ObjectiveResearch on non-thrombolysis therapeutic time window and intervention of compound prescription Zhongfengxingnao oral liquid in a cute cerebral infarction (ZFXN, the proved recipe of my tutor prof Chen Shao-hong)Methods1. Clinical research(1) Retrospective contrast!ve analysis were applied to statistical analysis of 462 in-patients who had ACI and treated by ZFXN in Chengdu TCM Affiliated Hospital and 481 in-patients with same disease in Gansu Zhangye Peoph's Hospital from January 1999 to December 2003, and described in detail all those in-patients.(2) The age , sex, part of infarct, anamnesis scale , concomitant diseases scale, National Institute of Health Stroke Scale (NIHSS), Glasgow Coma Scale (GCS), Barthel Index Scale (BI) and the time from stroke to seeing doctor were compared by stratified baseline regularize test with X-test, t-teat, u-test and variance analysis.(3) Evaluated these two groups about the case fatality rate, National Institute of Health Stroke Scale (NIHSS), Glasgow Coma Scale (GCS) , Barthel Index Scale(BI) and the relationship between the curative effect and deferent therapeutic time windows by variance analysis.(4) Related factors about Mortality, Barthel Index Scale (BI), recurrence and the best therapeutic time window of ACI were analyzed with logistic regression.2 Experimental research(1) The serum pharmacology and CSF pharmacology were applied to get CSF and serum of Beagle-dog and serum of SD model rate containing herb.(2) Cultured in vitro the cerebral cortex nerve cells of newborn. SD rats and appraised the purity of nerve cell.(3) The isolated models of ischemic and anoxic injury with glutamic acid (Glu) were established, and studied the injuried nerve cells using CSF of Beagle-dog and serum of SD rate containing herb.(4) Screened the best density of CSF of Beagle dog and serum of SD rats containing herb by MTT and testing OD.(5) Detected cell energy, lactate dehydrogenase (LDH) leaking and apoptosis of CSF and serum with best density of herb, and morphology, MTT ,FGM(Flow Cytometry) were used in studying the effect of ZFXN on primary culture cortex nerve cell with ischemic injury caused by antagonism to glutamic acid (Glu)3. Summary3. SummaryConsulting ancient TCM works and records about stroke, therapeutic time window and the ischemia injure by using the Chinese Medical Retrieval Optical Disk and MEDLINE database to search the correlated documents in recent ten years.Results1 Clinical research consequence(1) The compare of mortality : compared with control group, ZFXN could reduce mortality 90 days after ACI and there were significant difference (P<0.05), but no statistic difference between these therapeutic time windows.(2) The compare of Barthel Index Scale (BI): after 60 and 90days, the vs on BI of the patients who were treated by ZFXN in 24 hours after ACI were better than other groups, and were significant difference (P<0.05, P<0.01), but no difference of other time windows.(3) The compare of National Institute of Health Stroke Scale(NIHSS): after 90d the vs on NIHSS of the patients who were treated by ZFXN were much better than control group (P< 0.05, P<0.01); total effect after 60d had statistical value, but there were no difference in other therapeutic time window; the Compare between herbal and control were no significant difference after 30d and no statistic significance(P>0.05).(4) The compare of logistic regression:Among the factors which influenced Barthel Index Scale(BI) of the patients 90d after ACI were Glasgow Coma Scale (GCS) baseline, Concomitant Diseases Scale Baseline and therapeutic time window(P<0.05).It indicated that earlier treatment of ZFXN could enhance long-term life quality .The more serious the concomitant diseases scale baseline was, the worse prognosis was. It were therapeutic method and Glasgow Coma Scale (GCS) which influenced mortality of the patients after 90d (P<0.05, P<0.01) . Obviously, therapy with ZFXN could decrease mortality.
