| Acupuncture treatment for Obesity is very effective and long lasting with less side-effect method to loss weight,it is inexpensive and acceptable.In recent years,acupuncture obesity in clinicaland laboratory research has made great progress.This study is used the traditional method of catgut implantation treatment,to seeking a long term method to have a lasting effects, simple to operate,and less pain method to treat obesity.Objective:To explore the effective acupoint by using catgut embedding treatment for weight loss,and further embedding method points clear mechanism of weight loss,this study is in the tree areas of a literature review,clinical research and experimental study.Methods:the literature has Systematical reviewed of obesity in epidemiology, pathogenesis and pathological changes;it also included the pathogenesis of the traditional Chinese medical ways of treating obesity.From the Clinical and Experimental Research is to come up the rule of the best acupuncture treatment for obesity.All the literature results show that obesity has the common factors of genetic,environmental and certain biochemical caused by a chronic eating disorders and metabolic control of the disease,the incidence of more complicated,the exact mechanism is unclear.In the present,drug therapy is focus on the target of energy intake,energy consumption or other energy storage systems,mainly in the following categories:loss of appetite inhibitors;metabolism enhancers;hinder digestion and absorption agent; insulin sensitizer;lowering drugs such as lipid.But there are certain restrictions on the use and have more side effects.Chinese medicine treatment of obesity research has a unique advantage by using the acupuncture method such as body acupuncture,ear acupuncture,catgut embedding therapy in clinical of wide range.The applications treatment and its mechanism of acupuncture is mainly through the realization of a neural regulation of the endocrine system to control appetite,reduce the lipids.Clinical research:The case of a total of 60 cases,all the data is collected from the Guangzhou University of Traditional Chinese Medicine First Affiliated Hospital in August 2008 to May 2009.The weight patients come to the clinics for weight loss with abdominal obesity will included in this study.The 5th national Obesity Research Academic Council amended the "simple obesity and effects of diagnostic criteria" in 1997,it has confirmed by clinical examination for patients with simple obesity.Clinical studies is based on the observation of the obesity,it observed the effectiveness of using differentiation method of selected points and catgut embedding therapy.The evalution is base on the various indicators,such as(body weight,obesity degree,body mass index),measurements,blood lipids,blood glucose.The end result is to explore its mechanism.laboratory Study:It was randomly divided the obese rats into model control group,medication group,catgut embedding group put into the single-cage rearing to feed for a a week to adapt to the environment before the treatment. Both catgut embedding and drug treatment group were given a corresponding groups their treatment each day,but the control group gave no treatment at all.The catgut embedding group was implanted the catgut into acupuncture points zu san li,zhong wan after the sterilization.The length of the catgut is used by according to the appropriate deep of the acupuncture point of the tissue,and followed by setting a plaster after finished.In the process will Check the plaster every 3 days,if laid off then implanted the catgut back on and cover with plaster again.The catgut implant was done every Monday morning,and observed for 4 weeks.The medicinal group was using 35mg/kg of Xenical(Orlistat) by oral gavage administration,it also observed for 4 weeks.The laboratory study is observed the effect of rats weight by implanted through the catgut embedding method.The Lee's index was used as this study treatment effectiveness.It included the perirenal fat cells wet weight,as well as cholesterol,triglycerides as the indicators.Results:Clinical research:catgut embedding group compared the body weight differences with pre-treatment have significantly improve(p＜0.01),Xenical weight changed before and after treatment there was no significant difference (p＞0.05),compared the two groups after treatment there was a significant difference(p＜0.01).The two groups in the pre-treatment FPG,FINS,HOMA-IR was no significant difference(P＞0.05),catgut embedding group and medication group compared on FINS,HOMA-IR were statistically significant differences(P＜0.05).The treatment group on the impact of HOMA-IR with the control group were statistically significant differences(P＜0.05).The two groups' CHOL,TG,HDL,LDL were not significantly different before treatment(P＞0.05),and also in the normal range.Catgut embedding group and control groups' CHOL,TG before and after the treatment were lower,has the significant difference(P＜0.05),HDL rose,the difference was statistically significant(P＜0.05).Medicinal group in 30 cases were cured of 4(13.3%), effective rate of 21(70%),Invalid of 5(16.7%),the total effective rate of 83.3%.The Catgut embedding group in 30 cases were cured of 7(23.3%), effective rate of 22(73.4%),Invalid of 1(3.3%),total effective rate of 96.7%. In this study,the result of the Catgut embedding group was significantly better than the medication group.Comprehensive documentation of the statistic the catgut embedding group show this therapy has a simple way,less pain.The finding was positive effect,lasting,with no side effects,safety and reliability.In particular,the patients come only to the clinic for this therapy once a week and also reduce the number of visits,it is time-saving treatment of patients.Therefore,the majority of patients were more acceptable.Laboratory Study:rats' body weight had remarkable increased after being feeding the high-fat food.After two weeks treatments,the weight of the Catgut embedding group had almost no increased,but there is no significant difference between groups.The weight of the two treatment groups of the rats had significantly differences after 4 weeks.According to Lee's index,the catgut embedding group had significantly weight loss of(P＜0.01) after 4 weeks. After the treatment of different methods of fat wet weight and serum MDA content, medication group compared with the control group has P＜0.05;Catgut embedding group compared with the control group has P＜0.01.There was a significant difference.After different treatments,the total cholesterol and triglyceride levels and body weight of the control group compared to the treatment group had significantly increased.The two treatment groups had compared to the control group after 4 weeks.The rats' total cholesterol levels had no significant changes.But the triglyceride had reduced significantly.The medication group and control group has difference of P＜0.05.Catgut embedding group had compared to the control group on NPY, NO,NOS with significantly difference of(P＜0.01).From the literature, laboratory and the clinical study showed the catgut embedding treatment for obesity to loss weight has its mechanism for nerves system to control the endocrine regulation of food and water intake.The catgut embedding group was more effective than the medication group.For the result,it showed catgut embedding treatment was embedding into avariety acupuncture therapies,such as acupuncture,needles buried,and electrical needling.It has a wide range of stimulating role at the same time.The results of this study acupuncture in order to further clarify the pathogenesis of obesity and clinical research to provide an experimental basis and theoretical basis.Conclusion:1.Xenical treatment group and catgut embedding treatment group can reduce obesity patients' the body weight,BMI,waist decrease,Therefore,the two groups have a good therapeutic effect;which reduce the waistline,the catgut embedding group is better than embedding Xenical group(P＜0.01)。2.Through clinical observation and laboratory study confirm that Catgut embedding can lower levels of fasting insulin and improve insulin resistance may be effective in preventing glucose and lipid metabolism,and Catgut embedding is better than Xenical group(P＜0.01)。3.Through clinical observation and laboratory study confirm that the acupuncture points can regulate lipid metabolism embedding,so as CHOL,TG go back down,and HDL increased.4.Through clinical observation and laboratory study confirm that catgut embedding compared to Xenical group is more secure,effective and less toxic with less side effects.Further advance to be promoted and applicable.5.Through clinical observation and animal experiments confirm that catgut embedding group was superior to the long-term effect of Xenical group.Catgut embedding group has long lasting effects after treatment.The effectiveness of the catgut embedding has the ability to continuity of the disease,after cessation of treatment.
