Study On The Clinical Experience Of Professor Zhang Haojun In The Treatment Of Diarrhea By Syndrome Differentiation Of Six Meridians Based On The Ancient And Modern Medical Cloud Platform System

Objective:Through the use of data mining technology,the clinical experience of Professor Zhang Haojun in using the six meridians to identify and treat diarrheal diseases was sorted out and summarized,with a view to providing a new way of thinking for the treatment of diarrheal diseases in Chinese medicine.Methods:From 2019.01 to 2021.12,we collected the case data of Professor Zhang Haojun’s outpatient clinic for the treatment of diarrhea and collated cases that met the subject criteria,including 305 cases and 785 consultations in total.The prescriptions were entered into the "Ancient and Modern Medical Case Cloud Platform(V2.3)" software,and after data standardization,frequency statistics were performed on the patients’ basic information,clinical manifestations,and prescription drug composition and taste and ascription,and methods such as association rules,cluster analysis and complex network analysis were applied to mine the common pairs of drugs for treating diarrhea and The core prescriptions were identified.Results:1.A total of 305 patients with diarrhea were included in this study,of whom 178 were men and 127 were women,with a slightly higher incidence in men than in women;the age of onset was mostly 30-59 years old,accounting for 77.71% of the total cases.2.A total of 87 kinds of symptoms appeared in the subject with 4196 frequencies,among which,the percentages >20% were: loose stools,dry mouth,unheated limbs,abdominal pain,dullness,leaking down heavy,and chest discomfort;6 kinds of tongue texture were involved,with dark tongue and light red tongue predominating;7 kinds of tongue coating,with white coating leading the list;9 kinds of pulse signs,with the first two kinds of pulse signs being stringy and fine pulse.3.In this study,the syndrome type of diarrhea disease is Shaoyang Taiyin syndrome,followed by Jueyin syndrome,and Sanyang Reli syndrome and Taiyin Shaoyin syndrome are rare.4.There are 158 herbs involved in the subject,and the top 13 drugs used more frequently are,in order: Gui Zhi,Gan Jiang,Bai Shao,Fu Ling,Baked Licorice,Bai Zhu,Ginger,Jujube,Huang Lian,Chai Hu,Citrus aurantium,Scutellaria baicalensis,and Ginger Semen.5.In the analysis of the properties of Chinese medicine,the four qi are mostly warm in nature,and rarely use cool and cold drugs;the five flavors are mainly pungent drugs,followed by sweet and bitter flavors;the normal meridians mainly enter the spleen,lung,stomach,heart,kidney,liver and other meridians;the effects are mainly tonifying,warming Yang and clearing heat.6.Using the association rule analysis,52 combinations of Chinese medicines were obtained,and the analysis revealed that Poria,Atractylodes macrocephala,Radix et Rhizoma Paeoniae,Radix et Rhizoma Glycyrrhizae,Radix et Rhizoma Chai Hu and Radix et Rhizoma Citrus reticulatae were strongly associated with each other;in addition,the confidence and elevation of the combination of Radix et Rhizoma Ginger and Radix et Rhizoma Scutellariae were very high,indicating that the association was extremely strong.7.Using chi-square test analysis,the corresponding pairs of drugs for the treatment of different co-morbidities were obtained,and the strong correlations were as follows: gastric distress-Hou Pu,Chen Pi/Orange Red,poor sleep-Long Bone,Oyster,Lily,Di Huang,throat discomfort-Bitter Almond,Hou Pu,Mandarin,Licorice,neck soreness-Ge Gen,Ephedra,belching-Cyclamen,Dai Ochre.8.Cluster analysis of the top 40 herbs with the highest frequency of use yielded eight categories of drug combinations:(i)ginger and scutellaria;(ii)yellow lily of the valley;(iii)woodruff and heliotrope;(iv)shengqu;(v)poring and atractylodes;(vi)peony,cinnamon and ginger;(vii)ginseng,angelica,cypress,umeboshi,pepper,epilobium and hosanna;and(viii)red ginseng.9.Using complex network analysis,the core prescription was derived: dried ginger,cinnamon stick,white peony,poria,yellow lily,roasted licorice,and angelica.Conclusions:1.The clinical population of diarrhea is mainly young and middle-aged males,and the evidence type is mainly Shaoyang Taiyin evidence and Jueyin evidence,and the disease nature is mainly mixed with cold and heat,and mixed with deficiency and reality.2.Zhang Shi used mostly pungent and warm drugs,mainly to warm the Yang and nourish the deficiency,but also the use of heat-clearing herbs to clear dampness and heat to stop diarrhea,and his high-frequency drugs,associated and clustered drug combinations,and core prescriptions all reflect this medication feature.3.Zhang Shi believes that diarrhea is mainly responsible for the deficiency of the spleen and cold of the Taiyin,but it is related to the other five meridians.It can be divided into four syndrome types: Sanyang Reli syndrome,Shaoyang Taiyin syndrome,Taiyin Shaoyin syndrome and Jueyin syndrome.Its pathogenesis is dynamic.It is necessary to understand the transmission of evil Qi between the six meridians by observing the rise and fall of Yang Qi,so as to adjust the medication in time.
