Background Nourishing Blood and Cleaning Brain Principle (NBCNP) developed from famous ancient Chinese herb recipe "Si Wu Tang Recipe ", made from Radix Angelicas Sinensis, Rhizoma Ligustic Changxion, Radix Rehmanniac pracptrta, Semen Cassiac and Herba Asari, etc. The principal effects are enriching the blood and nourishing Yi as well as claming the liver-Yang hyperfunction to suppress hyperactivity of the liver-Yang, and promoting blood circulation and the flow of Qi. It is applied to cure headache caused by the Yi (blood)-insufficiency and liver-Yang hyperfunction. The prescription has been found antihypertensive effect recently. In order to studing the effect and mechanism of NBCNP on hypertension and target organs damage, following studies to be done.Methods and Results Clinic Studies: Thirty three patients with essential hypertension(EH) of stages II were treated with NBCNP for 8 weeks. Systolic blood pressure(SBP), diastoleic blood pressure(DBP) and pulse pressure(PP) were measured before and after treatment. The results showed that systolic blood pressure and pulse pressure were significantly lower after treatment than which were before treatment (p<0.05). The total marks of symptoms such as headache,dizziness,weakenss and soreness of waist and knee,insomnia and dreaminess,vexation induced by the EH with Yin-Deficiency and Yang-Excess syndrome were calculated. The results showed that total marks of symptoms reduced significantly after treatment(p<0.05). The total efficient rate of treating the symptoms was 96.77%.The serum ET, CGRP and Ang II concentration were measured before and after treatment, the results showed that ET and Ang II level were reduced remarkable, while CGRP concentration was slightily increased. All patients were given 24-hour ambulatory blood pressure monitoring(ABPM) before and after treatment. The trough to peak ratio(T/P ratio) of SBP and DBP and blood pressure loads(BPL) were calculated, and the circadian rhythm of blood pressure were observed at same time. The results showed that T/P ratio of SBP was higher than 50% after treatment, and SBPL was significantly lower than it was before treatment(p<0.05), non-dipper blood pressure phenomenon increased. Transcranial Doppler cerebral blood flow image was perform, the systoli velocity(Vs),mean velocity(Vm),diastolic velocity(Vd), pulsatility index(PI) and resistant index(RI) of middle cerebral artery(MCA) were measured. The results showed that Vm and PI were significantly lower after treatment(p<0.05). The large and small arterial elastic indexes ( Ci, C2), and automatic pulse wave velocity(PWV) were measured before and after treatment, the results showed that C2 was significantly increased after therapy(p<0.05), while a little change for Ci and PWV.Experimental Studies: Sixty spontaneous hypertension rats(SHR) were randomly divided into six groups, NBCNP high dose group(n=10,31.5g/kg/d), NBCNG middle dose group(n=10,15.7g/kg/d), NBCNP low dose group(n=10,7.8g/kg/d), model group(n=10), Niu Huang jiang Ya Wan group(n=10,0.168g/kg/d) and Captopril group(n=10,7.785g/kg/d). The results showed that SBP of high dose group reduced significantly compared with the model group (p<0.01), the antihypertensive effect was remaining and lasting at whole time. The left ventricular mass(LVM), the left ventricular mass index(LVMI), the content of left ventricular myocardical interstitial collagen and the collagen ratio of type I to type III at different dose groups of NBCNP and model group were measured. The results showed that LVM and LVMI of high dose group reduced remarkly compared to model group (p<0.05), the content of left ventricular myocardical collagen was lower than model group, the deposition of left ventricular myocardical interstitial collagen of high dose group decreased pronouncedly compared to model group. The genes different expression pattern in heart and kidney of high dose group of NBCNP and model group of SHR were observed and analysed.
