Study On The Clinical Experience Of Professor Qiu Lin’an In The Treatment Of Ankylosing Spondylitis By Syndrome Differentiation Based On The Ancient And Modern Medical Cloud Platform System

ObjectiveBy using the data mining technology of the cloud platform of ancient and modern medical records,this paper systematically summarizes Professor Qiu’s clinical experience in the treatment of ankylosing spondylitis,in order to provide a new way of thinking for the treatment of ankylosing spondylitis with traditional Chinese medicine.MethodsThis study mainly selected the outpatient medical records of patients with ankylosing spondylitis disease who visited Professor Qiu Lin’an’s Clinic from 2018 to 2022.According to the inclusion and exclusion criteria,92 patients were included in the outpatient records,with a total of 251 consultations.The prescriptions were entered into the "Ancient and Modern Medical Case Cloud Platform" software,and after data standardization,frequency statistics were performed on the patients’ basic information,clinical manifestations,and frequency of use of traditional Chinese medicine and efficacy.Association rule method,cluster analysis method and complex network were used to excavate and analyze traditional Chinese medicine pairs,commonly used traditional Chinese medicine combinations,core prescriptions.Results1.A total of 92 patients with diarrhea were included in this study,of whom 68 were men and 24 were women,with a significantly higher incidence in men than in women;the age of onset was mostly 20-49 years old,accounting for almost 67.39% of the total cases.2.A total of 28 symptoms with frequency≥10 were found in the study,among which lumbago accounted for 80.08%.More than 50% of the clinical symptoms are: lumbago,back pain,morning stiffness,neck stiffness;It involves 6 kinds of tongue and 7 kinds of tongue coating,among which the occurrence frequency of tongue dull red,tongue light red,tongue thin white,tongue red and tongue yellow greasy is more than 20%;There are 7 kinds of pulse patterns involved,among which the frequency of sinking pulse,slippery pulse,string pulse,thin pulse and number pulse is more than 10 times.3.There were 117 TCM flavors involved in the study,and the top 20 high frequency TCMs were Mulberry parasitica,Eucommia ulmoides,Pueraria pueraria,Poria cocos,Safflower,Rhizoma corydalis acetate,the root of bidentate achyranthes,Caulis spatholobi,Teasel root,Hairyvein agrimony,White peony,Cauliflower,Lagehead atractylodes,Clematis,Aconitum carmichaeli Debx,Cassiatwig,Orientalwaterplantain,Asarum,Preparedrehmanniaroot,Angelica dahurica.4.In the analysis of the properties of TCM,the four qi were mainly warm,followed by flat and lukewarm,mild cold,cold and cool,and few TCMs with major cold and major fever were used.The five tastes are sweet,hot and bitter followed by mild bitter,sour,light,salty and astringent.It was shown that TCMs mainly entered the liver,kidney,spleen,lung,stomach,heart,bladder and other channels.5.Association rule analysis was used to obtain 36 groups of associated TCM pairs,and it was found that Eucommia ulmoides,Mulberry parasitism,Pueraria,Poria cocos,Safflower,Rhizoma rhizoma acetate and other TCMs had strong correlation with each other.In addition,the combination of Angelica dahurica and Pueraria has a very high degree of confidence and promotion,indicating a strong correlation between the two.6.According to the correlation analysis of TCM and syndrome,it was found that when Master Qiu treated AS patients with deficiency syndrome of liver and kidney,he took Duhuojisheng decoction;Jitongling prescription and Sanmiao Powder was used to treat AS patients with kidney deficiency and heat-deficiency syndrome.In the treatment of patients with AS syndrome of the stagnation of Governer Meridian,derive from Jitongling prescription;Liuwei Dihuang Wan and Shenling Baizhu Powder were used to treat AS patients with spleen-kidney deficiency syndrome.7.Cluster analysis was conducted on the top 40 most frequently used Chinese herbs,and six categories were obtained: the first category: Eucommia ulmoides,Mulberry parasitic,Caulis spatholobi,Rhizoma corydalis acetate,Poria cocos,Pueraria root,the root of bidentate achyranthes and Safflower.The second category: Cassia twig,Paeony,Radix aconite,Angelica dahuricae;The third category: Cauliflower,Teasel root,Asarum;The fourth type:Clematis;The Fifth type: Hairyvein agrimony;The sixth category: Atractylodes,Alismatis and Prepared rehmannia root.8.Using complex network analysis,the core prescriptions were obtained: Mulberry parasite,Eucommia ulmoides,Poria cocos,Pueraria,Safflower,Rhizoma corydalis,Caulis spatholobi.ConclusionClinical ankylosing spondylitis patients are mainly young and middle-aged men,the pathogenesis is characterized by the deficiency of the primary and solid,the deficiency of the viscera as the primary,wind cold and dampness-heat stasis and mixed as the standard.When the treatment of the selection of warm in nature,sweet,xin,bitter TCMs,into the kidney,liver,spleen meridian,the main treatment for kidney,liver and spleen harmonization,dispel wind and dehumidification and remove blood stasis;The core prescriptions for the treatment of ankylosing spondylitis by Professor Qiu are Mulberry parasite,Eucommia ulmoides,Poriacocos,Pueraria,Safflower,Rhizoma corydalis,Caulis spatholobi.Inclinical practice,Duhuojisheng decoction,self-designed Jitongling prescription,Add Sanmiao powder,Liuwei Dihuang Wan,Shenling Baizhu powder and other prescriptions are often used to treat ankylosing spondylitis with different syndromes.Professor Qiu Lin’an attaches great importance to syndrome differentiation of zang-fu organs,deficiency and excess of cold and heat,attaches great importance to the combination of syndrome differentiation and disease differentiation,taking into account TCM samples,and treating both body and mind.
