Purpose:Chronic atrophic gastritis is a common and intractable disease in digestive diseases,acupoint catgut embedding therapy have a more prominent effect for treatment of the disease,but the specific mechanism of action is not very clear,so the embedding therapy in the treatment of chronic atrophic gastritis is difficult to promote.In the previous study,we have explored the mechanism of acupoint catgut embedding in improving chronic atrophic gastritis from the perspective of NF-kB signal transduction pathway.So we set up this subject,it is expected to further explore the mechanism of catgut embedding therapy in the treatment of chronic atrophic gastritis from the perspective of JAK-STAT signal transduction pathway through clinical and experimental research,we hope that catgut embedding therapy for chronic atrophic gastritis can be obtained by theoretical support,and we hope to provide a useful reference for clinical options for the treatment of chronic atrophic gastritis.Methods:60 patients with chronic atrophic gastritis were collected by the clinical study,all the standard cases were divided into two groups according to the random number table method.The catgut embedding group was the treatment group,while the body acupuncture group was the control group,with 30 cases in each group.The two groups had the same treatment points,treatment group and control group were obtained catgut embedding and body acupuncture treatment respectively,after the treatment,the evaluation of therapeutic efficacy was mainly based on the quantification table that attached to TCM symptom classification scale,changes on endoscopy and pathological,SF-36 quality of life table.80 adult male SD rats were fed for 1 week,and then they were divided into blank group(n = 15)and model group(n = 65).The blank group was fed with ordinary grade feed combined with distilled water,the model group was fed with 100 μg/ml N-methyl-N’-nitro-N-nitrosoguanidine(MNNG)solution,they can freely access the water,but they can only eat every other day.Successful CAG models were based primarily on gastroscopy and pathology,the model was copied successfully in the 22nd week,at this time,48 rats were left into the model group and 15 rats in the blank group.48 rats were randomly divided into three groups:the catgut embedding group,the body acupuncture group and the model group,with 16 cases in each group,at that moment,all 4 groups rats were fed with ordinary grade feed combined with distilled water.The catgut embedding group and body acupuncture group were treated with the intervention of embedding and body acupuncture,the rats in the model group and the blank group did not take any treatment,but with the same intensity of gripping,it lasted until the end of the experiment.At the end of the trial,all the rats were sacrificed,the gastric mucosa and blood samples were taken from the rats,they were tested and analyzed.Result:Symptoms of Chinese medicine:before treatment,there was no statistically significant difference between the two groups(P>0.05);after treatment,the difference between the two groups was statistically significant(P<0.05)、After treatment,in the treatment group,the difference of symptom score before and after treatment was statistically significant(P<0.05);after treatment,in the control group,the symptom score difference of most symptoms before and after treatment was statistically significant(P<0.05),but there was no statistically significant difference in the symptoms of Qi-deficiency and laziness(P>0.05).Observation on the Tongue Picture and Pulse:after treatment,the overall improvement rate of the tongue picture and tongue in the catgut embedding group was greater than that in the acupuncture group.Endoscopy and pathology:after treatment,the difference of the gastroscopic grades between the two groups was statistically significant(P<0.05);after treatment,the difference of gastric mucosal inflammation grading between the two groups was statistically significant(P<0.05);after treatment,There was no statistically significant difference in gastric gastric gland atrophy,intestinal metaplasia and dysplasia(P>0.05).SF-36 quality of life:after treatment,the difference between the two groups was statistically significant(P<0.05).Rat model replication:the CAG rat model was copied successfully at week 22.Light microscopy observation of gastric mucosa in rats:after treatment,the gastric mucosa of the rats in the two groups was improved,and the degree of improvement of the catgut embedding group was greater than that of the acupuncture group.There was no significant change in the gastric mucosa before and after treatment in the blank group and the model group.The expression of IL-6,EGF,VEGF and TNF-α in rat serum:there was no significant difference in the expression of serum IL-6,EGF,VEGF and TNF-a between the catgut embedding group and the blank group(P>0.05);the catgut embedding group,the acupuncture group and the model group,three groups were compared with each other,the difference of expression of serum IL-6,EGF,VEGF and TNF-α among the three groups was statistically significant(P<0.05);and expression of serum IL-6,EGF,VEGF and TNF-α was consistent with each other.The expression intensity order of serum IL-6,EGF,VEGF and TNF-a from weak to strong were as follows:blank group,catgut embedding group,acupuncture group and model group.Proteins expression of JAK2 and STAT3 in rat gastric mucosa:there was no significant difference in protein expression of JAK2 and STAT3 between the catgut embedding group and the blank group(P>0.05);the catgut embedding group,the acupuncture group and the model group,three groups were compared with each other,the difference of protein expression of JAK2 and STAT3 among the three groups was statistically significant(P<0.05);protein expression of was consistent,and expression intensity order of JAK2 and STAT3 from weak to strong were as follows:catgut embedding group,acupuncture group and model group.Proteins expression of SOCS3 in Rat gastric mucosal:the catgut embedding group,the acupuncture group and the model group,three groups were compared with each other,the difference of proteins expression of SOCS3 among the three groups was statistically significant(P<0.05);protein expression intensity order of SOCS3 from weak to strong were as follows:blank group,model group,acupuncture group and catgut embedding group.Conclusion:Therapy of catgut embedding and body acupuncture can effectively improve the clinical symptoms of patients with chronic atrophic gastritis,can alleviate the inflammation of gastric mucosa,also the quality of life of the patient can be improved,But the embedding treatment of chronic atrophic gastritis relative to the needle is more advantages.Therapy of catgut embedding have more advantages in the treatment of chronic atrophic gastritis compared with therapy of body acupuncture.According to the subject of modeling methods,the proportion of successful modeling is relatively high in the 22nd week.Experimental related indicators test results suggest:IL-6,EGF,VEGF,TNF-α,JAK2,STAT3 and SOCS3 play a major role in chronic atrophic gastritis,the activation of JAK-STAT transduction system is an important pathway that mediates the development and progression of chronic atrophic gastritis,STAT3 is the key protein for pathway activation,SOCS3 is an important negative feedback factor for JAK-STAT transduction system.Highly expressed SOCS3 can inhibit the abnormal activation of JAK-STAT transduction system;Chronic atrophic gastritis cannot self-healing in the short term.The mechanism that chronic atrophic gastritis treated with catgut embedding may have two aspects:On the one hand,therapy of catgut embedding may stimulate the central nervous system,which could promote or inhibit theexpression of specific proteins through the nerve-body fluid reaction,and further inhibit the abnormal activation of the JAK2/STAT3 transduction system;On the other hand,catgut as an allogeneic protein,can induce the body to produce allergic immune response,which can also inhibit the abnormal activation of JAK2/STAT3 transduction system.
