Ankylosing spondilitis (AS) is a kind of autoimmune disease of the entire body characterised primarily by an attack on the joints of the spine. In Taiwan there are 40 000 patients with AS, and since the majority of patients do not know or are misdiagnosed by doctors, the result is a delay in the treatment of the disease. This disease has a great influence on the daily quality of life.The present research has two purposes. This first is the therapeutic aspect, observing the effects of therapy by my supervisor in the clinic, and inquiring into and summarising the experiences of my supervisor; the second is the physiotherapeutic aspect, exploring the therapeutic results of the writer's manipulation techniques.This article first summarises the understanding of AS in view of modern medicine and Chinese medicine. Concerning the aspect of modern medicine, the causes and principles, and genetic inheritance of the disease are considered, urogenital infection, and the structures of invasive germs and that of B27 in the duplicate arrangement of amino acids which cause misrecognition by the immune cells. Some B27 structures produce an antigen and are attacked by immune cells resulting in disease. Chinese medicine considers this disease to belong to impediment of the kidney or the bones, a stooped back, and lower back pain, with clinical assessment of cold blockage of the Governing Meridian, deficiency of liver and kidneys, pathogenic blockage of the Governing Meridian leading to internal heat, pathogenic damp leading to impediment, blockage of phlegm and impediments of bone and kidneys.Secondly, there is clinical research concerning a herbal formula, Qiangzhi Dingtong, for the treatment of AS. According to the standards of diagnosis in integrated Western and Chinese medicine, there is a division between an experimental and a control group, with one given the Qiangzhi Dingtong formula and the other, a standard Western drug, and compared after three months of treatment, with the effectiveness of treatment in the experimental group 92.85%, much better than that of the control group, at 72.72%.In the discussion section, the views of my supervisor concerning rigid spine AS are summarized: the cause of the disease is due to congenital deficiency, overexertion in work and sexual activity, erratic eating habits, loss of physiological regulation after the onset of disease and exacerbation by external pathogens and work; the pathological aspect is due to the confluence of wind, cold, damp, and heat phlegm accumulation, blockage of the vessel network; the location of disease is in the bones, the sinews and the vessel network, the pathological change is in the internal organs, primarily in the kidneys, liver, and spleen; the characteristic of the pathological change is obstruction of the vessel network, leading to pain.
