This Doctoral Thesis consists of review articles of literature, experimental and clinical studies. The experimental studies were conducted to probe the possibility of sutural expansion osteogenesis for repair of cleft palate and hard palate lengthening. 7 articles were enclosed in the experimental studies. Suture expansion for the closure of cleft palate: An experimental study in dogsThe purpose of this study is to probe a new approach for the repair of cleft palate by use of the technique of suture expansion. By this technique we might hope to replace missing tissue in the clefts by tissue regeneration and achieve bony repair in the posterior hard palate and avoid the problems of speech and facial growth seen in traditional cleft surgery. Materials and methods: Nine 8-month mongrel dogs were used in the study. In six experimental dogs, an 8mm wide cleft was made by surgery. Suture expander made in NiTi-shape memory alloy with force levels of 200g, 360g and 480g were secured in the palatine bones. Results: The clefts in the experimental group were closed completely sooner or later in a period from 5 to 14days. While the clefts in the control group became a little larger than their original size. There was significant new bone formation at the edge of the expanded suture. Conclusion: The clefts in young dogs could be closed by the technique of suture expansion. NiTi-SMA expander used in the study was consistently effective and all the force levels could induce a great mount of suture new bone formation with the trabeculae thinner and longer in heavy forces.â…¡. Suture expansion for cleft palate closure: A serial histological study.In order to know the process of tissue response and regeneration of sutural expansion, sutural tissue changes after 3,7,14,28,56 days suture expansion were examined histologically and fluorescently. It was found that a zone of cystlike appeared in the conjuncture of bony front and sutural connective tissue at the early stage of sutural expansion, then a great amount of osteoblasts and fibroblasts proliferated. New bone apposited along the septum and the front of the cyst, until the new bone front formed and the suture restored its original morphology.â…¢.
