Objective:To study the clinical experiences of syndrome differentiation and application of meidicinals by the famous doctor, Zheng Shouquan, in Qing Dynasty, especially for yang deficiency syndrome, so as to elucidate his academic theories and experiences about application of warm and hot medicinals.Methods:The characteristics of syndrome differentiation by Zheng Shouquan were investigated by the way of documentary annotation and explanation, especially for yang deficiency syndrome, and the key points were extracted. Then, data in his book of Yi Fa Yuan Tong were collected including the methods for syndrome differentiation, key points of yang deficiency syndrome, classifications and frequencies of medicinal application, yang-warming formulas and medicinals, doses of warm and hot medicinal as well, and statistical data was analyzed to investigate the clinical experiences of syndrome differentiation and medicnal application, to elucidate the characteristics and regulations of application of warm and hot medicinals by Zheng Shouquan.â… The clinical syndrome differentiation and management by Zheng Shouquan were full of flexibilities and markedly based on clinical evidences. The key points for yang deficiency syndrome were primarily in the aspects of sufficiency and deficiency of body fluid, original qi and zang-fu activities.â…¡Medicinals used most frequently were tonics, exterior-releasing medicinals and interior-warming medicinals in book Yi Fa Yuan Tong. Interior-warming medicinals used most frequently, were fu zi and gan jiang.â…¢Yang-warming formulas used most frequently were Si Ni Tang, Bai Tong Tang, Ling Zhong Tang, Hui Yang Yin and so on. The combinations used most frequently were fu zi with gan jiang for all yang deficiency patterns, as the basic compatibility. â…£The average dose of Fu zi was from 7.4 grams to 37 grams. Fu zi was good at warming and transforming phlegm-damp, and controlling blood by supplementing yang when used in large dose, good at warming the interior, dispelling cold, and warming the center when used in moderate dose, and good at reinforcing the power of warming and transporting for others medicinals, and warming and strengthening the heart fire by supplementing the kidney fire when used in mild dose.â…¤Symptoms of healthy qi against pathogenic qi after taking warm medicinals, were signs of removal of yin pathogens by yang meidicnals. One doze of yin-nourishing medicinals might be applied when yin pathogen was expelled and yang qi was rehabilitated, to promote the mutual rooting of yin and yang.Conclusions:The syndrome differentiation by Zheng Shouquan is based on the guideline of yin-yang, comprehensive analysis of the four diagnostic methods and clinical manifestations. The characteristics of pattern identification for yang deficiency are predominantly innovative and insightful, especially in the aspect of application of fu zi. Its application bears clear criteria, suitable compatibilities and correct dose. Therefore, Zheng Shouquan is a reputational outstanding doctor underlining reinforcing yang to restrain yin and treating yang deficiency syndrome by warm and hot medicinals in late Qing Dynasty.
