Malignant tumor of liver has become one of the most common diseases, which threaten peoples' health, especially its morbidity and mortality is the 2nd highest in malignant tumor in China. In the western country, metastatic tumor is far more than primary carcinoma of liver and it always metastases from colorectal cancer and maybe the only metastatic focal lesion, so the patient is curable if the metastatic focal lesion is removed. With regard to plan, surgical resection is still generally be deemed to be the best measure for hepatic malignant tumor. However, the surgical resection rate of primary liver carcinoma is only about 20-30% and recurrence and metastasis are common even after resection. As to metastatic tumor of liver, the focuses are often multiple and they have a much lower surgical resection rate about 5-20%. For this reason, to find some non-operative therapies has become a hot spot in domestic and abroad. The new ways of interventional ultrasonic technology to treat hepatic malignant tumor are coming forth continually along with the development of radiologic and ultrasonic technology. And they become more and more mature. The interventional way has already been an important method of clinical therapy.Interventional ultrasonic therapies such as ultrasound guiding slender needle injection have already been widely used. Percutaneous ethanol injection therapy (PEIT) is one of the typical representations. Recently, new liquor injection has appeared one by one, such as acetic acid, high temperature distilled water, hightemperature saline, radioactive agent injection like 90Y and so on. But with the cases accumulating, the injection therapy's shortcoming has emerged. The main problem is that the diffusion range is limited and influenced by interstitials so the cancer cell can't be killed by once. It would add the risk of metastasis and local recurrence. It was approved by various way that there are still rudimental cancer tissue sub-envelope, extra-envelope, intra-envelope although the majority of tumor was necrotic.The other interventional ultrasonic therapies are importing energy into the neoplastic tissue by ultrasound guiding. Which is using microwave, laser, electricity and etc to import energy into the tumors and transfer them into heat then that high temperature would cause neoplastic tissue necrosis. In these therapies the needles using at present are thick needles (14G~16G) and the treatment risk increased obviously when there's server liver cirrhosis combined with ascites, great vessels or other important constitutions in front of tumor. HIFU is a new non-operative technology to cure tumor, but yet has shortcomings to overcome such as how to decrease the interferences come from high- echo tissue (such as skeleton, pulmonary alveolus and intestines gas) and how to avoid interferences cause by breathing and etc.The objective of this study is to find a more controllable and cancer-killed thoroughly inter-tumor injection, fewer times of therapy and greatly increasing patients' survival rate base on security.Japanese scholar named Tamai T reported low concentration alkaline injection into rat's liver, and compared the necrotic areas in the livers of rats that had received percutaneous low concentration alkaline injection therapy (PLAIT) with those that had received percutaneous ethanol injection therapy (PEIT) and percutaneous acetic acid injection therapy (PAIT) in 2000. The necrotic area increased with increasing concentrations of alkaline solution. The survival rate of rats decreased with the concentration of alkaline solution increased. They conclude that PLAIT has promised to be a new form of local therapy for HCC. In the same year domestic scholar Liu limin reported a research of sodium hydroxide solution targeting tumor and they concluded that sodium hydroxide solution had well corrosion ability, easily permeating, less resident of cancer cell and less side effect at a fixed dose.
