Study Purpose:According to the characteristic of dizziness of vertebrobasilar insufficiency(VBI) and the research of medical scientists in successive dynasties, combine the medication practical experience of dizziness of VBI of tutor who use traditional Chinese medicinal herbs to promote blood circulation, probe into the cause of disease and VBI cure . Study the change of the active material of blood vessel of VBI, and the impact on blood vessel function; Regard holding the pathology theory of LUO in Chinese medicine as the breakthrough point, probe into the connection between promoting LUO and regulating the functions of blood vessel openly, by examing TCD, soft little circulation of meninx, ET , NO , AngⅡ , bFGF , PDGF , NF - K B ,etc, probe into the curative effect mechanism of QingXuanKeLi. Study Method:1, Clinical researchChoose 60 of VBI patient, divide into two group at random: test group and contrast group each 30, using QingXuanKeLi and Ginkgo tablet to treat separately for 4 weeks. Observe the curative effect on Chinese syndrome , the curative effect of the individual symptom, the TCD, multiple indexes, such as plasma ET, Ang II , NO content ,etc.2, The experiment study:Use high fat diet ± vertebra artery oppressed model of VBI set up by big mouse,divide into: high dose QingXuanKeLi group , low dose QingXuanKeLi group, Ginkgo tablet contrast group, model contrast group. Set up the blank contrast group separately . Drill cranium window which diameter is 4 X 4mm on big mouse right side temporal lobe base bottom with dental bit , observe the soft little circulation of meninx; Use the immunohistochemical quantitatively technique etc .measure NF - K B , FGF-2 , PDGF-B , NO , ET, Ang II , t-PA, PAI, blood platelet gathering , blood rheology, fat of blood ,etc. Study Result:1, Clinical ResearchCurative effect on Chinese syndrome of test group is 86.67%; Contrast group is 66. 67% , the comparison of test group and Contrast group is obvious difference (P <0.05); accumulating mark of the individual symptom of test group are remarkably or obviously reduced (P <0.01 or P <0.05). Improving the index of TCD (Vd , Vm , Vs , PI , RI ), controling systolic pressure -. blood rheology index (fibrinogen , high shear rate of the whole blood viscidity), reducing content of plasma ET and Ang II, it is obviously that the test group is superior to contrast group (P <0.05); Regulating fat of blood , rising the level of NO ,it is not have obvious difference to compare between test group and contrast group (P> 0.05).2, Experiment ResearchThe function of suppressing the expression of NF - K B , FGF-2 , PDGF-B of high dose QingXuanKeLi group is superior to Ginkgoes tablet contrast group (P <0.05);.The comparison of plasma NO , ET , Ang II content between high dose QingXuanKeLi group and Ginkgo tablet contrast group is obviously different (P <0.05). QingXuanKeLi can rise t-PA , reduce PAI -. fibrinogen .The function of high dose QingXuanKeLi is remarkably superior to Ginkgoes tablet (P<0.01), The function of low dose QingXuanKeLi is obviously superior to Ginkgoes tablet (P <0.05). High shear rate of the whole blood viscidity of high dose QingXuanKeLi group and low dose QingXuanKeLi group contrasted with Ginkgo tablet group,is remarkably different (P <0.01).The improvement of soft meninx circulation of high dose QingXuanKeLi group and low dose QingXuanKeLi group have remarkable or obvious difference with Ginkgo tablet contrast group (P <0.01 or P <0.05).
