| Purpose:Through the integration of the clinical data of patients with chronic urticaria in our hospital in the past 5 years,a retrospective analysis was made to explore the clinical data characteristics,pathogenic factors,laboratory examination and treatment of traditional Chinese and western medicine of patients with chronic urticaria in our hospital.And then summarize and analyze the relevant clinical experience,in order to provide reference for clinical treatment.Material and method:The main purpose of this study is to collect the data of patients who were hospitalized in the Department of Dermatology of the First Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from October 2017 to September 2022.The clinical data of 226 patients with chronic urticaria who met the inclusion criteria were studied and analyzed retrospectively.Such as: the general condition of the patient(including sex,age,occupation,etc.),disease-related factors(such as onset season,solar terms,past medical history,history of allergy,etc.),relevant laboratory examination indicators,patient’s tongue and pulse,TCM syndrome types and treatment methods of traditional Chinese medicine and western medicine.The clinical data of the patients were recorded into the Excel table,and the frequency and proportion of disease-related factors were analyzed by IBM SPSS25.0 statistical software.IBM SPSS Modeler18.0 was used to analyze the medication rules of traditional Chinese medicine prescriptions.Results:1.General data: Among the 226 patients in this study,there were 74 males(67.26%)and 152females(32.74%).The age of the patients ranged from 19 to 87 years old,and the average age was 53.09 ±13.55 years old.Most of the patients were middle-aged and elderly women aged50-69 years old.The average course of disease was 5.84 ±9.72 years.In this study,the disease mainly occurred in summer(73 cases,32.3%),and often occurred in Lesser Heat season.The occupations of patients were mostly retired,with a total of 104(46.02%).2.Disease-related situation:Patients with history of smoking and drinking accounted for11.5% and 15.0% of the sample,respectively,and there were more males than females.The patients without inducement are the most,while the patients with inducement are mainly caused by diet,environmental factors and drugs.In the past history of allergy,there are more patients with drug allergy,and most of them are penicillin allergy.The disease history is mainly circulatory system diseases and endocrine system diseases,and hypertension,diabetes and thyroid diseases are the most common.Eczema was the most common skin diseases in the past.3.Clinical manifestations:The main skin lesions of the disease are wind mass and itching,positive for dermatographism.In severe cases,it is often accompanied by dyspnea and chest tightness.The main tongue images were red tongue(177 cases,78.32%)and thin yellow fur(104 cases,46.02%).The pulse was mostly floating pulse(130 cases,57.52%).The syndrome type is mainly wind-heat syndrome,and it is more common in women.4.Laboratory tests: Most patients who test for special allergens are allergic to only one component.Among them,house dust mite / Dermatophagoides farinae was the most in the inhalation group(43 cases,25.75%),and wheat(15.57%)was the most in the food group.The positive rate of allergen in inhalation group was higher than that in food group.Most of the positive species of food intolerance were 1,and most of them were eggs(mild)(63 cases,35.2%).There were 157 patients with total Ig E≥100IU/ml(70.09%),and the increase of Ig E in females was more common than that in males.The results of immunoassay were abnormal,and the values of Ig G and Ig M were significantly different from those of sex.5.Traditional Chinese medicine treatment: 202 prescriptions collected were statistically analyzed.Fangfeng,Glycyrrhiza uralensis,Radix Glycyrrhizae,Cortex moutan,Cortex moutan and other drugs are most commonly used in all drugs.And Fangfeng is the most frequently used drug.The frequency of medication is divided according to syndrome type.Fangfeng,Shengdi,Glycyrrhiza uralensis,Cortex moutan and fresh bark were widely used in the syndrome of wind-heat violation.The syndrome of wind-cold bundle shows that ramulus cinnamomi,Fangfeng,Angelica sinensis,Ephedra and Radix Astragali are widely used.The syndrome of blood deficiency and wind dryness is widely used in Fangfeng,Angelica sinensis,Radix Glycyrrhizae and Schizonepeta tenuifolia.The medicinal properties are mainly cold and lukewarm,and the taste is mainly sweet,bitter and pungent.The high frequency is composed of spleen meridian,lung meridian,liver meridian,heart meridian and stomach meridian.The strong links related to traditional Chinese medicine are Fangfeng-licorice,Fangfeng-raw land,licorice-raw land and so on.The drugs with high confidence are licorice-Fangfeng,Huangqi-Fangfeng,Honeysuckle-Fangfeng and so on.The three combinations with high confidence were Shengdi + Glycyrrhiza + Fangfeng,Moutan +Glycyrrhiza + Fangfeng,White fresh Bark + Glycyrrhiza + Fangfeng and so on.The cluster analysis of high-frequency drugs is divided into five categories,which reflects the main prescriptions and essential medicines for the treatment of CU,such as Xiaofeng San,Zhaobingnan Jingfang Formula,Yupingfeng San,Danggui Yinzi and so on,and embodies the treatment methods of CU,such as clearing heat,dispelling wind and relieving itching,invigorating the spleen and replenishing qi and solidifying the surface.6.Curative effect: 224 patients were effective in the treatment of skin lesions,and the total effective rate was 99.12%.Conclusion:1.In this study,most of the patients with CU are middle-aged and elderly women,and the syndrome type is mainly wind-heat syndrome,and the disease often occurs in summer.2.In this study,hypertension,diabetes and thyroid diseases are the most common concomitant diseases.3.In this study,the positive rate of allergen in inhalation group was higher than that in food group in CU patients.4.In this study,heat-clearing drugs,deficiency-tonifying drugs and surface-relieving drugs are commonly used in the treatment of CU.Single traditional Chinese medicine mainly uses Fangfeng,licorice,raw land,peony bark,white fresh bark and so on.Medicine for the use of Fangfeng-licorice,Fangfeng-raw land,licorice-raw land and so on. |
