Purpose:By collating the case data of hospitalized patients with psoriasis vulgaris blood-heat syndrome in the Department of Dermatology,Affiliated Hospital of Liaoning University of Traditional Chinese Medicine in the past three years,including basic information,clinical manifestations,laboratory tests,and treatment.To analyze the related factors and treatment experience of this disease,in order to provide a reference basis for the prevention and treatment of this disease.Material and method:A total of 163 patients with blood-heat syndrome of psoriasis vulgaris who were hospitalized in the Department of Dermatology,Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from January 2019 to December 2021 were collected.The medical records were collated,including the general conditions of the patients(gender,age,course of disease,family history,etc.),clinical manifestations(skin lesions,tongue texture,tongue coating,pulse,etc.),relevant laboratory tests(blood routine,liver and kidney function,immune function,etc.),and therapeutic medication(oral Chinese herbal decoction,topical drugs,etc.)data.The EXCEL database was established and entered into SPSS 26.0 software for statistical analysis such as frequency and frequency.The collected oral Chinese herbal decoction was entered into the ancient and modern medical record cloud platform(institutional version)for TCM frequency,four qi,five flavors,meridians,efficacy,and cluster analysis.Results:1.General conditions: A total of 163 patients were included in this study,99 males(60.74%)and 64 females(39.26%).The youngest inpatient was 14 years old and the oldest was 83 years old,with an average of 49.99 ± 16.90 years.The age of initial onset was20-29 years with the highest number of cases.The most patients(48 cases,29.44%)had a course of disease longer than 15 years.The most predisposing factors were above respiratory tract infection in 27 cases(16.58%).There were 28 cases(17.17%)of smoking history and 20cases(12.27%)of drinking history,which were more male than female(P < 0.05).There were15 patients with a family history of psoriasis(9.20%),and the early onset was more than the late onset(P < 0.05),and the first onset age of patients with positive family history of psoriasis was earlier than that of patients without family history of psoriasis(P < 0.05).2.Comorbidities: Common comorbidities were dyslipidemia,hypertension,diabetes,and cardiovascular disease.There were significant differences in hypertension,diabetes,respiratory disease and cerebrovascular disease between early-onset and late-onset patients(P< 0.05).3.Clinical manifestations: The skin lesions were widely distributed,and the top ones were trunk & limbs(131 cases,80.36%),followed by scalp(128 cases,78.53%).the skin lesions were distributed on the scalp in the elderly and non-elderly groups with differences(P < 0.05).Most of the tongue symptoms were red(144 cases,88.34%)and yellow(87 cases,53.53%),and most of the pulse symptoms were slippage(69 cases,42.33%).4.TCM treatment: A total of 158 oral Chinese herbal decoctions were collected in this study,and the top 3 medicinal properties of all Chinese herbs were cold,slightly cold,and flat;the top 3 medicinal tastes were bitter,sweet,and pungent;the top 3 were liver,heart,and lung;the top 3 drug efficacy were clearing away heat and toxic substances,clearing away heat and cooling blood,and cooling blood to stop bleeding;cluster analysis of the top 20 herbs was clustered into 3 categories,the first: Silybum marianum,Scrophularia,Tribulus terrestris,Atractylodes macrocephala,and Poria cocos;the second category: Salvia miltiorrhiza,Scutellaria barbata,Fructus Mume,Forsythia suspensa,and Lonicera japonica;the third category: Soil poria cocos,Radix arnebiae seu lithospermi,Caulis spatholobi,Cogongrass rhizome,Sophora japonica,Hedyotis diffusa,Cortex moutan,Radix paeoniae rubra,Raw radix rehmanniae,liquorice.5.Treatment effect:136 cases(83.44%)were markedly effective,25 cases(15.34%)were effective,2 cases(1.22%)were ineffective,with an overall response rate of 98.78%.Conclusion:1.In this study,psoriasis vulgaris with blood-heat syndrome was more common in males than in females,the age of initial onset was concentrated in young adults,the age of initial onset was earlier in patients with positive family history than in those without family history.2.In this study,psoriasis vulgaris with blood-heat syndrome have certain correlation with metabolic diseases.3.In this study,the cold and bitter taste of TCM decoction was the first,the liver meridian was the first,and the efficacy was to clear heat and detoxify and cool blood.The drugs were grouped into 3 categories by cluster analysis.The first category of drugs was suitable for patients with severe pruritus of psoriasis with blood heat and spleen deficiency;The second type of drugs are suitable for psoriasis blood heat syndrome due to exogenous wind heat accumulate poison into the blood induced;The third class of drugs can prevent the transition to blood stasis and blood dryness while treating blood heat syndrome.4.The treatment of psoriasis vulgaris with blood-heat syndrome has a good effect. |
