The theory of spleen and stomach is the central part of the viscera-state doctrine in the traditional Chinese medicine(TCM). Splenogastric damp heat syndrome is one of the most important sthenia syndromes,which is characterized with strong fight between vital qi and evil qi.It is reported that there exists close relationship between chronic gastritis belonging to splenogastric damp heat syndrome and Helicobacter pylori(HP).The associated-H,pylori chronic gastritis belonging to spleniogastric damp heat syndrome possesses special pathologic changes. Some people think that HP belongings to damp heat evil qi.Heat shock protein(HSP)belongs to stress proteins,which means that it can be induced by variable stressers.HSP70 is one of important members of HSPs.HSP70 can protect gastric mucosa from variable damages.Higher expression of HSPT0 in gastric mucosa has been found.It is reported that HSP70 can lessen HP.We think HSP70 plays a role as "vital qi"Nuclear factor kappa B(NF-κB)exists in variable tissues.It exists in somatoplasm and has no activition as usual.It will enter nucleus and be activated by lots of stimulus.It is related to HP.NF-κB is the central of associated-H,pylori gastritis.NF-κB can induce the expression of tumor necrosis factor alpha(TNF-α)and interleukin 8(IL-8),which can lead to the inflammation and the damage of gastric mucosa.We think NF-κB play a role as "evil qi".It is reported that HSP70 can compete the path entering nucleus with NF-κB.So HSP70 can inhibit the activation of NF-κBObjective:To search for the mechanism of associated-H,pylori chronic gastritis belonging to splenogastric damp heat syndrome through studing the relationship of HSP70 and NF-κB.We made some chronic gastritis patients belonging to splenic asthenia syndrome as control group so as to explain the theory of "the same disease with different:syndromes" And we hope the study will provide basis for the therapy with Chinese medicine.Experimentl Relationship between Associated-H.pylori Chronic Gastritis belonging to Splenogastric Damp Heat Syndrome and The Level of InflammationMethod:1 We choosed 51 chronic gastritis patients including 36 cases belonging to splenogastric damp heat syndrome and 15 cases belonging to splenic asthenia syndrome.In the 36 cases belonging to splenogastric damp heat syndrome,there were 19 cases infected by HP and 17 cases without HP.And we recruited 8 healthy people as normal control group.2 We got the mucous membrane of all objects in gastric antrum part under the help of electronic gastroscope.The inflammation situation of gastric mucosa was diagnosed through electronic gastroscope and common pathological staining technology.HP was detected through Methylthioninium Chloride staining andrapid urease test.3 We analyzed all of the results.Result:1 Analysis of Common Materials and The Infection of HPThe difference has statistical significance on sex,age,the course of disease and the rate of HP(P>0.05)in eVery group.So we thought that all of the groups can be compared each other.2 Relationship between HP and The Symptoms of Chronic Gastritis belonging to Splenogastric Damp Heat SyndromeIn the chronic gastritis belonging to splenogastric damp heat syndrome group,the difference has no statistical significance between HP-positive and HP-negative on the symptoms including yellow greasy tongue coating,stomach pain or gastric distention,anorexia,dry mouth or bitter taste of mouth,nausea,loose stool(P>0.05).3 Analysis of The Level of InflammationThe difference has statistical significance between chronic gastritis group and normal control group(P<0.05).The inflammation of chronic gastritis group is more severe.The difference has statistical significance between splenogastric damp heat syndrome and splenic asthenia syndrome(P<0.05)and splenogastric damp heat syndrome is more severe than splenic asthenia syndrome.In splenogastric damp heat syndrome,the difference has statistical significance between HP-positive group and HP-negative group(P<0.05)and HP-positive group is more severe than HP-negative group.4 Analysis of The Activity of InflammationThe difference has statistical significance between splenogastric damp heat syndrome group and normal control group(P<0.05)and splenogastric damp heat syndrome group is more severe than normal control group.In splenogastric damp heat syndrome group,the difference has statistical significance between HP-positive group and HP-negative group and HP-positive group is more severe than HP-negative group.5 Analysis of the Correlation between HP and The Level or Activity of Inflammation.There exists positive correlation between the level of inflammation and the level of HP.There exists positive correlation between the level of inflammation activation and the level of HP.Experiment2 Relationship between HP and The Proteinic Expression of HSP70 and NF-κB in The Gastric Mucosa of Chronic Gastritis belonging to Splenogastric Damp Heat SyndromeMethod:1 The cases were the same to experiment 1.2 We detected the expression of HSP70,NF-κB,IL-8 and TNF-αin gastric mucosa through immunohistochemistry(IHH)technology.Result:1 The Expression of HSP70The difference has statistical significance between splenogastric damp heat syndrome and splenic asthenia(P<0.05)and the expression of HSP70 in splenogastric damp heat syndrome group is higher than splenic asthenia syndrome group.There is no statistical significance between splenogastric damp heat syndrome group and normal control group(P>0.05). In splenogastric damp heat syndrome,there is statistical significance between HP-positive group and HP-negative group(P<0.05).The expression of HSP70 in HP-positive group is higher than HP-negative group.2 The Expression of NF-κBThe difference has statistical significance between