BackgroundDetrusor smooth muscle(DSM)dysfunctions could result in urinary retention(UR).The clinical trials have confirmed the effects of electro-acupuncture(EA)Zhongliao(BL33)acupoint on treating with UR induced by DSM dysfunctions.These clinical reports suggested that EA BL33 may have effects on exciting DSM contraction.In fact,the acupuncture effects closely related to the specific acupoint,depth of acupuncture,and EA parameters.Currently,mechanisms on EA exciting DSM contractions,influence factors are inadequate and there has no certain conclusion.Purpose(1)To assess the effect of EA BL33 exciting on DSM contractions and possible influence factors;(2)To explore the influences of EA BL33 on modulating sacral spinal cord discharging,pelvic efferent signals and DSM discharging activities;(3)To explain the possible effects' mechanisms of EA BL33 at DSM contractions.Methods and ContentsEighty male Sprague-Dawley(SD)rats were anesthetised with 10%urethane and modelled by urethra outlet obstruction.The experiment choose electrophysiology methods to record intravesical pressure(IVP)/DSM signal,IVP/Pelvic nerve signal or IVP/SPN signal on UR rats simultaneously.We used Spike 2.0 software to analyze the bioeletrical signal.(1)DSM contractions:?To compare different effects among Hegu(LI4),Zhongliao(BL33)and Sanyinjiao(SP6)acupoint on modulating DSM contractions in UR rats;?To compare different effects among EA BL33 with deep,shallow,or lateral needling on modulating DSM contractions in UR rats;?To compare different effects among 0.1mA,0.5mA and 1mA EA intensity on modulating DSM contractions in UR rats;? To compare different effects among MA,2Hz and 100Hz EA frequency on modulating DSM contractions in UR rats;(2)DSM excitement:?To compare different effects among EA Hegu(LI4),Zhongliao(BL33)and Sanyinjiao(SP6)acupoint on DSM excitement:?To compare different effects among deep,shallow,or lateral needling on modulating DSM excitement;? To compare different effects among MA,2Hz and 100Hz EA frequency on modulating DSM excitement;(3)To compare different effects among EA Hegu(LI4),Zhongliao(BL33)and Sanyinjiao(SP6)acupoint on modulating pelvic efferent signals and to explore the influences of EA BL33 on pelvic efferent signals;To explore different effects among MA,2Hz and 100Hz EA frequency on modulating pelvic efferent signals;(4)To observe the influences of EA Zhongliao(BL33)acupoint on sacral nerve excitement in UR rats.Results(1)DSM contractions:For normal rats:deep inserted Zhongliao(BL33,1mA,2Hz/100Hz EA or MA)could prolong the bladder contraction time.It has no function of modulating the maximal pressure,micturition interval,threshold pressure and residual pressure.The results suggested that no significant was shown on EA at Hegu(LI4)or Sanyinjiao(SP6)with other depth,intensities or modalities;For AUR rats:?EA at Zhongliao(BL33);Sanyinjiao(SP6)could prolong the bladder contraction time,shorten the micturition intervals and decrease the residual pressure(P<0.05)while Hegu(LI4)had no function on that.Compared with Sanyinjiao(SP6),Zhongliao(BL33)showed better effects on increasing bladder contraction time;? deep or shallow inserted in Zhongliao(BL33)could prolong the bladder contraction time,shorten the micturition intervals and decrease residual pressure(P<0.05).Lateral needling could reduce the residual pressure(P=0.048).Deep needling was proved to be better than lateral needling and it's better than shallow needling on prolonging bladder contraction;? 0.5mA and 1mA intensities could prolong the bladder contraction time,shorten the micturition intervals and decrease the residual pressure(P<0.05).EA with 1mA intensity was better than 0.1mA on shortening the micturition intervals and reducing residual pressure(P<0.05).? MA,EA 2Hz/100Hz up-regulated DSM contractions while no difference among them;(2)DSM excitement:For normal rats:EA Zhongliao(BL33,deep insertion,1mA)could enhance the DSM discharging frequency,prolong the duration of discharging;MA Zhongliao(BL33)could enhance the frequency while had no function on duration;For AUR rats:?EA Zhongliao(BL33)could enhance DSM discharging and was superior to Hegu(LI4)and Sanyinjiao(SP6)(P<0.05);? Deep and shallow needling could enhance DSM functions while lateral needling had no function on that.Compared with shallow needling,deep needling showed better effects on frequency;?All intensities could enhance DSM functions while 1mA was proved to be better than 0.1mA;? All modalities could enhance DSM functions while no difference among them was shown.(3)Pelvic efferent signal:For normal rats:deep inserted Zhongliao(BL33,MA,EA 2Hz/100Hz)could enhance pelvic efferent signal conduction and significant difference was shown before and after acupuncture.2 Hz EA at Sanyinjiao(SP6)could prolong the discharging time(P<0.05).For AUR rats:MA,EA 2Hz/100Hz at Zhongliao(BL33)or Sanyinjiao(SP6)could enhance pelvic nerve discharging frequency,and Zhongliao(BL33)was superior to Sanyinjiao(SP6)and Hegu(LI4).Zhongliao(BL33)with different modalities could prolong the discharging time and it was better than Hegu(LI4)in 2Hz frequency.(4)Sacral spinal cord discharging:For normal rats:EA at Zhongliao(BL33)could enhance the discharging frequency while it had no function on discharging duration.Hegu(LI4)had no regulations on that;For AUR rats:Zhongliao(BL33)could enhance the discharging and it's superior to Hegu(LI4)on improving frequency.Summary1.For AUR rats,EA Zhongliao(BL33)enhanced DSM contractions which manifested at prolonging the bladder contraction,shortening the micturition intervals and reducing the residual pressure.It has better effects than Hegu(LI4)or Sanyinjiao(SP6);the deep needling effects were superior to shallow or lateral needling;1mA EA intensity was superior to 0.5mA or 0.1mA;there was no significant difference among EA frequencies at BL33 while 2Hz had superior tendency on that.2.EA Zhongliao(BL33)excited DSM which manifested at adding discharging frequency,prolonging discharging time.It has better effects than Hegu(LI4)or Sanyinjiao(SP6);the deep needling effects were superior to shallow or lateral needling;1mA EA intensity was superior to 0.5mA or 0.1mA;No difference was shown among 2Hz EA,100Hz EA or MA;3.EA Zhongliao(BL33)enhanced the pelvic efferent signal conduction.No difference was shown among 2Hz EA,100Hz EA or MA;4.EA Zhongliao(BL33)enhanced the sacral spinal cord discharging and its effects were better than Hegu(LI4).ConclusionEA BL33 could excite DSM discharging and strengthen DSM contractions;the possible mechanism is because EA BL33 excites the sacral nerve discharging and enhances the pelvic efferent signal conduction therefore enhanced DSM contractions.
