The Curative Effects And The Mechanism Research In Treating UC By Coupling Use Of An-Chang-Yu-Yang-Tang With Sheng-Ji-San

Objective: The dissertation is attempt to find the theoretical bases of treating UC with TCM and discusses its possible pharmacodynamic mechanism through observing clinically and experimentally the effects of coupling use of An-Chang-Yu-Yang-Tang with Sheng-Ji-San (ACYYT and SJS) on UC.Methods: Seventy eight patients with the recrudesced or persisted type ulcerative colitis in active state were randomly divided into three groups: the ACYYT&SJS, the SJS and the Solfasalazine (SASP) group, then their clinic curative effects were observed after corresponding treatments. At the same time,the curative effects were checked and the curative mechanism was searched by the experiments of animal model.Results: According to the main observed indices including general clinical effect, curative effect of TCM syndrome, change of main clinical symptom, curative effect of mucous membrane under endoscopes, curative effect of mucous membrane under microscope ,the clinic results showed that all groups appeared to have certain positive effects, but the ACYYT&SJS group had the best result with a very significant statistic difference (P<0.01) .The animal experiments showed: First , the ACYYT and SJS have good effects of anti-inflammatory and relieve pain. Second,in the colon tissue of rat pathologic model group, the levels of IL-1B IL-8 TNF- a were clearly higher and the level of IL-10 was remarkably lower than that of the normal group ( P<0.01) . Second, compared to the pathologic model group, the levels of IL-1B , IL-8 and the TNF- a in both ACYYT and SJS group were low in some extents (P<0.01) and their IL-10 levels was obviously high (P<0.01) .Conclusions: In curing the recrudesced or persisted type ulcerative colitis in activestate, the ACYYT&SJS was both a favoring and effective way and a useful prescription .The mechanism of the ACYYT&SJS' effectiveness might be : ( 1 ) Decreasing the level of proinflammtory cytokine (IL-1B , IL-8, TNF- a) and raise the level of IL-10, which can then adjust the immure system and inhibit the inflammation reaction of abnormal enlarging and exacerbation in the course of UC. (2) the study of modern pharmacology show HuangQi and DangGui in the prescrintion have good effects of antagonizing the oxygen free radical, which can protect colon tissue from injury. The comprehensive curing method adopted in this subject combined the internal with the external treatment and djusted the whole together with the local. Such resolution conforms to the etiology and pathogenesis of UC and the treating principle of combining differentiating syndromes with differentiating disease.
