ObjectiveTo assess the efficacy and invasion of the cordectomy with suspension laryn-goscopy by the KTP (532nm) laser.MethodsAnalysis of 100 laryngeal cancer patients who underwent the cordectomy by laser from 1997 through May 2002. There were 96 patients with T1aN0M0, 4 with T1bN0M0(The staging system of UICC in 1997) , 92 males and 8 females. 16 patients underwent type I of cordectomy which resect the mucosa of vocal cord and the portion of vocal ligament. 63 patients underwent type E of cordectomy which resect the mucosa of vocal cord, vocal ligament and the portion of vocal musles ,and also including ventricular fold. 21 patients underwent type Iff of cordectomy which resect the mucosa of vocal cord , vocal ligament, vocal muscles and paraglotic space. The relapse rate and death rate were followed up.Results8 patients were followed up for 5 years, 53 patients for 3 years, 86 patients for 2 years, 100 patients for 1 years. 1 patient relapsed in 2 months after operation , 2 patients in 3 months, 1 patients in 6 months. Partial laryngectomy weredone for these 4 patients again and relapse were not seen over 2-4 years. 1 patient relapsed in 1 year after operation and total laryngectomy was done. 5 years of cumulative relapse rate was 5. 8% in these patients. Two years of survival rate were 100% and five years of survival rate were 97%.ConclusionsThe KIP laser cordectomy as a minimal trauma treatment was the radical operation for the earlier period laryngeal cancer. These were important for increasing the curative effect, including strict adaptation, correctly selecting the type of operation and practiced skill.
