of 31 provinces,municipalities and autonomous regions in China from 2009 to 2020 to analyze the efficiency of medical resources allocation in China.The objective is to explore the differences in the efficiency of medical resources allocation among provinces and cities in China and the causes,so as to provide the theoretical basis for further optimizing the allocation of medical resources in China.Firstly,the DEA-Malmquist index model was used to analyze the panel data of DMU.According to the number of DEA model indicators,input-output indicators were determined from three directions: manpower,material resources and financial resources,so as to comprehensively measure the allocation efficiency of medical resources in various regions of China from 2009 to 2020.At the same time,Arc GIS software was used to visualize the distribution of efficiency.Then,the efficiency values obtained by DEA-Malmquist index model were used to draw the kernel density curve and standard deviation ellipse diagram,and the traditional and spatial Markov chain transfer matrices were calculated.The temporal characteristics,spatial characteristics and dynamic evolution of medical resource allocation efficiency in China as a whole and in the four regions were analyzed respectively.At the same time,the efficiency value obtained by DEA-Malmquist index model was taken as the explained variable,and the internal and external influencing factors of medical resource allocation efficiency were taken as the explanatory variables.Tobit regression analysis was carried out using Stata software to further analyze the influencing factors of medical resource allocation efficiency in China.Finally through the use of various model method of medical resource allocation efficiency value,temporal,spatial characteristics,dynamic evolution and the analysis of the influencing factors,so as to achieve all-round,multidimensional reflect changes of health resources allocation efficiency in our country,for the study of medical resources allocation efficiency provides new train of thought and method,abundant empirical research results of medical treatment service,to promote the further development of medical service research.Based on the above research contents,the following conclusions are drawn.Firstly,the average value of the allocation efficiency of medical resources in China was0.985 from 2009 to 2020,showing an overall trend of fluctuation.There were great differences in the development of medical resource allocation efficiency among provinces in China.The first was Beijing and the last was Tibet Autonomous Region with an efficiency difference of14.7%.From the perspective of the eastern,central,western and northeastern regions,the comprehensive ranking of medical resource allocation efficiency during 2009—2020 was northeast region = eastern region > central region > western region.Secondly,from the perspective of temporal characteristics,the national medical resource allocation efficiency showed a small degree of polarization during the study period.The overall medical resource allocation efficiency showed an upward trend,with occasional slight fluctuations,and fell back to a lower state after the COVID-19 epidemic in 2020.There are significant regional differences in the temporal evolution characteristics of medical resource allocation efficiency in the four regions.From the perspective of spatial characteristics,the distribution center of the standard deviation ellipse is generally located in the western part of Henan Province,indicating that the efficiency level of medical resource allocation in the eastern region is superior to that in the western region.The standard deviation ellipse deviation of medical resource allocation efficiency in the four regions is as follows: there is almost no migration in the eastern region;the deviation degree of the central region is slightly larger than that of the eastern region;the western region rotated due north on the whole;the northeast has shifted mainly to the northeast.From the perspective of dynamic evolution,whether spatial factors are taken into account or not,the regions with medium and high levels of medical resource allocation efficiency are more stable,and the probability of transfer to other levels is smaller.There are significant differences in the elements of the two Markov chains,indicating that the allocation efficiency of medical resources in China has obvious spatial spillover effect.Thirdly,as for the analysis of influencing factors,the Tobit regression results showed that the proportion of health technicians,the daily visits of physicians and GDP had no significant impact on the allocation efficiency of medical resources.The average daily inpatient days,per capita medical expenses and population density had significant negative effects on the efficiency of medical resource allocation.The utilization rate of hospital beds,average length of stay,average medical expenses per outpatient visit and health budget index all had significant positive effects on the efficiency of medical resource allocation. |