splenogastric damp heat syndrome and splenic asthenia(P<0.05)and the expression of NF-κB in splenogastric damp heat syndrome group is higher than splenic asthenia syndrome group.The difference has no statistical significance between splenogastric damp heat syndrome group and normal control group (P>0.05).In splenogastric damp heat syndrome,the difference has statistical significance between HP-positive group and HP-negative group (P<0.05).The expression of NF-κB in HP-positive group is higher than HP-negative group.3 The Expression of IL-8The difference has statistical significance between splenogastric damp heat syndrome and splenic asthenia(P<0.05)and the expression of IL-8 in splenogastric damp heat syndrome group is higher than splenic asthenia syndrome group.The difference has no statistical significance between splenogastric damp heat syndrome group and normal control group (P>0.05).In splenogastric damp heat syndrome group,The difference has no statistical significance between HP-positive and HP-negative group (P>0.05).4 The Expression of TNF-αThere is significant difference between splenogastric damp heat syndrome and splenic asthenia(P<0.05)and the expression of TNF-αin splenogastric damp heat syndrome group is higher than splenic asthenia syndrome group.There is no significant difference between splenogastric damp heat syndrome group and normal control group(P>0.05).In splenogastric damp heat syndrome group,there is significant difference between HP-positive group and normal control group(P<0.05).The expression of TNF-αin HP-positive group is higher than normal control group.There is no significant difference between HP-positive and HP-negative group(P>0.05).5 Analysis of The Correlation between The Level of HP and The Expression of HSP70,NF-κB,IL-8 and TNF-αThere is positive correlation between the level of HP and the expression of HSPT0,NF-κB,IL-8.There is no correlation between the level of HP and the level of TNF-α.6 Analysis of The Correlation between The Proteinic Expression of HSP70 and NF-κB in Splenogastric Damp Heat SyndromeThere exists positive correlation between HSP70 and NF-κB in splenogastric damp heat syndrome and splenogastric damp heat syndrome with HP-negative group.Experiment3 Relationship of HP and The Expression of HSP70mRNA and NF-κBmRNA in Gastric Mueosa of Chronic Gastritis belonging to Splenogastric Damp Heat SyndromeMethod:1 The cases were the same to experiment 1.2 We detected the expression of HSP70mRNA and NF-κBmRNA through FQ-PCR.Result:1 The Expression of HSP70mRNAThe difference has statistical significance between splenogastric damp heat syndrome group and normal control group(P<0.05).The expression of HSP70mRNA in splenogastric damp heat syndrome group is higher than normal control group.In splenogastric damp heat syndrome,the difference has no statistical significance between HP-positive group and HP-negative group(P>0.05),and the difference has statistical significance between HP-negative group and normal control group(P<0.05)and the expression of HSP70mRNA in HP-negative group is higher than normal Control group.2 The Expression of NF-κBmRNAThe difference has statistical significance between splenogastric damp heat syndrome group and splenic asthenia syndrome group(P<0.05). The expression of NF-κBmRNA in splenogastric damp heat syndrome group is higher than splenic asthenia group.The difference has statistical significance between splenogastric damp heat syndrome group and normal control group(P<0.05).The expression of NF-κBmRNA in splenogastric damp heat syndrome group is higher than normal control group.In splenogastric damp heat syndrome,The difference has no statistical significance between HP-postive group and HP-negative group(P>0.05),and t The difference has statistical significance between HP-negative group and normal control group(P<0.05),and the expression of NF-κBmRNA in HP-negative group is higher than normal control group.3 Analysis of The Correlation Between The Level of HP and The Expression of HSPT0mRNA,NF-κBmRNAThere doesn't exist correlation between the level of HP and the expression of HSP70 mRNA,NF-κBmRNA4 Analysis of The Correlation Between HSP70mRNA and NF-κBmRNA in Splenogastric Damp Heat SyndromeThere exists positive correlation between HSP70 and NF-κB in splenogastric damp heat syndrome and splenogastric damp heat syndrome with HP-negative group.Conclusion:1 There is no significant correlation between HP and The symptoms of chronic gastritis to splenogastric damp heat syndrome.The level of inflammation in associated-H,pylori chronic gastritis group belonging to splenogastric damp heat syndrome is the most severe.The level of inflammation is related to splenogastric damp heat syndrome and HP.2 It is possible that splenogastric damp heat syndrome and HP can induce higher proteinic expression of HSP70 and NF-κB.3 There is tendency that splenogastric damp heat syndrome could induce higer expression of HSPT0mRNA and NF-κB mRNA.4 There is tendency that splenogastric damp heat syndrome and HP could induce higer expression of IL-8 and TNF-α5 The level of chronic gastritis belonging to splenogastric damp heat syndrome is very severe,which may indicate that vital qi and evil qi of splenogastric damp heat syndrome are fighting very severe.HSP70 which could protect the mucosa from damage may personalize the effect of vital qi.NF-κB,IL-8 and TNF-αwhich could induce the damage of mucosa may personalize the effect of evil qi.In splenogastric damp heat syndrome, the level of inflammation in HP-positive group is more severe than HP-negative group,which may indicate that the evil qi in HP-positive group is stronger than HP-negative group.So the fight of vital qi and evil qi is more severe in HP-positive group than HP-negative group.
