Abstract/Summary:
Osteoarthritis (OA) is a kind of common disease happened to the elderly, which leads to joint pain, stiffness and dysfunction. X-ray often suggest the occurrence of inflammation of joint. It has high disability rate, and the incidence of women is higher than that of male, which impacts on patients’ normal life hardly. By gradually increased treatment costs has become in patients with a heavy burden of medical treatment. Osteoarthritis (OA) occurres in the weight-bearing joints, including knee joints which is called knee osteoarthritis (KOA).It Divides into primary and secondary. The etiology and pathogenesis of knee osteoarthritis (KOA) are not clear so far, in fact there is no effective measures to reverse hyperosteogeny or inhibit the progression of disease. The treatment includes non-drug therapy, drug therapy and surgical treatment. The first two can be classified as conservative treatment. This disease has a long course and recurs repeatedly. Drug therapy operates quickly, but it will cause common side effects and recurrence after withdrawing. Surgical treatment belongs to the traumatic treatment, and takes expensive, which the older patients are afraid. Based on this, safe and effective non-drug therapy is preferred in clinic. Acupuncture is a kind of special therapy of traditional Chinese medicine, which has a history of thousands of years. It has easy operation, satisfactory effect in the treatment of pain, and no toxic side effect. It is one of the effective conservative treatment to treat knee osteoarthritis. Based on this point, the study observed the clinical cases, through the evaluation of evidence-based medicine, to look for a safe and effective treatment.Objective:Random control test is choose in study. We observed the cases of knee osteoarthritis in early-middle period objectively and scientifically, which were treated with warm acupuncture and electro-acupuncture combined, through the evaluation of evidence-based medicine, in order to find new ideas and new method for clinical.Methods:This study selected 60 early metaphase cases of knee osteoarthritis, which from the clinical of First Affiliated Hospital of Guangzhou University of traditional Chinese medicine from October 2014 to February 2016. All the suitable patients were randomly assigned according to the sequence of treatment, for treatment group:control group=1:1. The treatment group took warm acupuncture and electro-acupuncture, and the control group took electro-acupuncture only. Main points:Inner Eye of the Knee(EX-LE4), Dubi(Outer Eye of the Knee), Xuehai, Liangqiu, Yinlingquan, Yanglingquan, Zusanli, which are in the affected side.Operation method:After groovy disinfection of the acupuncture points, angle of needle insertion of Inner Eye of the Knee(EX-LE4) and Dubi(Outer Eye of the Knee) were toward direction of knee joint, for the rest took perpendicular insertion 1.5 inch depth. The needling response (acid, hemp, bilge feeling) would be felt when pinking in specified depth in uniform reinforcing-reducing method. And then the needles were connected to the electric machine in the density wave (with a frequency of 16 ~ 8Hz). The inner Eye of the Knee(EX-LE4) and Dubi(Outer Eye of the Knee), Xuehai and Liangqiu, Yinlingquan and Yanglingquan were in pairs to power. Besides of this, the treatment group added the treatment of warm acupuncture. The operation method:2.5cm moxa stick were respectively sheathed on the needle handle, which kept 2-3cm from the skin and were separated by a piece of cardboard to prevent scalding. We took the medical tape to fix the needle with moxa stick on the skin, in order to prevent the moxa stick felling down and be scalded. The electricity were cut off when the Moxa stick were burned off, and we cleaned the ash, pulled the needle. The conduction time were about 30 minutes, and took treatment 3 times a week, once every two days. It took 2 weeks as a course of treatment, rest for 2 days between the course. The study observed 2 courses.Before and after the treatment and after treatment 3 months follow-up, we evaluated. We took four of evaluation scales, like visual analogue scale(VAS), Symptom grading of knee osteoarthritis rating scale, Lequensne MG and Western Ontario and McMaster University Osteoarthritis Index (WOMAC). Finally, the test data were took a statistical analysis by SPSS16.0. All the statistics were used two-sided test, it had significant difference (P<0.05).Results:Clinical effect:The effective power is 93.3% in treatment and 70% in control.3 successful cases,10 markedly effective cases,15 effective cases, and 2 failed cases in the treatment group, and the total effective number were 27 cases.1 successful cases,4 markedly effective cases,16 effective cases, and 9 failed cases in the control group, and the total effective number were 21 cases.The valuation of clinical effectiveness:After the end of treatment, the data of the treatment group and the control group in VAS score, Symptom grading of knee osteoarthritis rating scale, Lequensne MG and scale WOMAC scale has significant difference (P<0.05). The results showed that warm acupuncture and electro-acupuncture combined and electro-acupuncture only had curative effect on treating the knee osteoarthritis, which could reduce the pain, lessen the clinical symptoms and improve the patient’s quality of life. There is significant difference between the groups (P<0.05). The treatment group is more effective, and its short-term effect is good.After the treatment of 3 months later, the data of the treatment group and the control group in VAS score, Symptom grading of knee osteoarthritis rating scale, Lequensne MG scale and WOMAC scale has significant difference (P<0.05). There is significant difference between the groups(P<0.05). The treatment group is more effective, and its long-term effect is good.Conclusion:There is curative effect on knee osteoarthritis in early-middle period through treating by warm acupuncture and electro-acupuncture combined. The effective rate is 93.3%, and its short-term and long-term effect are good. The study shows that the therapy of acupuncture and electro-acupuncture combined to treat knee osteoarthritis could reduce the pain, lessen the clinical symptoms and improve the patient’s quality of life. It is an effective treatment that would be choose.
