The aim of this study was to measure the anatomic features of the tibial plateau and to assess variations in the offset of the tibial shaft from the tibial plateau of osteoarthritis (OA) patients, then analyzed the occurrence of mismatch of femoral component in total knee arthroplasty in China. The results showed that significant variations in the proximal tibial anatomy among the subjects. The mean offset was 7.61 mm ± 3.04 mm at the resection just distal to the subchondral bone. Anatomic variations should be fully evaluated before undergoing TKA. The mechanical stability of tibial fixation in primary TKA is significantly increased by using the long-stem tibial component with metallic tibial wedge knee prosthesis. Post-operative radiographs revealed that a higher prevalence of femoral component underhang than overhang in men and women. The current knee implants do not fulfill the requirements of their patient population in China. In addition, implant design should be sex-specific.
